TAMPA, FL. Salem's Fresh Eats on North 50th Street was ordered closed on August 18, the first day of the inspection week, after inspectors found roach activity inside the restaurant. It would not be the last closure.
A second emergency shutdown followed three days later at Sushi Masa Premium on Palm Pointe Drive, also for roach activity, on August 21. Across the same seven days, state inspectors documented high-severity violations at 15 Tampa-area food establishments, ranging from unapproved food sources to employees not reporting illness symptoms to a restaurant serving shellfish with no traceability records on file.

VIOLATIONS CITED AT CLOSURE SITES
Salem's Fresh Eats had accumulated 30 prior inspections before this week's closure. The two high-severity violations documented alongside the roach finding included food contact surfaces not properly cleaned or sanitized and no consumer advisory posted for raw or undercooked foods. Sushi Masa Premium's closure record did not include a broader violation count in this week's data.
The Worst of the Week
Maikels Kitchen on Waters Avenue led all facilities this week with 10 high-severity violations and 7 intermediate citations. Inspectors found the person in charge was not present or not performing duties, no employee health policy in place, an employee not reporting illness symptoms, inadequate handwashing facilities, improper handwashing technique, food from an unapproved or unknown source, inadequate shellfish traceability records, and food contact surfaces that had not been properly cleaned or sanitized. Every layer of food safety oversight, from management to sourcing to hygiene infrastructure, failed in a single inspection.
Luis's Garden Grille on West Hillsborough Avenue drew 8 high-severity violations. Inspectors cited food not cooked to the required minimum temperature, toxic chemicals improperly stored or labeled, toxic substances improperly identified or used, and no allergen awareness demonstrated. The allergen citation is particularly acute: with 32 million Americans living with food allergies, a kitchen that cannot demonstrate basic allergen awareness puts customers with severe reactions at direct risk.
Yummy House China Bistro on East Hillsborough Avenue drew 7 high-severity violations. Among them: no approved potable water supply, food not cooked to the required minimum temperature, inadequate shellfish traceability records, no consumer advisory for raw or undercooked foods, and required procedures for specialized processes not followed. A restaurant serving shellfish and operating specialized cooking methods without a verified potable water source represents compounding risk on a single menu.
Riverside Seafood Tampa on East Fletcher Avenue also drew 7 high-severity violations, including food from an unapproved or unknown source, inadequate handwashing facilities, no consumer advisory for raw or undercooked foods, and required procedures for specialized processes not followed. A seafood restaurant sourcing from unverified suppliers has no traceability if a customer gets sick.
LongHorn Steakhouse on East Fowler Avenue drew 7 high-severity violations, including food contaminated by chemical, physical, or biological hazards, food from an unapproved or unknown source, inadequate shellfish traceability records, time as a public health control not properly used, and no consumer advisory for raw or undercooked foods. The contamination citation is among the most serious in this week's data: food that has already been adulterated by a chemical or physical hazard cannot be made safe by cooking.
La Cabana Restaurant on North Armenia Avenue drew 5 high-severity violations, including an employee not reporting illness symptoms, improper handwashing technique, food from an unapproved or unknown source, inadequate shellfish traceability records, and no consumer advisory for raw or undercooked foods. Five intermediate violations accompanied those findings, including inadequate cooling equipment and improper sanitizing procedures.
California Pizza Kitchen at WestShore Plaza drew 5 high-severity violations: no person in charge performing duties, an employee not reporting illness symptoms, food not cooked to the required minimum temperature, time as a public health control not properly used, and no consumer advisory for raw or undercooked foods.
Saki Endless Sushi and Hibachi Restaurant on North Dale Mabry Highway drew 5 high-severity violations, including improper handwashing technique, food contact surfaces not properly cleaned or sanitized, no consumer advisory for raw or undercooked foods, and toxic chemicals improperly stored or labeled.
Harborview Cafe on North Rocky Point Drive drew 5 high-severity violations, including inadequate shellfish traceability records, parasite destruction procedures not followed, food contact surfaces not properly cleaned or sanitized, and no consumer advisory for raw or undercooked foods. A waterfront cafe that cannot document parasite destruction protocols for fish is serving raw or lightly prepared seafood without the safety backstop that freezing or proper cooking is supposed to provide.
