JACKSONVILLE, FL. Inspectors flagged Salsas Mexican Restaurant on West Beaver Street for 13 high-severity violations during the week of July 31, the highest single-facility tally among 15 Jacksonville restaurants cited for serious food safety failures in seven days.

The violations at Salsas included food sourced from unapproved or unknown suppliers, food in poor condition or adulterated, inadequate handwashing by employees, improper handwashing technique, no employee health policy, no employee illness reporting, inadequate handwashing facilities, and no person in charge present or performing duties. That last violation, the absence of active managerial control, appeared across seven of the 15 facilities cited this week.

Two restaurants did not make it through the day. Cruz Mexican Grille at 13423 Beach Blvd and Mod Pizza at 12547 Bartram Park Blvd were both emergency-closed on July 31, Cruz for roach activity and Mod Pizza for fly activity.

Thai Spice Kitchen on Bowden Road drew 12 high-severity citations, including inadequate shell stock identification records and failure to follow parasite destruction procedures for fish. Both violations are specific to raw seafood handling and appeared repeatedly across Jacksonville's sushi and Japanese restaurants this week.

Shibuya Sushi on Crosshill Boulevard was cited for 11 high-severity violations, including food contact surfaces not properly cleaned or sanitized, failure to follow parasite destruction procedures, inadequate shell stock records, and improper use of time as a public health control. No person in charge was present or performing duties during the inspection.

Nagasaki Japanese Restaurant on Yellow Bluff Road collected 10 high-severity violations, among them food from unapproved sources, inadequate shell stock records, parasite destruction failures, and improperly cleaned food contact surfaces.

The Popeyes franchise at 4108 S University Blvd, Popeyes Louisiana Kitchen #162, was cited for food not cooked to the required minimum temperature, a direct Salmonella risk in a poultry-focused kitchen. The same location also had no person in charge, no employee health policy, no illness reporting by employees, inadequate handwashing, food in poor condition, improperly cleaned food contact surfaces, and no consumer advisory for raw or undercooked foods.

Blue Boy Sandwich Shop on Norwood Avenue drew nine high-severity citations including food from unapproved sources, food not cooked to required temperature, improperly stored or labeled toxic chemicals, and no consumer advisory. That combination, untraced ingredients plus undercooked food plus chemical storage failures, is among the more dangerous violation clusters in this week's data.

Mudville Grille on Beach Boulevard was cited for eight high-severity violations including inadequate handwashing facilities, improperly stored toxic chemicals, food in poor condition, and improper use of time as a public health control. Pour Taproom on North Laura Street was also cited for eight high-severity violations, including no allergen awareness demonstrated, inadequate shell stock records, improperly stored toxic substances, and no consumer advisory.

Whiskey Cake on Rivercoast Drive drew eight high-severity violations including food from unapproved sources, parasite destruction failures, improperly cleaned food contact surfaces, improperly stored chemicals, and no consumer advisory. Directly nearby, Wasabi Japanese Steakhouse on Rivercoast Drive was cited for seven high-severity violations including no person in charge, inadequate handwashing facilities, improperly stored chemicals, inadequate shell stock records, and no consumer advisory.

Sandwich House on Beach Boulevard was cited for eight high-severity violations including food from unapproved sources, no allergen awareness, improper use of time as a public health control, and improperly cleaned food contact surfaces.

Three Popeyes locations appeared in this week's data. Popeyes Louisiana Kitchen 196 on Duval Station Road was cited for seven high-severity violations including food from unapproved sources, parasite destruction failures, improperly stored chemicals, and improper sewage or wastewater disposal. Popeyes Louisiana Kitchen #151 on Dunn Avenue was cited for seven high-severity violations including food not cooked to required temperature, no person in charge, improperly stored chemicals, improperly identified toxic substances, and improper sewage disposal.

Sushi House on River Marsh Drive drew seven high-severity and seven intermediate violations, the highest intermediate count of any facility this week. Among the high-severity findings: parasite destruction failures, inadequate shell stock records, improperly stored chemicals, and improperly cleaned food contact surfaces. The intermediate list included improper sewage or wastewater disposal.

Pig Seafood on Lem Turner Road drew the fewest high-severity violations of any facility in this week's roundup, two: no person in charge and no employee illness reporting.

What These Violations Mean

The food-from-unapproved-sources violation, cited at Salsas Mexican, Thai Spice Kitchen, Nagasaki Japanese, Blue Boy Sandwich Shop, Sandwich House, Whiskey Cake, and Popeyes 196, is not a paperwork technicality. When ingredients bypass USDA or FDA inspection, there is no supply chain record to trace if customers get sick. A Listeria outbreak linked to uninspected meat or produce can take weeks to identify, and by then dozens of people may be affected.

Parasite destruction failures at Thai Spice Kitchen, Shibuya Sushi, Nagasaki Japanese, Whiskey Cake, Sushi House, and Popeyes 196 carry a specific risk for raw fish restaurants. Parasites including Anisakis, found in raw salmon and tuna, survive if fish is not frozen to the correct temperature for the required duration before serving. Customers who eat sushi or sashimi from a kitchen that skips this step have no way of knowing the fish was not treated.

The employee illness and health policy violations, which appeared at nearly every facility cited this week, form a direct transmission pathway. Norovirus, the leading cause of foodborne illness outbreaks in restaurants, spreads person-to-person and through food handled by infected workers. A written health policy is the mechanism that keeps a sick cook off the line. Without one, the decision is left to chance.

Sewage and wastewater disposal violations at Sushi House, Popeyes 196, and Popeyes #151 represent a different category of contamination. Improper disposal creates the possibility of fecal bacteria reaching food preparation surfaces, a route that bypasses every other safety control a kitchen might have in place.

The Longer Record

Wasabi Japanese Steakhouse has 52 prior inspections on record, the highest count of any facility in this week's data, and this week added seven more high-severity violations to that history. Shibuya Sushi on Crosshill Boulevard follows with 39 prior inspections and an 11-violation week that included parasite destruction failures and no manager present.

Pig Seafood on Lem Turner Road has 34 prior inspections and drew only two high-severity violations this week. But those two, no person in charge and no illness reporting, are precisely the management-layer failures that, across 34 inspections, suggest the problem is structural rather than situational.

Popeyes Louisiana Kitchen #162 on University Boulevard has 32 prior inspections, and this week's nine high-severity violations included undercooked food, no manager, and no illness reporting. The Dunn Avenue Popeyes (#151) has 22 prior inspections and also cited for undercooked food and sewage disposal failures this week.

Pour Taproom on North Laura Street has only four prior inspections on record, the fewest of any facility in this week's roundup. Eight high-severity violations in what is still an early inspection history, including no allergen awareness and improperly stored toxic substances, is a steep start.

Salsas Mexican Restaurant, the week's most-cited facility with 13 high-severity violations, has 17 prior inspections. Among those violations was the absence of a person in charge during the inspection itself, the same management failure that state data shows correlates with higher overall violation counts across an establishment's history.
