JACKSONVILLE, FL. State inspectors cited Thai Spice Kitchen on Bowden Road for 12 high-severity violations during the week of August 4, the highest single-facility count in Duval County that week, including food from unapproved sources, failed parasite destruction procedures, and workers not reporting illness symptoms.
The same week, two Jacksonville restaurants were emergency-closed on August 4 for roach activity: Apna Addaa at 8350 Baymeadows Road and Popeyes Louisiana Kitchen #1369 at 6310 103rd Street. Both were shuttered the same day.
The Violations
Thai Spice Kitchen's record this week included a citation for inadequate shell stock identification, meaning inspectors found no documentation tracing shellfish back to their harvest source. It also drew violations for both inadequate handwashing by employees and improper handwashing technique, a combination that means workers were not washing their hands enough and were not washing them correctly when they did.
Shibuya Sushi on Crosshill Boulevard collected 11 high-severity violations, including failed parasite destruction procedures and no person in charge present or performing duties. Inspectors also found inadequate handwashing facilities, meaning the physical infrastructure for proper hygiene was absent, and cited the restaurant for improperly using time as a public health control.
Xtreme Wings on North Main Street matched Shibuya's count at 11 high-severity violations. The citations included food from unapproved or unknown sources, no employee health policy, and food contact surfaces not properly cleaned or sanitized.
Tulum Flavors of Mexico on East Town Parkway drew 10 high-severity violations, including food contaminated by chemical, physical, or biological hazards, food from unapproved sources, and failed parasite destruction. Inspectors also found no person in charge present.
Que Padre Mexican Restaurant on University Boulevard South was cited for nine high-severity violations, among them two separate chemical storage failures: toxic chemicals improperly stored or labeled, and toxic substances improperly identified, stored, or used. The restaurant also had no person in charge present and no employee health policy.
Wok N Roll on Dunn Avenue drew nine high-severity violations including food not cooked to required minimum temperature, improperly stored toxic chemicals, and no consumer advisory for raw or undercooked foods. No person in charge was present during the inspection.
Popeyes Louisiana Kitchen #162 on South University Boulevard was cited for nine high-severity violations including food not cooked to required minimum temperature and no consumer advisory for raw or undercooked foods. This is a separate location from the Popeyes on 103rd Street that was emergency-closed the same week for roach activity.
Blue Boy Sandwich Shop on Norwood Avenue also reached nine high-severity violations, with citations for food from unapproved sources, food not cooked to required minimum temperature, and improperly stored toxic chemicals. No person in charge was present.
Mudville Grille on Beach Boulevard drew eight high-severity violations including inadequate handwashing facilities, food contact surfaces not properly cleaned, and toxic chemicals improperly stored or labeled. Inspectors also found food in poor condition and no consumer advisory for raw or undercooked items.
Dream of Istanbul Cuisine on Beach Boulevard was cited for eight high-severity violations, including food contaminated by chemical, physical, or biological hazards, failed parasite destruction, and food not cooked to required minimum temperature. What These Violations Mean
The most common violation across all 15 facilities this week was employees not reporting symptoms of illness, cited at 14 of the 15 restaurants.
