JACKSONVILLE, FL. Blu Diner of Jax on Norwood Avenue drew nine high-severity violations in a single inspection last week, including toxic chemicals stored improperly near food, no records to trace shellfish back to their source, and a person in charge who was either absent or not performing required duties.
That was the worst single tally among 15 Jacksonville-area restaurants that state inspectors cited for high-severity violations during the week of September 19 through September 25, 2026.
The Violations
At Dick's Wings and Grill on Old St. Augustine Road, inspectors documented eight high-severity violations, including no written employee health policy, employees not reporting illness symptoms, and no allergen awareness demonstrated by staff. Inspectors also cited the restaurant for failing to properly use time as a public health control, meaning food was allowed to sit in the bacterial growth zone without adequate tracking.
Zen Dumpling on Baymeadows Road also drew eight high-severity violations. Among the most serious: food sourced from unapproved or unknown suppliers, no person in charge present or performing duties, and no allergen awareness. The combination of unverifiable food sourcing and absent management in the same inspection is notable.
JJ Wing and Burger on Duval Station Road matched that count with eight high-severity violations of its own, including food not cooked to required minimum temperatures, food from unapproved sources, and inadequate shellfish traceability records. Salmonella in poultry survives below 165 degrees Fahrenheit, and the absence of shellfish records means there is no way to trace contaminated product back to its origin if someone gets sick.
Secret Garden Cafe on Beach Boulevard drew seven high-severity violations, including food not cooked to minimum required temperatures, no person in charge, and an intermediate violation for improper sewage or wastewater disposal. That sewage citation puts it in company with Banh Mi and Pho Express on Saint John Bluff Road South, which also drew an improper sewage disposal citation alongside seven high-severity findings that included parasite destruction procedures not followed for fish and shellfish without adequate traceability records.
Flame Sushi Japanese Grill and Ramen on Beach Boulevard was cited for food from unapproved sources, food in poor condition or adulterated, and improperly cleaned food contact surfaces, among seven high-severity violations. A sushi operation sourcing fish from unverifiable suppliers is a direct parasite and pathogen risk.
Twin Peaks Restaurant on Atlantic Boulevard drew seven high-severity violations including food contaminated by chemical, physical, or biological hazards, shellfish without adequate identification records, and food not cooked to required minimum temperatures.
Joseph's Pizza on Main Street was cited for seven high-severity violations including no allergen awareness, no consumer advisory for raw or undercooked foods, and improper toxic substance storage. A pizza restaurant with no allergen awareness protocol is a direct risk to customers with wheat, dairy, or nut allergies.
Elevation Lounge on Norwood Avenue, just blocks from Blu Diner of Jax, drew seven high-severity violations including food not cooked to required minimum temperatures, time as a public health control not properly used, and no consumer advisory for raw or undercooked foods.
J Lemon and Pepper on North Main Street was cited for food from unapproved sources, no allergen awareness, and improperly stored toxic chemicals, among seven high-severity violations.
McDonald's on North Main Street drew seven high-severity violations including parasite destruction procedures not followed, no allergen awareness, and no consumer advisory for raw or undercooked foods. The allergen finding at a national chain with standardized training protocols is worth noting.
Lily on Southside Boulevard was cited for both inadequate handwashing and improper handwashing technique in the same inspection, alongside food not cooked to required minimum temperatures and no employee health policy. An intermediate violation for inadequate or improperly maintained toilet facilities rounded out the report.
JVille Seafood and More on Blanding Boulevard drew six high-severity violations and seven intermediate citations, the highest intermediate count of any facility this week. Among the findings: food in poor condition or adulterated, improper sewage disposal, and no allergen awareness.
St. James Cafe on West Duval Street was the only facility among the 15 to record zero high-severity violations, drawing a single intermediate citation for improperly reusing single-use items.
What These Violations Mean
The illness-reporting failures documented at Blu Diner of Jax, Dick's Wings and Grill, Zen Dumpling, Twin Peaks, Lily, and JVille Seafood and More represent the most direct transmission risk in this week's data. When a food worker handles ingredients while symptomatic with norovirus or hepatitis A and does not report it, there is no barrier between that worker and every plate leaving the kitchen. Norovirus accounts for roughly 20 million infections in the United States each year, and food service workers are a primary transmission vector.
The unapproved food source violations at Zen Dumpling, JJ Wing and Burger, Flame Sushi, and J Lemon and Pepper carry a different kind of risk. Food purchased outside licensed, inspected supply chains bypasses USDA and FDA oversight entirely. If a customer becomes ill after eating at one of those restaurants, investigators have no supply chain documentation to follow. The traceability gap is the problem, not just the food itself.
Parasite destruction failures at Banh Mi and Pho Express and McDonald's are particularly acute in a week that also saw multiple raw-fish operations cited. Parasites including Anisakis in fish and Trichinella in pork survive unless the product is either cooked to required temperatures or frozen according to specific time-and-temperature protocols. Neither restaurant had documentation showing those protocols were followed.
The sewage and wastewater disposal violations at Secret Garden Cafe, Banh Mi and Pho Express, and JVille Seafood and More introduce a contamination pathway that touches the entire facility. Improperly managed wastewater carries fecal bacteria that can migrate to food preparation surfaces, equipment, and employee hands.
The Longer Record
Blu Diner of Jax has 54 prior inspections on record, more than any other facility cited this week. Nine high-severity violations on the 54th inspection is not a new operation finding its footing. That is a facility with an extensive documented history receiving a high-severity citation count that still ranks at the top of this week's list.
Twin Peaks Restaurant has 39 prior inspections on record and drew seven high-severity violations including food contamination and undercooking citations. J Lemon and Pepper has 36 prior inspections, Banh Mi and Pho Express has 35, and Joseph's Pizza has 35 as well. All four drew seven or more high-severity violations this week despite inspection histories that extend well into the dozens.
The newer facilities tell a different story. JJ Wing and Burger has only four prior inspections on record and drew eight high-severity violations, including undercooking and unapproved food sourcing. Lily on Southside Boulevard has six prior inspections and drew seven high-severity violations including both handwashing categories cited simultaneously.
JVille Seafood and More has eight prior inspections and produced the week's highest intermediate violation count at seven, alongside six high-severity findings. That volume of citations across both severity tiers in the early inspections of a seafood operation is a pattern worth watching.
The Longer Pattern
Across 14 of the 15 facilities cited this week, improperly cleaned food contact surfaces appeared as a high-severity violation. Cutting boards, prep tables, and slicers that are not properly sanitized between uses transfer bacteria from one food item to the next without any visible sign of contamination.
Shellfish traceability failures appeared at Blu Diner of Jax, JJ Wing and Burger, Banh Mi and Pho Express, Twin Peaks, Joseph's Pizza, Elevation Lounge, and McDonald's. Seven facilities in a single week, across cuisines ranging from pizza to fast food to seafood, all without adequate records to trace oysters, clams, or mussels back to their harvest source.
McDonald's on North Main Street has 17 prior inspections on record. It is the only national chain in this week's data, and it drew seven high-severity violations including parasite destruction failures and no allergen awareness protocol documented. The chain's standardized operating procedures did not prevent those findings on this inspection.
