Inspectors also noted no consumer advisory for raw or undercooked foods, meaning customers with compromised immune systems had no warning on the menu.
Culhane's Irish Pub at 9720 Deer Lake Court also drew eleven high-severity violations. Inspectors found no person in charge present or performing duties, no employee health policy, an employee not reporting illness symptoms, improper handwashing technique, inadequate shellfish records, parasite destruction procedures not followed, food contact surfaces not properly cleaned, and time not properly used as a public health control.
Seafood Handling Failures Across the City
Tony Pepperoni at 2158 Mayport Road drew ten high-severity violations, among them food contaminated by a chemical, physical, or biological hazard, food from an unapproved source, parasite destruction procedures not followed, and food contact surfaces not properly cleaned. The facility also had no consumer advisory for raw or undercooked foods.
Southern Coast Seafood at 1301 Monument Road accumulated ten high-severity violations, including no person in charge, no employee health policy, inadequate handwashing facilities, improper handwashing technique, food contact surfaces not properly sanitized, time as a public health control not properly used, and no consumer advisory for raw or undercooked items.
Mestizo Mex Cocina at 5907 Roosevelt Boulevard drew nine high-severity violations, including no person in charge, no employee health policy, an employee not reporting illness, food in poor condition, inadequate shellfish records, unclean food contact surfaces, and food not cooked to required minimum temperature.
Eight high-severity violations were documented at Bangkok Thai Cuisine at 7159 Philips Highway, including food from an unapproved source, inadequate shellfish records, food contact surfaces not properly cleaned, food not cooked to required minimum temperature, and no consumer advisory.
Charley's Philly Steaks at 8808 Beach Boulevard drew eight high-severity violations, including no person in charge, food in poor condition, parasite destruction procedures not followed, toxic chemicals improperly stored or labeled, and no allergen awareness demonstrated.
Cracker Barrel #341 at 4680 Lenoir Avenue also drew eight high-severity violations: no person in charge, inadequate handwashing by food employees, food from an unapproved source, inadequate shellfish records, parasite destruction procedures not followed, toxic chemicals improperly stored, and no allergen awareness demonstrated.
The Sheraton Jacksonville Hotel at 10605 Deerwood Park Boulevard drew eight high-severity violations, among them food from an unapproved source, food not cooked to required minimum temperature, toxic chemicals improperly stored, toxic substances improperly identified or used, and no allergen awareness demonstrated.
Hot Wok at 6765 Dunn Avenue drew eight high-severity violations, including inadequate handwashing facilities, food in poor condition, inadequate shellfish records, food contact surfaces not properly cleaned, and toxic chemicals improperly stored.

What These Violations Mean
The most common single violation across this week's inspections was the failure to report employee illness symptoms, cited at Pho Today, Mr Snappers, Culhane's Irish Pub, Tony Pepperoni, Southern Coast Seafood, Mestizo Mex Cocina, Charleys Philly Steaks, Bodrum Mediterranean Kitchen, and Beignets Caribbean Cafe. Food workers who continue working through vomiting or diarrhea are the primary driver of multi-victim Norovirus outbreaks. Norovirus spreads through direct contact with contaminated food, and a single ill worker can expose hundreds of customers in a single shift.
Shellfish traceability failures appeared at ten of the fifteen facilities, including Pho Today, Mr Snappers, Culhane's Irish Pub, Tony Pepperoni, Mestizo Mex Cocina, Bangkok Thai Cuisine, Charleys Philly Steaks, Cracker Barrel, Hot Wok, and Queens Harbour Yacht and Country Club. Charleys Philly Steaks has only 2 prior inspections on record, making it one of the newest facilities in this week's data. It still drew eight high-severity violations, including toxic chemicals improperly stored and no allergen awareness demonstrated. Mr Snappers, with only 6 prior inspections, drew eleven high-severity violations including parasite destruction failures and unclean food contact surfaces. Bangkok Thai Cuisine has 10 prior inspections and drew eight high-severity violations including food not cooked to minimum temperature and no consumer advisory for raw items. Three of the four facilities with the fewest inspections on record accumulated some of the highest violation counts this week.
Flavorikan, the facility with the most inspections on record at 41, was emergency-closed for roach activity on September 2. Its formal violation tally this week was one intermediate citation. Whether the roach activity that triggered the closure had appeared in any of those prior 41 inspection reports is not reflected in the data available.
