Pho Today's 11 high-severity violations included no written employee health policy, an employee not reporting illness symptoms, inadequate handwashing facilities, improper handwashing technique, food from an unapproved source, food in poor condition or adulterated, inadequate shellfish identification records, and food not cooked to the required minimum temperature. That last violation, undercooking, means Salmonella in poultry can survive and reach a customer's plate.

Miller's Jacksonville Ale House, also on Deer Lake Court, matched Pho Today's 11 high-severity count. Mr. Snappers on 103rd Street drew the same 11 high-severity total. Culhane's Irish Pub on Deer Lake Court also reached 11 high-severity violations, including a finding that no person in charge was present or performing duties. Inspectors also cited the pub for parasite destruction procedures not followed, improperly sanitized food contact surfaces, and a failure to properly use time as a public health control, meaning food was left in the temperature danger zone without adequate tracking.

Three restaurants share Deer Lake Court addresses. Pho Today, Miller's Ale House, and Culhane's Irish Pub were all cited in the same inspection week, each accumulating 11 high-severity violations.

Tony Pepperoni on Mayport Road drew 10 high-severity violations, including food contaminated by chemical, physical, or biological hazards, food from an unapproved source, parasite destruction not followed, unsanitized food contact surfaces, and no consumer advisory for raw foods.

Charley's Philly Steaks on Beach Boulevard drew eight high-severity violations, including toxic chemicals improperly stored or labeled and no allergen awareness demonstrated. Inspectors also cited the restaurant for food in poor condition, inadequate shellfish records, and parasite destruction not followed.

Sheraton Jacksonville Hotel on Deerwood Park Boulevard received eight high-severity violations, among them toxic chemicals improperly stored or labeled, toxic substances improperly identified or used, no allergen awareness demonstrated, food from an unapproved source, and food not cooked to the required minimum temperature.

Cracker Barrel No. 341 on Lenoir Avenue also drew eight high-severity violations, including no person in charge, inadequate handwashing by food employees, food from an unapproved source, toxic chemicals improperly stored, and no allergen awareness demonstrated.

Bodrum Mediterranean Kitchen on Roosevelt Boulevard accumulated eight high-severity violations, including no person in charge, inadequate handwashing facilities, inadequate handwashing by food employees, improper handwashing technique, food from an unapproved source, food in poor condition or adulterated, and no consumer advisory for raw foods.

Beignets Caribbean Cafe on Barnes Road drew a single high-severity violation: an employee not reporting symptoms of illness.

What These Violations Mean

The most widespread violation this week was the failure to report illness symptoms, cited at Pho Today, Miller's Ale House, Mr. Snappers, Culhane's Irish Pub, Tony Pepperoni, Southern Coast Seafood, Mestizo Mex Cocina, Bangkok Thai Cuisine, Charley's Philly Steaks, Bodrum Mediterranean Kitchen, and Beignets Caribbean Cafe. Food workers shedding Norovirus are the documented source of the majority of multi-victim restaurant outbreaks. A single infected employee who continues working can contaminate hundreds of meals before anyone knows there is a problem.

The shellfish traceability violation appeared at Pho Today, Miller's Ale House, Mr. Snappers, Culhane's Irish Pub, Tony Pepperoni, Mestizo Mex Cocina, Bangkok Thai Cuisine, Charley's Philly Steaks, Cracker Barrel No. 341, and Southern Coast Seafood. Oysters, clams, and mussels are frequently consumed raw or lightly cooked. Without harvest tags and proper records, there is no way to trace a contaminated batch back to its source if customers fall ill.

Sichuan Family on Old St. Augustine Road has 29 prior inspections and drew only a single intermediate violation this week. It and Flavorikan are the two facilities in this dataset with the longest inspection histories and the lightest findings this week.

The Sheraton Jacksonville Hotel on Deerwood Park Boulevard, with 32 prior inspections, drew eight high-severity violations including two separate chemical handling citations: toxic chemicals improperly stored and toxic substances improperly identified or used. Whether those two findings reflect the same storage area or separate problems in the hotel kitchen is not specified in the inspection record.
