That closure was not the week's most layered inspection record. That distinction belongs to Bounxou Thai on San Jose Boulevard, which drew nine high-severity violations in a single visit, the highest count among all facilities inspected this week.

Indochine on East Adams Street drew seven high-severity violations, including two separate chemical-related citations: toxic chemicals improperly stored or labeled, and toxic substances improperly identified, stored, or used. Inspectors also cited employees not reporting illness symptoms, inadequate handwashing, missing shellfish records, food contact surfaces not properly cleaned, and required procedures for specialized processes not followed.

Subway No. 60865 on San Marco Boulevard drew six high-severity violations, including one that stands out for a national chain: no allergen awareness demonstrated. Sr Ceviche Jax on San Jose Boulevard drew six high-severity violations including food from an unapproved or unknown source and no consumer advisory, a particularly acute pairing for a restaurant whose menu centers on raw seafood.

Wok N Roll, a third San Jose Boulevard restaurant cited this week, was flagged for improper handwashing technique, food contact surfaces not properly cleaned, time as a public health control not properly used, toxic chemicals improperly stored, and required procedures for specialized processes not followed.

Waffle House No. 1050 on J Turner Butler Boulevard was cited for parasite destruction procedures not followed, food contact surfaces not properly cleaned, no consumer advisory, and toxic chemicals improperly stored. Inspectors also documented improper sewage or wastewater disposal and inadequate cooling and cold holding equipment.

Alder and Oak on Riverside Avenue drew three high-severity violations: employees not reporting illness symptoms, food not cooked to required minimum temperature, and no consumer advisory for raw or undercooked foods.

Bob Evans Restaurant No. 420 on Hartley Road drew a single high-severity violation: employees not reporting symptoms of illness.

Seven Wonders Bakery and Grill on Timuquana Road drew no high-severity violations, with one intermediate citation for single-use items improperly reused.

What These Violations Mean

The most consequential cluster this week involves illness reporting failures.
