Shogun Sushi on North Dale Mabry Highway racked up 13 high-severity violations in a single inspection during the week of August 2, including food from an unapproved or unknown source, inadequate shellfish traceability records, and food contact surfaces that inspectors determined were not properly cleaned or sanitized.

The sushi restaurant also had no written employee health policy and no system for employees to report illness symptoms, two conditions that state health data consistently links to multi-victim outbreaks. Inspectors further cited inadequate handwashing facilities and improper handwashing technique, meaning the physical infrastructure for hygiene was deficient and the technique being used was also wrong.

That combination, 13 high-severity violations across food sourcing, hygiene infrastructure, and illness reporting, made Shogun Sushi the week's most-cited facility among the 15 Tampa restaurants flagged for serious violations.

China Buffet on East Fowler Avenue drew 12 high-severity violations, second only to Shogun Sushi. Inspectors also noted improper sewage or wastewater disposal as an intermediate violation, a finding that carries its own serious contamination risk.

Mi Bandera Bar and Restaurant on West Hillsborough Avenue received six high-severity violations as well, including toxic chemicals improperly stored, no consumer advisory for raw or undercooked foods, and inadequate shellfish identification records. Inspectors also cited improper sewage or wastewater disposal and inadequate toilet facilities as intermediate violations.

Six high-severity violations at a downtown Tampa location that draws a lunch crowd from nearby office buildings.

Mangroves Grill on Spectrum Boulevard was cited for six high-severity violations including time as a public health control not properly used, no consumer advisory for raw or undercooked foods, and required procedures for specialized processes not followed. The specialized process violation is notable: when smoking, curing, fermenting, or using reduced-oxygen packaging, facilities must follow precise written protocols or pathogens can survive at levels that cause illness.

Foody's Restaurant on West Hillsborough Avenue was cited for six high-severity violations, all without a single intermediate violation, including missing shellfish records, unsanitized food contact surfaces, and no consumer advisory.

Palm River Pizza on Palm River Road had food from an unapproved source among its five high-severity violations, alongside inadequate handwashing, improper handwashing technique, employee illness reporting failures, and no consumer advisory.

Hot Rocks Pizza on Memorial Highway was cited for five high-severity violations including toxic chemicals improperly stored, missing shellfish records, and no consumer advisory. Culver's of Carrollwood on North Dale Mabry also received five high-severity violations, including inadequate handwashing by employees, improper technique, unsanitized food contact surfaces, and no consumer advisory.

Fryer House on North Nebraska Avenue drew two high-severity violations, the lowest count among the 15 flagged facilities, but one of those was unsanitized food contact surfaces and the other was no consumer advisory for raw or undercooked foods. Norky's Pollos a la Brasa on Hanley Road received a single high-severity violation, no person in charge present or performing duties, which inspectors and CDC data both treat as a predictor of cascading failures across other categories.

What These Violations Mean

The most common high-severity violation across this week's inspections was improper handwashing, cited in some form at eleven of the fifteen facilities.
