Lolis 2 on West Hillsborough Avenue accumulated seven high-severity violations in a single inspection this week, including food sourced from unapproved suppliers, no employee health policy, inadequate shellfish traceability records, and no demonstrated allergen awareness among staff. That tally made it the most-cited facility in a week when inspectors flagged 15 Tampa restaurants for high-severity violations.
The seven findings at Lolis 2 span the full range of what inspectors consider immediately dangerous. An employee was observed not reporting symptoms of illness. Handwashing technique was cited as improper. Required procedures for specialized food processes were not being followed. Two intermediate violations, including improperly cleaned multi-use utensils, rounded out the inspection.
Two restaurants did not make it through the week at all.
Two Closures, Two Days Apart
On September 16, state inspectors ordered Outside BBQ LLC at 1613 East Martin Luther King Jr Boulevard closed for roach and fly activity. The following day, Oasis Los Cuatro Vientos Corp at 4908 East Broadway Avenue was shut down after inspectors documented a sewage backup on the premises.

Toxic chemicals were both improperly stored and improperly identified, creating a direct contamination risk to food preparation areas.
Siam Thai Restaurant on West Linebaugh Avenue, located just a few doors down from Shindig, drew five high-severity violations. An employee was cited for not reporting illness symptoms. Handwashing technique was improper. Shellfish records were inadequate. Food was not cooked to minimum required temperature. No consumer advisory was posted. The facility also had improperly cleaned utensils and inadequate toilet facilities.
Isekai Sushi Cafe on Gunn Highway presented a specific set of concerns tied to its menu. Inspectors cited food in poor condition, inadequate shellfish traceability records, and, critically, parasite destruction procedures not being followed. For a sushi restaurant, that last violation is direct: without proper freezing protocols, parasites in raw fish can survive and reach customers. Blasys Pizzeria on West Hillsborough Avenue had four high-severity violations including no person in charge and no consumer advisory for raw or undercooked foods.
Sal Rosa on North Florida Avenue drew four high-severity findings, including both inadequate handwashing facilities and improper handwashing technique. Those two violations cited together mean employees faced structural barriers to proper hygiene and were not compensating for them.

What These Violations Mean
