FORT MYERS, FL. A Mexican restaurant on Cleveland Avenue walked into this week's inspections carrying 30 prior visits from state inspectors and still managed to rack up eight high-severity violations in a single visit, the most of any facility cited in Fort Myers during the week of August 2 through August 8, 2026.
The Worst of the Week
Monarcas Authentic Mexican Cuisine Bar and Grill at 4125 Cleveland Ave drew citations covering nearly every layer of food safety: no person in charge performing duties, an employee not reporting illness symptoms, inadequate handwashing facilities, food contact surfaces not properly cleaned or sanitized, food not cooked to required minimum temperatures, no consumer advisory for raw or undercooked items, toxic substances improperly stored, and no allergen awareness demonstrated.
That combination, an absent or non-functioning manager alongside sick employees who aren't required to report symptoms and surfaces that aren't being sanitized, is the inspection equivalent of every warning light on a dashboard coming on at once.
Los Molcajetes at 2847 Cleveland Ave was cited for seven high-severity violations, including no employee health policy, an employee not reporting illness symptoms, improper hand-washing technique, and food described as being in poor condition, mislabeled, or adulterated. The facility also had no person in charge performing duties and food contact surfaces not properly cleaned. Inspectors noted improperly cleaned multi-use utensils as an intermediate violation.
Nomiki's Plakka at 12901 McGregor Blvd drew six high-severity citations, among them inadequate handwashing facilities, food contact surfaces not properly cleaned, toxic chemicals improperly stored or labeled, and no allergen awareness. Inspectors also flagged improper sewage or wastewater disposal as an intermediate violation, meaning waste removal systems at the facility were not functioning as required.
Handwashing, Illness Policies, and Parasites
Flock Wine Bistro at 9405 Cypress Lake Dr was cited for six high-severity violations with no intermediate violations at all. The list included an employee not reporting illness symptoms, inadequate handwashing by food employees, improper hand and arm washing technique, food in poor condition or adulterated, and a citation for failing to follow parasite destruction procedures. That last violation means the facility was serving fish, pork, or game meat without the required freezing or cooking protocols to kill parasites including Anisakis and tapeworm.
Kumo Six Mile at 7950 Dani Dr was cited for five high-severity violations, including improper handwashing technique, food contact surfaces not properly cleaned, food not cooked to required minimum temperatures, and toxic substances improperly stored. Inspectors also noted improper sewage or wastewater disposal and equipment in poor repair as intermediate violations.
Begonia's Restaurant and Pizzeria Inc. at 2215 Winkler Ave drew five high-severity violations, including inadequate shell stock identification and records. That citation means the restaurant could not document where its shellfish, oysters, clams, or mussels, came from. Without that paperwork, there is no way to trace the source if a customer becomes ill. Inspectors also cited improper handwashing technique, food contact surfaces not properly cleaned, and toxic chemicals improperly stored.
House of Omelets at 14261 S Tamiami Trail was cited for five high-severity violations including no person in charge, no employee health policy, an employee not reporting illness symptoms, improper handwashing technique, and toxic chemicals improperly stored. Inspectors additionally noted single-use items being improperly reused and inadequate toilet facilities.
Pirate Hook Seafood and Chicken at 4065 Fowler St was cited for four high-severity violations, including failure to follow parasite destruction procedures and no consumer advisory for raw or undercooked foods. A seafood restaurant without either of those safeguards in place offers customers no protection and no warning.
Pita Fast Fresh Mediterranean at 8595 College Pkwy drew four high-severity violations, including improper use of time as a public health control. That citation means food was being held in the temperature danger zone, between 41 and 135 degrees, without the required written procedures documenting how long it had been there. Inspectors also cited food contact surfaces not properly cleaned and no consumer advisory.
Steak n Shake at 8950 Colonial Center Dr was cited for four high-severity violations: food not cooked to required minimum temperature, no consumer advisory for raw or undercooked foods, toxic chemicals improperly stored, and no allergen awareness demonstrated. A national chain with 28 prior inspections on record in this location.
