JACKSONVILLE, FL. Tsunami Red Bowl on Bartram Park Boulevard walked into the week of August 23 carrying 11 high-severity violations in a single inspection, including food sourced from unapproved or unknown suppliers, no employee health policy, no person in charge performing duties, and food contact surfaces inspectors found improperly cleaned and sanitized. It was the worst single-inspection tally among 15 Jacksonville restaurants flagged for high-severity violations during the week.
Fourteen of those 15 facilities each drew eight or more high-severity citations. Together they represent a concentrated snapshot of what inspectors found across Duval County in one seven-day stretch.
The Violations
Cuban Pizzeria on University Boulevard South drew 10 high-severity violations, including food inspectors found contaminated by chemical, physical, or biological hazards, food in poor condition or adulterated, inadequate shellfish identification records, and a failure to follow parasite destruction procedures for fish. That last citation means the restaurant was not properly freezing or cooking fish to kill parasites such as Anisakis before serving it.
Señoritas Mexican Kitchen on Bowden Road also reached 10 high-severity violations. Inspectors found no person in charge performing duties, no employee health policy, employees not reporting illness symptoms, inadequate handwashing by food employees, improper handwashing technique, food from unapproved sources, missing shellfish identification records, and food contact surfaces not properly cleaned.
Taco Way on Parental Home Road collected nine high-severity violations and six intermediate ones, the highest intermediate count of the week. Among the high-severity findings: no person in charge, no employee health policy, employees not reporting illness, inadequate handwashing, improper handwashing technique, food in poor condition, missing shellfish records, and unclean food contact surfaces.
Hwy 55 Burgers on Danmac Lane drew nine high-severity violations that included food not cooked to required minimum temperature, no consumer advisory for raw or undercooked foods, and toxic chemicals improperly stored or labeled alongside food. Inspectors also cited improperly used toxic substances, a combination that creates risk of chemical contamination of food or surfaces.
Juicy Crab on Blanding Boulevard reached nine high-severity violations, including food from unapproved sources, inadequate handwashing facilities, missing shellfish identification records, food not cooked to minimum temperature, and no consumer advisory for raw items. A seafood restaurant without shellfish traceability records and without adequate handwashing infrastructure is a particular concern given the high-risk nature of raw shellfish.
Spice Grill Indian Cuisine on Bartram Park Boulevard received nine high-severity violations, including food not cooked to required temperature, time used improperly as a public health control, and no consumer advisory for raw or undercooked foods. Time-as-a-control violations mean food was allowed to sit in the bacterial growth zone, between 41 and 135 degrees, without a documented time limit or discard protocol.
Bamboo Wok on Old St. Augustine Road also drew nine high-severity violations, including no person in charge, improper handwashing technique, toxic chemicals improperly stored, and no allergen awareness demonstrated by staff. Allergen awareness failures put the estimated 32 million Americans with food allergies at risk of a reaction with no warning from the restaurant.
Pupuseria y Restaurante Maria on University Boulevard South received eight high-severity violations, including food contaminated by chemical, physical, or biological hazards, food in poor condition or adulterated, and food contact surfaces not properly cleaned.
Sake Sushi on Philips Highway drew eight high-severity violations, among them food from unapproved sources, food contaminated by chemical or biological hazards, inadequate handwashing facilities, toxic chemicals improperly stored, and no allergen awareness. A sushi restaurant sourcing fish from unverified suppliers compounds the risk already present in raw fish service.
Rosalia's Italian Restaurant and Pizzeria on Old St. Augustine Road collected eight high-severity violations, including no person in charge, food from unapproved sources, food contaminated by hazards, and no consumer advisory for raw or undercooked foods.
Olympia Cafe on Mayport Road drew eight high-severity violations, including parasite destruction procedures not followed, missing shellfish identification records, and no consumer advisory for raw or undercooked foods. Parasite destruction failures at a cafe serving fish mean customers may have consumed fish harboring live parasites.
