JACKSONVILLE, FL. Inspectors ordered two Jacksonville restaurants closed within 48 hours of each other the week of August 26, one for roach activity and one for rodents, while fifteen other Duval County establishments drew high-severity violations ranging from unapproved food sources to employees not reporting symptoms of illness.

Sweet Mama's at 1527 Cesery Blvd was shut down August 26 after inspectors documented rodent activity on the premises. Two days later, Church's Chicken #10465 at 3007 Edgewood Ave W was closed for roach activity on August 28.

Neither closure is unusual in isolation. Together they mark a week when pest-driven emergency orders bookended a broader sweep of violations across the county.

The Violations

Three restaurants tied for the highest single-week violation count in this inspection cycle, each drawing 11 high-severity citations. Miller's Jacksonville Ale House at 9711 Deer Lake Ct was cited for food from an unapproved or unknown source, inadequate shell stock identification records, parasite destruction procedures not followed, and food contact surfaces not properly cleaned or sanitized, in addition to a missing employee health policy, employees not reporting illness symptoms, inadequate handwashing facilities, and improper handwashing technique. That combination, eight distinct high-severity categories in a single visit, is notable even against a week this busy.

Tsunami Red Bowl at 12553 Bartram Park Blvd matched that count with its own 11 high-severity violations, including no person in charge present or performing duties, food from an unapproved source, and time as a public health control not properly used, alongside the same cluster of missing health policy, unreported illness, and handwashing failures.

Beignets Caribbean Cafe at 4770 Barnes Rd also reached 11 high-severity violations. Inspectors there cited food not cooked to the required minimum temperature, food in poor condition or adulterated, inadequate shell stock records, and food contact surfaces not properly cleaned, on top of the handwashing and illness-reporting failures.

Cuban Pizzeria at 4347-2 University Blvd S drew 10 high-severity violations, including food contaminated by chemical, physical, or biological hazards, food in poor condition or adulterated, parasite destruction procedures not followed, and inadequate shell stock records.

Senorita's Mexican Kitchen at 6426 Bowden Rd also reached 10 high-severity citations. No person in charge was present, food was sourced from an unapproved supplier, and shell stock identification records were inadequate.

Jood Shawarma at 1821 Parental Home Rd accumulated 9 high-severity violations and the highest intermediate count of any facility this week, 6, including no person in charge, food in poor condition, inadequate shell stock records, and unsanitized food contact surfaces.

Mestizo Mex Cocina at 5907 Roosevelt Blvd was cited for food not cooked to the required minimum temperature, food in poor condition, no person in charge, and inadequate shell stock records, totaling 9 high-severity violations.

Hwy 55 Burgers at 1027 Danmac Ln drew 9 high-severity violations including toxic chemicals improperly stored or labeled, toxic substances improperly identified or used, food not cooked to the required minimum temperature, and no consumer advisory for raw or undercooked foods.

Spice Grill Indian Cuisine at 12709 Bartram Park Blvd was cited for food not cooked to required temperature, time as a public health control not properly used, and no consumer advisory for raw or undercooked foods, among 9 high-severity violations.

Juicy Crab at 8106 Blanding Blvd drew 9 high-severity violations including inadequate handwashing facilities, food from an unapproved source, food in poor condition, inadequate shell stock records, food not cooked to the required minimum temperature, and no consumer advisory for raw or undercooked foods.

Bamboo Wok at 13820 Old St Augustine Rd was cited for no person in charge, time as a public health control not properly used, toxic chemicals improperly stored, and no allergen awareness demonstrated, totaling 9 high-severity violations and zero intermediate violations.

Sake Sushi at 8206 Philips Hwy drew 8 high-severity violations including food contaminated by chemical, physical, or biological hazards, food from an unapproved source, toxic chemicals improperly stored, and no allergen awareness demonstrated.

Cracker Barrel #341 at 4680 Lenoir Ave was cited for 8 high-severity violations including food from an unapproved source, parasite destruction procedures not followed, no consumer advisory for raw or undercooked foods, toxic chemicals improperly stored, and no allergen awareness demonstrated.

Olympia Cafe at 2550 Mayport Rd drew 8 high-severity violations including no person in charge, food in poor condition, parasite destruction procedures not followed, inadequate shell stock records, and no consumer advisory for raw or undercooked foods.

