FORT MYERS, FL. A sushi restaurant on Cleveland Avenue that has been inspected 43 times by state regulators was cited again this week for failing to properly clean food contact surfaces, a violation that creates a direct bacterial transfer route between raw fish and every plate that follows.

Ginza Sushi was not alone. Fifteen Fort Myers restaurants drew high-severity citations in a single week, a volume that touched nearly every corridor of the city, from Palm Beach Boulevard to San Carlos Boulevard to South Tamiami Trail.
The Violations
Sip and Sizzle at 2236 First St and Auto Inn Tavern and Restaurant at 4805 S Cleveland Ave each drew five high-severity violations, the highest single-facility count of the week. Sip and Sizzle's citations included food from an unapproved or unknown source, no employee health policy, improperly cleaned food contact surfaces, improper handwashing technique, and no consumer advisory for raw or undercooked foods.
Auto Inn Tavern's five violations included food from an unapproved source and food not cooked to the required minimum temperature. That second citation is among the most direct pathogen risks in the food code: Salmonella in poultry survives below 165 degrees Fahrenheit, and undercooking is a leading documented cause of foodborne illness outbreaks. The inspector also noted no person in charge present, inadequate handwashing facilities, and improper handwashing technique.
Poke Hawaii at 3242 Forum Blvd drew a violation that is specific to raw-fish restaurants: parasite destruction procedures not followed. Without proper freezing protocols, parasites including Anisakis and tapeworm can survive in raw fish served directly to customers. Fortune Cookie at 4017 Palm Beach Blvd was cited for four high-severity violations, including no allergen awareness demonstrated. Angel Restaurant and Jerk Center at 3380 Dr Martin Luther King Blvd combined three high-severity violations with three intermediate ones. The high-severity citations included no person in charge, employees not reporting illness symptoms, and toxic chemicals improperly stored or labeled. The intermediate violations included improper sewage or wastewater disposal, a citation that signals risk of fecal contamination spreading through the facility.
Tiki Subs and Mexican Place Inc at 16970 San Carlos Blvd and Martis Family Dining at 16200 San Carlos Blvd both drew improper sewage or wastewater disposal citations as intermediate violations, alongside high-severity findings that included time-as-public-health-control misuse and employees not reporting illness symptoms at Martis Family Dining.
Ceviches by Divino at 7950 Dani Dr was cited for no employee health policy, improperly cleaned food contact surfaces, and no consumer advisory, along with improper sewage disposal as an intermediate violation. A restaurant specializing in ceviche, a dish that relies on acid rather than heat to address raw seafood, without a consumer advisory leaves customers with no disclosure of the inherent risks of that preparation method.
Sandbar and Grille at 4606 S Cleveland Ave drew two high-severity citations: food from an unapproved or unknown source and improperly cleaned food contact surfaces. Matt's Red Hots at 18911 S Tamiami Trail was cited for improperly cleaned food contact surfaces, no consumer advisory, and toxic chemicals improperly stored or labeled. Seven Lakes Tee Room at 1750 Augusta Dr drew citations for improper handwashing technique, improperly cleaned food contact surfaces, and no allergen awareness demonstrated.
What These Violations Mean
The food-from-unapproved-source citation appeared at four facilities this week: Sip and Sizzle, Auto Inn Tavern, Sandbar and Grille, and Poke Hawaii. Sandbar and Grille has 37 prior inspections on record. Matt's Red Hots has 36. Beef O' Brady's Verandah has 31, and Martis Family Dining also carries 31. Each has a substantial inspection record against which this week's high-severity findings sit as continuing pattern rather than isolated incident.