Bono's Pit BBQ on Beach Boulevard received eight high-severity violations, including toxic chemicals improperly stored, toxic substances improperly identified or used, no consumer advisory, and no person in charge performing duties. A barbecue restaurant with dual chemical storage violations raises immediate questions about what was stored near or above food.
Pepe's Hacienda and Restaurant on Merrill Road also drew eight high-severity violations, including food from unapproved sources, inadequate handwashing, improper handwashing technique, no person in charge, and toxic chemicals improperly stored.
La Nopalera Mexican Restaurant on Philips Highway drew a single high-severity violation, employees not reporting illness symptoms, the lowest tally of any facility on this week's list.
What These Violations Mean
The single most repeated high-severity violation this week was the absence of a written employee health policy, cited at Tsunami Red Bowl, Cuban Pizzeria, Señoritas Mexican Kitchen, Taco Way, Hwy 55 Burgers, Juicy Crab, Spice Grill, Bamboo Wok, Pupuseria y Restaurante Maria, Rosalia's, Olympia Cafe, and others. Without that policy, there is no mechanism requiring a sick worker to stay home. Norovirus, the most common cause of foodborne illness outbreaks in restaurant settings, spreads directly from an infected food handler to food, and a single ill employee can expose dozens of customers in a single shift.
Food from unapproved or unknown sources, cited at Tsunami Red Bowl, Señoritas Mexican Kitchen, Juicy Crab, Sake Sushi, Rosalia's, and Pepe's Hacienda, is a traceability problem as much as a safety one. Food that enters a kitchen through unverified suppliers has bypassed USDA and FDA inspection. If a customer becomes ill, investigators have no supply chain to trace back. That gap can turn a contained illness into an unresolved outbreak.
Parasite destruction failures, cited at Cuban Pizzeria and Olympia Cafe, are specific to fish and shellfish. Freezing fish to required temperatures for a required duration kills parasites such as Anisakis before the fish is served raw or lightly cooked. When that step is skipped, parasites can survive and cause illness in customers who have no way of knowing the fish was not properly treated.
Toxic chemical storage violations at Hwy 55 Burgers, Bamboo Wok, Sake Sushi, Bono's Pit BBQ, and Pepe's Hacienda represent a different category of risk. Cleaning chemicals stored near or above food preparation surfaces can contaminate food directly through spills, splashes, or mislabeled containers. Bono's Pit BBQ carried two separate chemical-related high-severity citations in the same inspection, one for improper storage and one for improper identification and use.
The Longer Record
La Nopalera Mexican Restaurant has 69 prior inspections on record, the longest history of any facility in this week's data. That volume of inspections across years of operation makes a single high-severity violation look modest by comparison, but the employee illness reporting failure it drew this week is the same violation type that appears in the records of restaurants with far more serious findings.
Juicy Crab carries 41 prior inspections and drew nine high-severity violations this week, including food from unapproved sources and missing shellfish traceability records. Señoritas Mexican Kitchen and Olympia Cafe each have 39 prior inspections on record. Señoritas drew 10 high-severity violations this week. Olympia drew eight, including parasite destruction failures. Facilities with that many inspections behind them are not encountering these standards for the first time.
Rosalia's Italian Restaurant and Pizzeria and Spice Grill Indian Cuisine each have 32 prior inspections. Both drew nine or more high-severity violations this week. Taco Way has 34 prior inspections and drew nine high-severity violations plus six intermediate ones, the highest combined tally of the week.
At the opposite end of the history spectrum, Pepe's Hacienda and Restaurant has only three prior inspections on record, making this week's eight high-severity violations notable for how quickly they accumulated. Pupuseria y Restaurante Maria has 13 prior inspections and drew eight high-severity violations, including food contaminated by hazards. Hwy 55 Burgers has 11 prior inspections and drew nine high-severity violations, among them food not cooked to minimum temperature and improperly stored toxic chemicals.
Pepe's Hacienda's inspection history is short enough that this week's findings could reflect early-operation problems or a facility still building compliance habits. What the record does not yet show is whether those eight violations will be corrected before a fourth inspection arrives.
