JACKSONVILLE, FL. Inspectors flagged Tsunami Red Bowl on Bartram Park Boulevard with 11 high-severity violations during the week of August 24, the highest single-facility count among 15 Duval County restaurants cited for serious food safety failures in seven days.
Among the violations at Tsunami Red Bowl: food obtained from unapproved or unknown sources, inadequate handwashing facilities, improper hand and arm washing technique, food contact surfaces not properly cleaned or sanitized, and the misuse of time as a public health control. No person in charge was present or performing duties during the inspection. No employee health policy existed, and employees were not reporting illness symptoms.
That combination, management absent, illness reporting absent, and food of unknown origin in the kitchen, is the pattern inspectors most associate with high outbreak risk.
The Violations
Cuban Pizzeria on University Boulevard South drew 10 high-severity violations, including food contaminated by chemical, physical, or biological hazards, food in poor condition or adulterated, and inadequate shellfish identification records. Inspectors also cited the restaurant for failure to follow parasite destruction procedures, a violation that applies when fish is served raw or undercooked without the required freezing protocol.
Señoritas Mexican Kitchen on Bowden Road matched Cuban Pizzeria's count with 10 high-severity violations. No person in charge was present. Food from unapproved or unknown sources was documented, along with inadequate shellfish records and food contact surfaces not properly cleaned or sanitized. Employees were not reporting illness symptoms, and no health policy was in place.
Jood Shawarma on Parental Home Road was cited for nine high-severity violations and six intermediate ones, the largest intermediate count of any facility this week. Food in poor condition, inadequate shellfish records, and improperly cleaned food contact surfaces were among the findings. No person in charge was present, and both the employee health policy and illness reporting requirements were unmet.
Juicy Crab on Blanding Boulevard drew nine high-severity violations, including food from unapproved sources, food not cooked to the required minimum temperature, and no consumer advisory posted for raw or undercooked items. Inadequate handwashing facilities were also documented.
Spice Grill Indian Cuisine on Bartram Park Boulevard was cited for nine high-severity violations, including food not cooked to required minimum temperature, the misuse of time as a public health control, and no consumer advisory for raw or undercooked foods. Inadequate shellfish records were also on the list.
Hwy 55 Burgers on Danmac Lane drew nine high-severity violations, among them two separate citations involving toxic chemicals: improper storage or labeling of toxic chemicals, and improper identification, storage, or use of toxic substances. Food not cooked to minimum required temperature and food in poor condition were also documented.
Bamboo Wok on Old St. Augustine Road was cited for nine high-severity violations and zero intermediate ones. Inspectors documented no allergen awareness demonstrated, toxic chemicals improperly stored or labeled, and the misuse of time as a public health control, alongside a missing person in charge and no employee health policy.
More Facilities, More Failures
Pupuseria y Restaurante Maria on University Boulevard South received eight high-severity violations, including food contaminated by chemical, physical, or biological hazards, food in poor condition, and food contact surfaces not properly cleaned or sanitized. Six intermediate violations accompanied the high-severity findings.
Sake Sushi on Philips Highway was cited for eight high-severity violations, including food from unapproved sources, toxic chemicals improperly stored or labeled, and no allergen awareness demonstrated. Inadequate handwashing facilities and the misuse of time as a public health control were also on the record.
Rosalia's Italian Restaurant and Pizzeria on Old St. Augustine Road drew eight high-severity violations, including food from unapproved sources, food contaminated by chemical, physical, or biological hazards, and no consumer advisory for raw or undercooked foods. No person in charge was present during the inspection.
Olympia Cafe on Mayport Road was cited for eight high-severity violations, including parasite destruction procedures not followed, inadequate shellfish identification records, and no consumer advisory for raw or undercooked foods. No person in charge was present.

Pepe's Hacienda and Restaurant on Merrill Road drew eight high-severity violations, including food from unapproved sources, inadequate shellfish records, and toxic chemicals improperly stored or labeled. Employees were not adequately washing their hands, and no person in charge was on duty.
La Nopalera Mexican Restaurant on Philips Highway received a single high-severity violation: employees not reporting symptoms of illness.
What These Violations Mean
The most repeated high-severity violation this week was the absence of a written employee health policy, documented at eleven of the fifteen facilities. That citation sounds administrative. It is not. Without a formal policy requiring workers to report symptoms like vomiting, diarrhea, or jaundice, a sick employee has no documented obligation to stay out of the kitchen. Norovirus, which causes the majority of foodborne illness outbreaks in restaurant settings, spreads through exactly this pathway. Señoritas Mexican Kitchen, Jood Shawarma, Bamboo Wok, and nine other facilities on this week's list had no such policy in place.
Food from unapproved or unknown sources was documented at Tsunami Red Bowl, Señoritas Mexican Kitchen, Juicy Crab, Sake Sushi, Rosalia's Italian Restaurant and Pizzeria, and Pepe's Hacienda and Restaurant. The practical problem is traceability. When a food item cannot be traced to a licensed, inspected supplier, there is no way to pull it from the supply chain if a contamination event is later identified. Listeria, Salmonella, and E. coli have all entered restaurant kitchens through uninspected supply channels.
Parasite destruction failures at Cuban Pizzeria and Olympia Cafe carry a specific risk that is distinct from bacterial contamination. Fish served raw or undercooked, including sushi and certain Mediterranean preparations, must be frozen to kill parasites such as Anisakis before service.
