JACKSONVILLE, FL. Across Jacksonville that week, 15 facilities accumulated high-severity violations. The problems ranged from unapproved food sources at a Korean fried chicken chain to improperly stored chemicals at a Riverside arepa spot to sewage disposal violations at two downtown restaurants within blocks of each other.

La Marsa's violations were notable not just for their number but for their cluster around the same failure point. La Nopalera Mexican Restaurant 7 on Beach Boulevard drew eight high-severity violations including toxic chemicals improperly stored or labeled, food from an unapproved or unknown source, and no consumer advisory for raw or undercooked foods. Inspectors also cited the restaurant for parasite destruction procedures not followed.

Downtown and Riverside

Arepa Please on Riverside Avenue received eight high-severity violations, among them two separate chemical storage citations: toxic chemicals improperly stored or labeled, and toxic substances improperly identified, stored, or used. The restaurant was also cited for no consumer advisory for raw or undercooked foods and for inadequate shellfish identification records.

Two Forsyth Street restaurants, less than a quarter-mile apart in the heart of downtown, each drew intermediate citations for improper sewage or wastewater disposal.

Spliff's Gastropub on East Forsyth was cited for that sewage violation alongside seven high-severity findings, including inadequate handwashing facilities, toxic chemicals improperly stored or labeled, and no consumer advisory for raw or undercooked foods.

Kingdom Sushi on Town Center Parkway, one of two sushi operations cited this week, also drew the sewage disposal intermediate violation. Its seven high-severity citations included food from an unapproved or unknown source and parasite destruction procedures not followed, a combination that matters acutely for a raw fish menu.

Indulgence Southern Bistro on West Forsyth received a single high-severity citation, for food contact surfaces not properly cleaned or sanitized.

Hotels, Sports Bars, and a Sushi Counter

Courtyard Jacksonville on Lantern Street drew seven high-severity violations including food not cooked to required minimum temperature, two separate chemical storage citations, and parasite destruction procedures not followed. The hotel's kitchen also lacked adequate shellfish identification records.

MVP's Sports Grille on Atlantic Boulevard was cited for seven high-severity violations, one of which stands apart: time as a public health control not properly used. When a kitchen relies on time rather than temperature to keep food safe, precise recordkeeping is required. Inspectors found those procedures were not being followed.

Green Papaya Thai Sushi Bar on Blanding Boulevard drew six high-severity violations including food not cooked to required minimum temperature, no consumer advisory for raw or undercooked foods, and toxic chemicals improperly stored or labeled.

Time Out Sports Grill on Beach Boulevard was cited for six high-severity violations including parasite destruction procedures not followed, inadequate shellfish identification records, and toxic chemicals improperly stored or labeled. An intermediate citation flagged inadequate cooling and cold-holding equipment.

J Lemon Pepper on West Beaver Street drew six high-severity violations including two chemical storage citations, inadequate shellfish identification records, and parasite destruction procedures not followed.

Ramada by Wyndham Jacksonville Hotel and Conference Center on Hartley Road received two high-severity violations: no consumer advisory for raw or undercooked foods and required procedures for specialized processes not followed. An intermediate citation flagged single-use items being reused.

What These Violations Mean

The single most common high-severity finding this week was employees not reporting illness symptoms, cited at La Marsa Grill, Broadway Deli, BonChon, La Nopalera, Krystal, Arepa Please, Kingdom Sushi, Spliff's Gastropub, Green Papaya Thai Sushi Bar, and J Lemon Pepper. A food worker with Norovirus who handles ready-to-eat food can infect dozens of customers before a single complaint is filed. The violation is not about paperwork.
