WEST PALM BEACH, FL. Inspectors visiting Hot Wok on Forest Hill Boulevard this week cited the restaurant for food contaminated by chemical, physical, or biological hazards, an employee failing to report symptoms of illness, improperly cleaned food contact surfaces, and missing shellfish traceability records, among six total high-severity violations in a single inspection.
That combination of citations, contaminated food alongside an unreporting sick worker, represents one of the more dangerous profiles inspectors can document in a single visit. Fourteen other West Palm Beach restaurants drew high-severity citations during the week of August 10 through August 16.
The Violations
China Wok on West Forest Hill Boulevard matched Hot Wok's citation count with six high-severity violations of its own. Inspectors cited the restaurant for food from an unapproved or unknown source, toxic chemicals improperly stored or labeled, inadequate shellfish traceability records, no consumer advisory for raw or undercooked foods, improper handwashing technique, and no person in charge present or performing duties.
Palm Beach Pizza on Okeechobee Boulevard drew five high-severity citations. Inspectors found no employee health policy, employees not reporting illness symptoms, improper handwashing technique, improperly cleaned food contact surfaces, and no person in charge on duty.
That last citation matters more than it might appear. When no qualified manager is present, the violations that follow tend to multiply.
O'Shea's Pub on Clematis Street received four high-severity citations, including improperly stored or labeled toxic chemicals and food contact surfaces that were not properly cleaned or sanitized. Inspectors also cited the pub for improper handwashing technique and no consumer advisory for raw or undercooked items, plus an intermediate violation for inadequate ventilation and lighting.
Tarbooch Hookah Cafe on South Military Trail also drew four high-severity violations. An employee not reporting illness symptoms was among them, alongside inadequate shellfish traceability records, improper handwashing technique, and no consumer advisory for raw or undercooked foods.
Blind Monk on South Olive Avenue collected four high-severity citations: inadequate handwashing by food employees, improper handwashing technique, inadequate shellfish traceability records, and food contact surfaces not properly cleaned or sanitized. Inspectors cited both the inadequacy of handwashing and the technique used, two separate failures in the same fundamental practice.
Las Americas Latin Restaurant on South Military Trail was cited for three high-severity violations, including food from an unapproved or unknown source and improper handwashing technique. Inspectors also flagged the absence of a consumer advisory for raw or undercooked foods.
Sour Seed Bagels on Fern Street received three high-severity citations, including food contaminated by chemical, physical, or biological hazards and food from an unapproved or unknown source. That pairing of contaminated product and unverified sourcing at a single location draws concern from a traceability standpoint.
Eataly on South Rosemary Avenue was cited for three high-severity violations: no person in charge present, an employee not reporting illness symptoms, and no allergen awareness demonstrated by staff.
Pete's Place 2 on North Jog Road drew one high-severity citation for required procedures for specialized processes not being followed. Specialized processes such as smoking, curing, fermenting, and reduced-oxygen packaging require precise controls; without them, anaerobic bacteria including Clostridium botulinum can proliferate without any visible sign of spoilage.
Wing Stop on South Military Trail was cited for inadequate shellfish traceability records and improperly cleaned food contact surfaces, plus an intermediate violation for improper sewage or wastewater disposal.
Maman on South Rosemary Avenue received two high-severity citations, for an employee not reporting illness symptoms and improper handwashing technique, alongside an intermediate citation for inadequate or improperly maintained toilet facilities.
Bamby Restaurant and Bar on South Dixie Highway was cited for no person in charge present and an employee not reporting illness symptoms.
McDonald's at 2605 South Military Trail drew two high-severity citations: food contact surfaces not properly cleaned or sanitized, and toxic substances improperly identified, stored, or used.
Pura Vida Miami on South Rosemary Avenue was cited for improperly cleaned food contact surfaces and time not being properly used as a public health control. When a facility uses time rather than temperature to keep food safe, any failure in tracking that window means food sits in the bacterial growth zone with no safety net in place.
What These Violations Mean
The most alarming combination this week appeared at Hot Wok and Palm Beach Pizza: employees not reporting illness symptoms, paired with no written health policy and improperly cleaned surfaces. Food workers who do not report symptoms are the leading driver of multi-victim outbreaks. Norovirus, which causes the majority of foodborne illness outbreaks in the United States, can be transmitted by a single sick worker touching a surface that dozens of customers subsequently contact.
Food from unapproved or unknown sources, cited at Las Americas Latin Restaurant, China Wok, and Sour Seed Bagels, carries a specific danger that has nothing to do with how the food looks or smells. Unapproved sources bypass USDA and FDA inspection checkpoints entirely. If a customer gets sick, investigators have no supply chain to trace, no lot numbers to pull, no distributor to contact.
Shellfish traceability failures appeared at Hot Wok, China Wok, Tarbooch Hookah Cafe, Blind Monk, and Wing Stop. Oysters, clams, and mussels are high-risk foods eaten raw or lightly cooked. State law requires shellstock tags to be kept on file so that contaminated harvesting beds can be identified quickly if an illness cluster emerges. Without those records, a hepatitis A or Vibrio exposure cannot be traced to its source.
The allergen citation at Eataly stands apart from the others. Allergen reactions cause an estimated 30,000 emergency room visits annually in the United States. When staff cannot demonstrate awareness of the allergens present in the food they are serving, customers with tree nut, shellfish, or dairy allergies are eating without any functional safety net.
The Longer Record
Pete's Place 2, with 36 prior inspections on record, has the longest documented history of any facility cited this week. One high-severity citation after 36 inspections is a different profile than what inspectors found at China Wok, which has 24 prior inspections and drew six high-severity violations this week, including toxic chemicals stored improperly and food from an unapproved source. That is not a first-time stumble at an established location.
Hot Wok has 27 prior inspections on record, as does Palm Beach Pizza. Both drew six and five high-severity violations respectively this week. Las Americas Latin Restaurant has 29 inspections on record and was cited again for both unapproved food sourcing and improper handwashing technique, categories that are foundational enough to appear in the earliest inspections any new facility receives.
Blind Monk has only 6 prior inspections on record and already carries four high-severity citations from this week, including both inadequate handwashing and improper handwashing technique as separate documented failures. Eataly, with just 3 prior inspections, drew three high-severity violations including no allergen awareness and no person in charge. Maman, with 7 inspections, adds to a cluster of newer South Rosemary Avenue locations accumulating serious citations early in their inspection histories.
Wing Stop has 23 inspections on record and was cited this week for an intermediate sewage or wastewater disposal violation alongside two high-severity findings. Improper sewage disposal creates the risk of fecal contamination throughout a facility, and it is not a violation that appears in a kitchen that has been carefully maintained. That citation, at a location with more than two dozen inspection visits behind it, is the one that has not been answered yet.
