FORT MYERS, FL. Lenny's Grill and Subs on South Cleveland Avenue drew nine high-severity violations in a single inspection this week, the highest count among 15 Fort Myers restaurants flagged between July 21 and July 27, 2026, with inspectors citing toxic chemicals improperly stored near food, no shellfish traceability records, and no consumer advisory for raw or undercooked items.
The violations at Lenny's stretched across nearly every category inspectors use to assess a facility's safety culture. There was no person in charge present or performing duties, no written employee health policy, and documented failures in hand-washing technique. Inspectors also found that parasite destruction procedures for fish were not being followed.
Parasite destruction requires fish served raw or undercooked to be frozen at specific temperatures for specific durations before service. Without that step, parasites including Anisakis and tapeworm can survive and reach a customer's plate.
The Violations
Token Ramen on Dani Drive was the second-most cited facility this week, with eight high-severity violations. Inspectors found employees not reporting illness symptoms, food in poor or adulterated condition, missing shellfish identification records, and parasite destruction procedures not being followed. Toxic substances were also improperly stored or identified.
Token Ramen sits on the same stretch of Dani Drive as Lis China King, which drew five high-severity violations at the same address block. Inspectors at Lis China King found an employee not reporting illness symptoms, food in poor condition, and an intermediate violation for improper sewage or wastewater disposal.
El Local on South Tamiami Trail was cited for seven high-severity violations, including food sourced from an unapproved or unknown supplier. That citation appeared alongside inadequate shellfish traceability records, no consumer advisory for raw or undercooked foods, and a failure to properly use time as a public health control.
Banh Mi and Tea on College Parkway also drew seven high-severity violations. Inspectors documented inadequate handwashing by food employees, no allergen awareness demonstrated, toxic substances improperly stored, and an intermediate violation for improper sewage or wastewater disposal.
The allergen finding at Banh Mi and Tea is notable. Food allergies affect roughly 32 million Americans, and when a facility cannot demonstrate allergen awareness, customers with life-threatening allergies to peanuts, shellfish, or gluten have no reliable way to assess their risk before ordering.
Cadeau Cafe on Metro Parkway was flagged for six high-severity violations, including food sourced from an unapproved supplier, food in poor condition, and parasite destruction procedures not followed. Inspectors also found improper sewage or wastewater disposal as an intermediate violation.
Panda Express at the Six Mile Cypress Parkway location drew five high-severity violations, including food not cooked to the required minimum temperature. That is among the most direct pathogen risks in a kitchen: Salmonella in poultry survives below 165 degrees Fahrenheit, and the chain's menu is built around cooked chicken and beef dishes.
Shoeless Joe's Sports Cafe on Bell Tower Drive was cited for five high-severity violations, including no allergen awareness demonstrated and no consumer advisory for raw or undercooked foods. Both gaps leave customers without the information they need to make safe choices.
GameTime on Gulf Center Drive drew five high-severity violations, including food from an unapproved or unknown source and toxic substances improperly identified or stored. An intermediate violation for improper sewage or wastewater disposal was also recorded.
Vuelve a la Vida Seafood Restaurant on Boy Scout Drive was cited for four high-severity violations. A seafood-focused restaurant with no allergen awareness documented and improperly cleaned food contact surfaces presents a compounded risk for customers with shellfish allergies.
India's Grill on South Cleveland Avenue drew four high-severity violations, including no employee health policy and no consumer advisory for raw or undercooked foods. Inadequate cooling equipment was cited as an intermediate violation.
Dave's Hot Chicken on Dani Drive was flagged for four high-severity violations including an employee not reporting illness symptoms and no consumer advisory for raw or undercooked foods. Improperly cleaned food contact surfaces rounded out the high-severity findings.
Wendy's on Cleveland Avenue drew four high-severity violations, including toxic chemicals improperly stored or labeled and no allergen awareness demonstrated.
Grimaldi's Pizzeria on Cleveland Avenue was cited for three high-severity violations, including food from an unapproved source and no allergen awareness. Inadequate cooling equipment was flagged as an intermediate concern.
Orange Pepper on College Parkway drew no high-severity violations, recording only a single intermediate citation for inadequate ventilation and lighting.
What These Violations Mean
The most widespread high-severity finding this week was improperly cleaned food contact surfaces, cited at Lenny's Grill and Subs, Token Ramen, El Local, Banh Mi and Tea, Cadeau Cafe, Panda Express, Shoeless Joe's, GameTime, Vuelve a la Vida, India's Grill, Dave's Hot Chicken, and Wendy's. Cutting boards, prep tables, and slicers that are not properly sanitized between uses become direct transfer points for bacteria including Salmonella, E. coli, and Listeria. The surface looks clean. The contamination is microscopic.
The second most common finding was the absence of a written employee health policy or employees not reporting illness symptoms, documented at Lenny's, Lis China King, Token Ramen, Banh Mi and Tea, GameTime, Vuelve a la Vida, India's Grill, and Dave's Hot Chicken. Norovirus, the most common cause of foodborne illness outbreaks in the United States, spreads most efficiently when a symptomatic employee handles food. A written policy is not paperwork for its own sake. It is the mechanism that keeps a sick worker out of the kitchen.
Food from unapproved or unknown sources was cited at El Local, Cadeau Cafe, GameTime, and Grimaldi's Pizzeria. When food bypasses federally inspected supply chains, there is no traceability if customers become ill. An outbreak linked to an uninspected supplier can take investigators weeks to trace, during which time the source continues reaching customers.
Improper sewage or wastewater disposal, flagged at Lis China King, Banh Mi and Tea, Cadeau Cafe, and GameTime, carries a risk that most customers would not associate with a restaurant visit. Raw sewage contains pathogens including Hepatitis A and E. coli O157:H7. A drainage failure or improperly routed waste line can introduce fecal contamination into food prep areas without any visible sign.
The Longer Record
Vuelve a la Vida Seafood Restaurant carries the longest inspection history of any facility flagged this week, with 47 prior inspections on record. Four high-severity violations in the most recent visit, including no allergen awareness at a seafood restaurant, represent a significant finding at a location that has been reviewed by state inspectors more than any other on this list.
El Local has 33 prior inspections on record and drew seven high-severity violations this week, including food from an unapproved source. Grimaldi's Pizzeria has 32 prior inspections and was also cited for food from an unapproved source. GameTime has 31 prior inspections and drew the same violation. Three facilities with a combined 96 prior inspections all arrived at the same unapproved sourcing citation in the same week.
India's Grill and Shoeless Joe's Sports Cafe each carry 31 and 26 prior inspections respectively, and both were cited for no consumer advisory for raw or undercooked foods. Panda Express has 29 prior inspections and was cited for food not reaching minimum cooking temperatures, a finding that carries particular weight at a chain with that volume of prior review.
At the other end of the history scale, Dave's Hot Chicken has only two prior inspections on record and already carries four high-severity violations including an employee illness reporting failure. Banh Mi and Tea has five prior inspections and drew seven high-severity violations this week, including improper sewage disposal and no allergen awareness. Both locations are relatively new to the inspection record and both are already accumulating serious citations.
Vuelve a la Vida's 47 inspections and its continued failure to demonstrate allergen awareness at a restaurant whose entire menu centers on seafood remains the unresolved fact at the end of this week's record.