Baan Tawan Sushi on South Dale Mabry Highway drew 5 high-severity violations, including food from an unapproved or unknown source, food in poor condition or adulterated, inadequate shellfish traceability records, and food contact surfaces not properly cleaned or sanitized.
Tampa Midtown Aloft and Element Floor 6 on Midtown Drive drew 5 high-severity violations, including inadequate shellfish traceability records, parasite destruction procedures not followed, food not cooked to the required minimum temperature, and no allergen awareness demonstrated.
China 1 on North Dale Mabry Highway drew 5 high-severity violations: no employee health policy, an employee not reporting illness symptoms, improper handwashing technique, parasite destruction procedures not followed, and food contact surfaces not properly cleaned or sanitized.
Thinh An Kitchen and Tofu on West Waters Avenue drew 3 high-severity violations, including no employee health policy, an employee not reporting illness symptoms, and no consumer advisory for raw or undercooked foods.
Mi Pueblito Bakery and Restaurant on North Armenia Avenue drew 1 high-severity violation for toxic substances improperly identified, stored, or used, alongside 2 intermediate citations for inadequate cooling equipment and inadequate ventilation.
What These Violations Mean
The employee illness reporting failures documented this week at Maikels Kitchen, La Cabana Restaurant, California Pizza Kitchen, Thinh An Kitchen and Tofu, and China 1 are not paperwork problems. Food workers who do not report illness symptoms are the primary driver of multi-victim outbreaks. Norovirus, which can incapacitate a customer within 12 to 48 hours of exposure, spreads through direct hand-to-food contact. A kitchen without a written health policy has no mechanism to remove a sick worker before the shift begins.
The food-from-unapproved-sources citations at Maikels Kitchen, Riverside Seafood, LongHorn Steakhouse, La Cabana Restaurant, and Baan Tawan Sushi carry a specific consequence that goes beyond the meal being served: if a customer gets sick, investigators cannot trace the ingredient back to a farm, distributor, or processing facility. USDA and FDA inspections exist precisely to create that chain of accountability. Food that bypasses those inspections bypasses the only early-warning system that can stop a contaminated batch from reaching additional restaurants.
Shellfish traceability failures appeared at six facilities this week: Maikels Kitchen, Yummy House China Bistro, LongHorn Steakhouse, La Cabana Restaurant, Riverside Seafood, Harborview Cafe, Baan Tawan Sushi, and Tampa Midtown Aloft. Oysters, clams, and mussels are frequently consumed raw or minimally cooked, meaning any pathogen present in the shellfish at harvest survives to the plate. The tags and records that restaurants are required to maintain exist so that a Vibrio or Hepatitis A outbreak can be traced to a specific harvest date and harvest bed within hours. Without those records, the trail goes cold.
Parasite destruction failures at Harborview Cafe, Tampa Midtown Aloft, and China 1 mean that fish served raw or undercooked, including sushi and sashimi preparations, may not have gone through the required freezing protocol that kills parasites like Anisakis. These are not visible to the customer and cannot be identified by smell or appearance.
The Longer Record
Yummy House China Bistro has the longest inspection history of any facility cited this week, at 48 prior inspections, and still drew 7 high-severity violations including a non-potable water supply and no shellfish traceability records. Thinh An Kitchen and Tofu, with 47 prior inspections on record, drew violations this week for no employee health policy and an employee not reporting illness symptoms, categories that require no equipment and no capital investment to correct.
Saki Endless Sushi and Hibachi has 44 prior inspections and drew citations this week for toxic chemicals improperly stored near food alongside improper handwashing technique. La Cabana Restaurant, with 38 inspections on record, was cited for food from an unapproved source and inadequate cooling equipment, two categories that have appeared in the histories of long-inspected facilities across this week's data.
The newest entry in this week's record is Baan Tawan Sushi, with only 2 prior inspections on file. It drew 5 high-severity violations, including food in poor condition or adulterated and food from an unapproved source. A restaurant that accumulates that citation load within its first two inspections has not established basic sourcing controls from the start.
Riverside Seafood Tampa has only 9 prior inspections and drew 7 high-severity violations this week, including both unapproved food sourcing and no consumer advisory for raw seafood. Whether those violations persist into a follow-up inspection has not yet been determined.