Ninja Thai and Sushi Bar at 13550 Reflection Pkwy was cited for two high-severity violations, food contact surfaces not properly cleaned and toxic chemicals improperly stored. First Watch Restaurant No. 96 at 2059 Altamont Ave drew three high-severity violations, including inadequate shell stock identification and records and toxic substances improperly stored.
Duffy's Sports Grill at 13721 S Tamiami Trail and Longhorn Steakhouse at 13701 S Tamiami Trail, located less than a block apart on the same road, were each cited for two high-severity violations. Duffy's drew inadequate shell stock identification and no consumer advisory. Longhorn was cited for an employee not reporting illness symptoms and toxic chemicals improperly stored. Panaderia y Restaurante La Bamba Inc. at 3900 Palm Beach Blvd drew one high-severity violation, no consumer advisory for raw or undercooked foods.
What These Violations Mean
The most common high-severity citation this week was the absence of an employee illness reporting policy or an employee who was not reporting symptoms. That violation appeared at Monarcas, Los Molcajetes, Flock Wine Bistro, House of Omelets, First Watch, and Longhorn Steakhouse. Norovirus spreads through a single sick food worker handling ready-to-eat food, and it can sicken dozens of customers from a single shift. A written policy that requires employees to report symptoms before they come to work is the first and most effective barrier. When that policy doesn't exist, there is no barrier.
Improper handwashing technique, cited at Los Molcajetes, Flock Wine Bistro, Kumo Six Mile, Begonia's, House of Omelets, and Pirate Hook, is distinct from simply not washing hands. Studies show that incomplete technique, skipping steps, not scrubbing long enough, or rinsing without soap, leaves enough pathogens on hands to contaminate every surface and food item touched afterward. The citation documents that employees were making an attempt and still getting it wrong.
The parasite destruction citations at Flock Wine Bistro and Pirate Hook Seafood and Chicken carry a specific risk that temperature violations do not. Parasites including Anisakis in fish and Trichinella in pork are not killed by brief exposure to heat. They require either sustained cooking to required temperatures or a documented freezing protocol. Without those steps, a customer eating undercooked fish or pork is consuming a living organism.
Toxic chemicals improperly stored or labeled appeared as a high-severity violation at Nomiki's Plakka, Kumo Six Mile, Begonia's, House of Omelets, Ninja Thai, Steak n Shake, and Longhorn Steakhouse. That is seven of the fifteen facilities cited this week. When cleaning chemicals are stored near or above food or are unlabeled, accidental contamination is not a remote possibility.
The Longer Record
Kumo Six Mile and Begonia's Restaurant and Pizzeria both carry 38 prior inspections on record, the highest count among facilities cited this week. Both were still cited for five or more high-severity violations in August 2026. Nomiki's Plakka, with 35 prior inspections, drew six high-severity violations including sewage disposal problems and toxic chemical storage. A facility that has been inspected three dozen times and is still generating citations in the same core categories, sanitation, chemical storage, handwashing infrastructure, is not trending toward compliance.
Monarcas, with 30 prior inspections, produced eight high-severity violations this week, the week's highest total. Ninja Thai and Sushi Bar carries 29 prior inspections and was cited for two high-severity violations. Steak n Shake at Colonial Center Drive has 28 prior inspections and drew four high-severity citations including a cooking temperature failure and no allergen awareness. Longhorn Steakhouse has 26 prior inspections on record.
Los Molcajetes, by contrast, had only 10 prior inspections on record when it was cited for seven high-severity violations this week. That is the second-highest violation count of the week at a facility that is relatively early in its inspection history. Flock Wine Bistro had 22 prior inspections and drew six high-severity violations including the parasite destruction failure.
House of Omelets and Pita Fast Fresh Mediterranean each carry 25 prior inspections. Both were cited this week for multiple failures in the same foundational categories: illness policies, handwashing, and surface sanitation. Panaderia y Restaurante La Bamba, with 17 prior inspections, drew only one high-severity violation, but its cooling equipment was flagged as inadequate, a condition that does not resolve itself between visits.
