MIAMI, FL. State inspectors emergency-closed Tribute to Tobacco Road by Kush on South Miami Avenue on October 8 after finding roach and fly activity at the Brickell-area bar, one of 15 Miami-Dade restaurants to draw high-severity violations during the week of October 7 through October 13, 2026.
The closure was not the only serious finding at the address. Inspectors also cited the restaurant for an employee not reporting symptoms of illness, inadequate handwashing by food employees, food contact surfaces not properly cleaned or sanitized, improper sewage or wastewater disposal, and reuse of single-use items. That is six violations total, three of them high-severity, at a location that had already accumulated 24 prior inspections on record.
The Week's Most Serious Findings
Sushi Sake Mimo on Biscayne Boulevard and Havana 40 on SW 75th Avenue each drew 10 high-severity violations, the highest single-facility totals of the week. At Sushi Sake Mimo, inspectors documented the absence of an employee health policy, an employee not reporting illness symptoms, improper handwashing technique, inadequate shell stock identification records, food contact surfaces not properly cleaned or sanitized, improper use of time as a public health control, no consumer advisory for raw or undercooked foods, and toxic chemicals improperly stored or labeled.
That last citation carries particular weight at a sushi operation. Toxic chemicals stored near or above food preparation surfaces can contaminate food through spills or mislabeling, and Sushi Sake Mimo's lack of shellfish traceability records means there would be no reliable way to trace the origin of oysters, clams, or mussels served to customers if someone became ill.
Havana 40's 10 high-severity violations included food from an unapproved or unknown source, food in poor condition or adulterated, and food not cooked to the required minimum temperature. That combination, unapproved sourcing plus undercooking, represents the full contamination chain: potentially unsafe food entering the kitchen and then not being rendered safe by heat.
Blue Matisse Restaurant on NW 87th Avenue drew 10 high-severity violations as well, including no person in charge present or performing duties, no employee health policy, an employee not reporting illness symptoms, inadequate handwashing facilities, improper handwashing technique, food from an unapproved source, inadequate shell stock identification records, and food contact surfaces not properly cleaned or sanitized.
Blue Matisse and IHC Ballroom and Meeting Rooms share the same address at 2505 NW 87th Avenue. IHC Ballroom drew eight high-severity violations of its own that week, including food from an unapproved source, inadequate shell stock identification records, toxic chemicals improperly stored or labeled, and no consumer advisory for raw or undercooked foods.
FlyFuel Food Co on NE 29th Street accumulated nine high-severity violations, among them no person in charge, no employee health policy, an employee not reporting illness, inadequate handwashing facilities, improper handwashing technique, inadequate shell stock identification, food contact surfaces not properly cleaned, and food not cooked to the required minimum temperature.
Casablanca Seafood Bar and Grill on NW North River Drive was cited for eight high-severity violations, including parasite destruction procedures not followed. At a seafood restaurant serving raw or lightly cooked fish, that citation means the facility cannot document that fish intended for raw service was frozen to the temperatures required to kill parasites such as Anisakis.
Lucca Modern Trattoria Rustic Soul on NW 104th Avenue drew eight high-severity violations including food from an unapproved source, no allergen awareness demonstrated, and toxic chemicals improperly stored or labeled. Lucca had only two prior inspections on record before this week.
Fuegos del Sur on Mills Drive was cited for eight high-severity violations including inadequate handwashing facilities, improper handwashing technique, toxic chemicals improperly stored or labeled, no consumer advisory for raw or undercooked foods, and improper use of time as a public health control.
La Brasa Grill on SW 87th Avenue drew eight high-severity violations including no person in charge, food from an unapproved source, and toxic substances improperly identified, stored, or used.
Mi Ranchito Salvadoreno Restaurante on West Flagler Street was cited for eight high-severity violations including food not cooked to the required minimum temperature, no allergen awareness demonstrated, and toxic chemicals improperly stored or labeled.
Regener8 Cafe on NW 2nd Avenue drew eight high-severity violations including no person in charge, food from an unapproved source, inadequate handwashing facilities, food not cooked to the required minimum temperature, and no consumer advisory for raw or undercooked foods.
