WEST PALM BEACH, FL. A sewage backup forced the emergency closure of IHOP on Belvedere Road on September 10, the same week state inspectors documented high-severity violations at 14 other restaurants across the city, including failures involving unapproved food sources, undercooking, and facilities where no one in charge was present when inspectors arrived.

The IHOP closure came alongside two high-severity citations at that location: food contact surfaces not properly cleaned or sanitized, and toxic chemicals improperly stored or labeled. Inspectors also noted inadequate cooling equipment and ventilation problems. The restaurant has 37 prior inspections on record, more than almost any facility cited this week.

Bull Top Taste Restaurant on 45th Street led the week with six high-severity violations and no intermediate violations. Inspectors cited the restaurant for having no employee health policy, failing to report illness symptoms, improper handwashing technique, inadequate shellfish traceability records, food contact surfaces not properly cleaned, and toxic chemicals improperly stored or labeled.

That combination, no health policy, no illness reporting, and flawed handwashing, represents a full breakdown of the basic barriers that keep sick workers from spreading illness to food. Bull Top Taste has 20 prior inspections on record.

Sunday Motor Co. on North Railroad Avenue drew five high-severity violations, including food from an unapproved or unknown source and food not cooked to the required minimum temperature. Inspectors also cited the restaurant for failing to properly use time as a public health control, a practice that allows food to sit in the temperature danger zone when not tracked with precision. The restaurant has only four prior inspections on record.

Grand Lake Chinese Restaurant on Okeechobee Boulevard also drew five high-severity violations. No person in charge was present when inspectors arrived. Food was sourced from an unapproved or unknown supplier. The menu offered raw or undercooked items with no consumer advisory posted to warn diners.

Pubbelly Sushi on Clematis Street matched that count with five high-severity citations. Inspectors found improper handwashing technique, food contact surfaces not properly cleaned, inadequate shellfish identification records, and food not cooked to the required minimum temperature. A sushi restaurant with unverified shellfish sourcing and undercooking violations draws particular scrutiny because the menu depends heavily on raw and lightly cooked proteins.

Across the Rest of the List

Cucina Italiana Ristorante Pizza Vino on Fairway Drive had four high-severity violations, including no person in charge, employees not reporting illness symptoms, improper handwashing, and no consumer advisory for raw or undercooked foods. Inspectors also noted multi-use utensils not properly cleaned and wiping cloths used improperly.

Zipitios on Florida Avenue drew the same high-severity count: no person in charge, no employee health policy, employees not reporting illness symptoms, and food contact surfaces not properly cleaned or sanitized. Inadequate toilet facilities rounded out the intermediate violations.

Tradicion Michoacana on Cresthaven Boulevard was cited for food in poor condition, mislabeled, or adulterated, alongside improper handwashing, failure to report illness symptoms, and inadequate shellfish records. Food described as in poor condition or adulterated is a direct quality and safety hazard, not a paperwork problem.

Leo's Pizza on North Haverhill Road had four high-severity violations including no allergen awareness demonstrated. That citation matters: food allergies affect 32 million Americans, and a staff that cannot identify allergens in its own menu items cannot protect customers who ask.

Il Bellagio on South Rosemary Avenue drew three high-severity violations including no person in charge and no consumer advisory for raw or undercooked foods. Off Tha Bone BBQ on North Haverhill Road, in the same shopping center as Leo's Pizza, also drew three high-severity violations, including no employee health policy and inadequate shellfish records.

Frost Ice Cream on South Dixie Highway had three high-severity violations despite having only two prior inspections on record. No person in charge was present. An employee had not reported illness symptoms. And food was not cooked to the required minimum temperature, a citation that carries particular weight at a location where customers expect a straightforward product.

India Grill and Bar on Royal Palm Beach Boulevard was cited for food not cooked to required minimum temperature and toxic substances improperly identified, stored, or used. Medi Bowls on Banyan Boulevard drew citations for inadequate handwashing and food not cooked to required minimum temperature. La Ventanita on South Military Trail was cited for inadequate shellfish records and no consumer advisory for raw or undercooked foods.

What These Violations Mean

The most frequently cited violation this week was employees not reporting symptoms of illness, which appeared at 13 of the 15 facilities. This is not a paperwork failure. A food worker with Norovirus who handles ready-to-eat food can transmit illness to dozens of customers before a single complaint is filed. When that worker's employer has no written health policy, as was the case at Bull Top Taste, Zipitios, and Off Tha Bone BBQ, there is no documented standard for the worker to follow or the employer to enforce.

Food from unapproved or unknown sources, cited at Sunday Motor Co. and Grand Lake Chinese Restaurant, removes the traceability that makes outbreak investigation possible. If a customer gets sick, inspectors need to trace the food back through the supply chain. Unapproved sources break that chain entirely, and they bypass the federal safety inspections that catch Listeria and Salmonella before product reaches a kitchen.

Inadequate shellfish records appeared at six facilities this week: Bull Top Taste, Sunday Motor Co., Pubbelly Sushi, Tradicion Michoacana, Leo's Pizza, and Off Tha Bone BBQ. Oysters, clams, and mussels are frequently consumed raw. Without harvest tags and dealer records on file, there is no way to identify the source if a customer develops a shellfish-related illness.

Undercooking citations, documented at Sunday Motor Co., Pubbelly Sushi, Frost Ice Cream, India Grill and Bar, and Medi Bowls, reflect a failure at the last line of defense. Proper cooking temperatures kill Salmonella, E. coli, and Campylobacter. Food that does not reach those temperatures delivers live pathogens to the plate.

The Longer Record

India Grill and Bar carries the longest inspection history of any facility cited this week: 46 inspections on record. Two high-severity violations this week, including improperly stored toxic substances, at a location with that many inspections behind it suggests the facility has not resolved underlying compliance problems despite repeated state contact.

IHOP on Belvedere Road has 37 inspections on record. The sewage backup that forced its emergency closure on September 10 was accompanied by chemical storage violations and inadequate cooling equipment. A facility with that inspection history and a sewage event in the same week presents a pattern that the record does not explain away.

Grand Lake Chinese Restaurant on Okeechobee Boulevard has 36 prior inspections. This week's visit found no person in charge, food from an unapproved source, and no consumer advisory for raw items. Thirty-six inspections is a long relationship between a facility and the state. The violations documented this week are not new categories of concern.

At the other end of the spectrum, Sunday Motor Co. and Pubbelly Sushi each have only four prior inspections. Both drew five high-severity violations this week. Sunday Motor Co. was cited for food from an unapproved source; Pubbelly Sushi for undercooking and unverified shellfish sourcing. Frost Ice Cream had only two inspections on record before this visit, and its first significant contact with inspectors produced three high-severity citations including a cooking temperature failure.

Leo's Pizza has 26 inspections on record. The allergen awareness citation documented this week, at a pizza operation where cross-contact with common allergens is an everyday operational concern, remained unresolved as of the inspection date.