Sichuan Family at 10584 Old St Augustine Rd drew 1 high-severity violation, no person in charge present, along with 2 intermediate citations for multi-use utensils not properly cleaned and single-use items improperly reused.

What These Violations Mean

The most common high-severity violation this week, appearing at nearly every facility on this list, was the combination of no written employee health policy and employees not reporting symptoms of illness. These two failures are linked. Without a written policy, workers have no formal instruction about when to stay home. Without reporting requirements, a worker with Norovirus handles food, touches surfaces, and serves customers before anyone intervenes. Norovirus spreads through fewer than 20 viral particles, a quantity that transfers easily from an unwashed hand to a plate. Facilities cited for both failures this week include Miller's Jacksonville Ale House, Tsunami Red Bowl, Beignets Caribbean Cafe, Senorita's Mexican Kitchen, Jood Shawarma, Hwy 55 Burgers, Spice Grill Indian Cuisine, Bamboo Wok, and Olympia Cafe.

Food from unapproved or unknown sources was cited at Miller's Jacksonville Ale House, Tsunami Red Bowl, Senorita's Mexican Kitchen, Juicy Crab, Sake Sushi, and Cracker Barrel #341. The hazard is traceability. When a restaurant buys food through uninspected channels, there is no paper trail connecting that product to a farm, processor, or distributor. If a customer gets sick, investigators cannot identify the source, cannot issue a recall, and cannot determine how many others were exposed. That traceability gap is why unapproved sourcing is classified as high-severity regardless of whether the food appears visibly spoiled.

Shellfish traceability failures appeared at Miller's Jacksonville Ale House, Beignets Caribbean Cafe, Cuban Pizzeria, Senorita's Mexican Kitchen, Jood Shawarma, Mestizo Mex Cocina, Spice Grill Indian Cuisine, Juicy Crab, and Olympia Cafe. Oysters, clams, and mussels are filter feeders that concentrate whatever is in the water around them, including Vibrio bacteria and norovirus. Florida requires shell stock tags to travel with each batch from harvest to table. Without those tags, a restaurant cannot tell inspectors, or a sick customer's doctor, where its shellfish came from or when it was harvested.

Hwy 55 Burgers and Bamboo Wok were both cited for improperly stored or labeled toxic chemicals. When cleaning agents or sanitizers are stored near food prep areas without proper labeling, the contamination risk is immediate and acute. A mislabeled spray bottle used on a cutting board can introduce chemical residue directly into food. Unlike bacterial contamination, chemical poisoning does not require time to develop, it can cause illness within minutes of ingestion.

The Longer Record

Two facilities in this week's data carry the longest inspection histories and the most serious pattern questions. Juicy Crab on Blanding Boulevard has 41 prior inspections on record and drew 9 high-severity violations this week, including food from an unapproved source, inadequate handwashing facilities, and food not cooked to the required minimum temperature. Olympia Cafe on Mayport Road also has 40 prior inspections on record and drew 8 high-severity violations this week, including parasite destruction procedures not followed and no consumer advisory for raw or undercooked foods. Forty-plus inspections represent years of regulatory contact. The presence of foundational violations at both locations, food sourcing, cooking temperatures, consumer disclosure, after that volume of visits is a fact the records document without embellishment.

Senorita's Mexican Kitchen on Bowden Road has 40 prior inspections on record and drew 10 high-severity violations this week, including no person in charge and food from an unapproved source. Jood Shawarma on Parental Home Road has 34 prior inspections and drew 9 high-severity violations. Mestizo Mex Cocina on Roosevelt Boulevard has 32 prior inspections, as does Spice Grill Indian Cuisine on Bartram Park Boulevard, and both were cited for food not cooked to the required minimum temperature this week.

Hwy 55 Burgers on Danmac Lane has only 11 prior inspections on record, the fewest of any facility in this week's data, and already drew 9 high-severity violations including two separate chemical storage and labeling failures. A location with that few inspections drawing that many serious citations in a single visit is a different kind of concern than the longer-tenured repeat offenders, but it is a concern the record now documents.

Tsunami Red Bowl and Beignets Caribbean Cafe each have between 24 and 29 prior inspections and each drew 11 high-severity violations this week. Both were cited for the full cluster of illness-reporting, handwashing, and food sourcing failures. Whether either location has been cited for the same categories in prior visits is not resolved by this week's data alone.