Mayas Kitchen 305 on SW 40th Street was cited for seven high-severity violations including parasite destruction procedures not followed, food not cooked to the required minimum temperature, and improper sewage or wastewater disposal.
La Granja Le Jeune on NW 42nd Avenue drew four high-severity violations, including no employee health policy, inadequate handwashing, food contact surfaces not properly cleaned, and no consumer advisory for raw or undercooked foods.
Patacon Pisao on SW 120th Street received one high-severity violation, an employee not reporting symptoms of illness.
What These Violations Mean
The most common thread running through this week's inspections is the collapse of handwashing infrastructure and practice. Nine facilities were cited for improper handwashing technique, inadequate handwashing facilities, or both. At Blue Matisse, FlyFuel Food Co, Fuegos del Sur, Mi Ranchito Salvadoreno Restaurante, Mayas Kitchen 305, and Regener8 Cafe, inspectors found that the physical infrastructure for hand hygiene was inadequate, meaning employees could not wash their hands properly even if they tried. Improper technique, cited at Sushi Sake Mimo, Havana 40, Lucca Modern Trattoria, La Brasa Grill, and others, means pathogens remain on hands even after a washing attempt.
The illness-reporting failures are a direct outbreak risk. At Tribute to Tobacco Road by Kush, Sushi Sake Mimo, Blue Matisse, FlyFuel Food Co, Lucca Modern Trattoria, Casablanca Seafood Bar and Grill, Fuegos del Sur, La Brasa Grill, Regener8 Cafe, and Patacon Pisao, inspectors found either no employee health policy or employees not required to report illness symptoms. Norovirus, the leading cause of foodborne illness outbreaks in restaurant settings, spreads overwhelmingly through infected food workers who continue to handle food.
Food from unapproved sources, cited at Havana 40, Blue Matisse, Lucca Modern Trattoria, La Brasa Grill, IHC Ballroom, and Regener8 Cafe, means that food entered those kitchens without passing through USDA- or FDA-regulated supply chains. If a customer became ill after eating at one of those establishments, there would be no reliable way to trace the food back to its origin, identify other affected consumers, or issue a recall.
The parasite destruction citations at Casablanca Seafood Bar and Grill and Mayas Kitchen 305 are specific to raw or undercooked fish service. Parasites including Anisakis survive in raw fish and are only killed by sustained freezing at regulated temperatures. A restaurant that cannot document that process has no proof the fish it serves raw is safe.
The Longer Record
Several of this week's worst performers are not newcomers to the inspection process. Havana 40 had 33 prior inspections on record before this week's 10 high-severity citations, the longest inspection history of any facility in this roundup. La Granja Le Jeune, Casablanca Seafood Bar and Grill, La Brasa Grill, and Mi Ranchito Salvadoreno Restaurante each had 27 or more prior inspections on record. Tribute to Tobacco Road by Kush had 24.
That volume of prior inspections means state inspectors have visited those facilities dozens of times. For Havana 40 to draw 10 high-severity violations after 33 prior inspections, including citations for food from unapproved sources and food not cooked to minimum temperature, is not a record of a facility that corrected problems early and maintained compliance.
At the other end of the history scale, Lucca Modern Trattoria Rustic Soul and Regener8 Cafe each had only two prior inspections on record before this week. Both drew eight high-severity violations. Lucca was cited for food from an unapproved source, no allergen awareness, and toxic chemicals improperly stored. Regener8 was cited for no person in charge, food from an unapproved source, food not cooked to minimum temperature, and inadequate handwashing facilities. Two inspections is not enough history to establish a pattern, but eight high-severity violations in the first few inspections is not a promising start.
FlyFuel Food Co and Fuegos del Sur each had 10 prior inspections on record. Sushi Sake Mimo had eight. All three drew among the highest violation counts of the week.
Blue Matisse and IHC Ballroom, sharing the same address at 2505 NW 87th Avenue, drew a combined 18 high-severity violations between them. Blue Matisse had 23 prior inspections on record. IHC Ballroom had 16. Both were cited for food from unapproved sources and inadequate shell stock identification records at the same address on the same inspection week.
