JACKSONVILLE, FL. An employee illness reporting failure, the violation food safety experts most directly link to multi-victim outbreaks, was among six high-severity citations handed to Señor Loco Tacos and Tequila on Baymeadows Road during a July 29 inspection. The restaurant was not closed.

State inspectors documented the six high-severity violations alongside two intermediate citations during a single visit to the Baymeadows Road location. The facility has now accumulated 69 total violations across eight inspections on record.

Required procedures for specialized food processes were not being followed, a citation that applies to techniques like smoking, curing, or reduced-oxygen packaging, each of which demands precise protocols to prevent bacterial growth.

Inspectors also found that time was not being properly used as a public health control. When a kitchen tracks time rather than temperature to keep food safe, the system only works if the timing is strictly documented and enforced. It was not. The restaurant also lacked a required consumer advisory for raw or undercooked menu items, meaning customers who are pregnant, elderly, or immunocompromised had no way of knowing they were ordering food that carried elevated risk.

The facility has never been emergency-closed.
