FORT MYERS, FL. Kumo Six Mile on Dani Drive drew the most serious inspection findings in Fort Myers during the week of July 28, accumulating 10 high-severity violations in a single visit, including food from unapproved or unknown sources, inadequate shell stock identification records, food not cooked to required minimum temperature, and no person in charge present or performing duties.

Eight other restaurants in the city also drew high-severity citations during the same seven-day stretch, bringing the week's total to 34 high-severity violations across nine facilities.

The Kumo Six Mile inspection also documented inadequate handwashing facilities, improper hand and arm washing technique, and food contact surfaces not properly cleaned or sanitized. That combination, no responsible manager, no functional handwashing infrastructure, improper technique even when washing was attempted, and unverified food sourcing, represents a compounding set of failures rather than isolated oversights.

Hot Pot and Korean BBQ on 6 Mile Cypress Parkway drew four high-severity violations, including the same combination of no person in charge, employees not reporting illness symptoms, and food contact surfaces not properly cleaned or sanitized. Inspectors also cited improper hand and arm washing technique and, at the intermediate level, multi-use utensils not properly cleaned.

Keke's Breakfast Cafe on Daniels Parkway drew four high-severity violations centered on a different cluster of risks. Inspectors cited inadequate shell stock identification records, food contact surfaces not properly cleaned or sanitized, no consumer advisory posted for raw or undercooked foods, and no allergen awareness demonstrated by staff.

Wendy's #12397 on Dani Drive was cited for food not cooked to required minimum temperature, employees not reporting illness symptoms, and toxic substances improperly identified, stored, or used. Intermediate violations included improper sewage or wastewater disposal, inadequate ventilation and lighting, and improper use of wiping cloths.

Mikuu Ramen on Dani Drive drew three high-severity violations: employees not reporting illness symptoms, food in poor condition or adulterated, and toxic substances improperly identified, stored, or used. An intermediate violation for improper sewage or wastewater disposal was also documented.

Red Lobster #0079 on Cleveland Avenue was cited for employees not reporting illness symptoms, improper hand and arm washing technique, and toxic substances improperly identified, stored, or used. An intermediate citation for improper sewage or wastewater disposal accompanied those findings.

Cooper's Hawk Winery and Restaurant on Olympic Court drew three high-severity violations: no person in charge, employees not reporting illness symptoms, and food from unapproved or unknown sources. Cooper's Hawk had only 11 prior inspections on record, making this a relatively early entry in its inspection history.

Dairy Queen on Colonial Boulevard was cited for no person in charge and food contact surfaces not properly cleaned or sanitized, along with intermediate violations for improper sewage or wastewater disposal and multi-use utensils not properly cleaned.

Taco Bell #042849 on Palm Beach Boulevard drew two high-severity violations, both chemical-related: toxic chemicals improperly stored or labeled, and toxic substances improperly identified, stored, or used. An intermediate citation for improper waste disposal or recycling was also documented.

What These Violations Mean

The employee illness reporting violation, cited at Kumo Six Mile, Hot Pot and Korean BBQ, Wendy's, Mikuu Ramen, Red Lobster, and Cooper's Hawk, is among the most direct outbreak pathways regulators track. Norovirus and Salmonella spread rapidly when symptomatic employees continue handling food, and a single infected worker can expose dozens of customers before any illness is reported. Six of nine facilities cited this week had this violation.

The food from unapproved or unknown source violations at Kumo Six Mile and Cooper's Hawk carry a different kind of risk. Food that enters a restaurant outside the licensed supply chain bypasses USDA and FDA safety inspections entirely. If a customer gets sick, investigators have no chain of custody to trace. At a sushi bar like Kumo Six Mile, which also had inadequate shell stock records, the traceability gap is compounded: shellfish consumed raw or lightly cooked are among the highest-risk foods in any kitchen, and without proper tagging records, there is no way to link a specific harvest lot to an illness.

The toxic substance violations at Wendy's, Mikuu Ramen, Red Lobster, and Taco Bell represent a separate category of harm entirely. Chemical contamination from improperly stored or unlabeled cleaning agents or pesticides can cause acute poisoning with no warning signs before consumption. Taco Bell drew two separate citations in this category, one for toxic chemicals improperly stored or labeled and one for toxic substances improperly identified, stored, or used.

The no-allergen-awareness finding at Keke's Breakfast Cafe is particularly notable for a breakfast-focused chain. Food allergies send roughly 30,000 Americans to emergency rooms each year, and a staff that cannot identify or communicate allergen risks in dishes removes the last line of defense for customers managing life-threatening sensitivities.

The Longer Record

Kumo Six Mile has 37 prior inspections on record, the most of any facility cited this week. Ten high-severity violations in a single visit, after 37 previous inspections, means these problems are not attributable to a new or inexperienced operation still learning compliance. The facility has been visited by state inspectors more than three dozen times.

Red Lobster #0079 on Cleveland Avenue has 30 prior inspections on record and drew three high-severity violations this week, including the employee illness reporting failure. Dairy Queen on Colonial Boulevard has 28 prior inspections on record and was still cited for no person in charge and unsanitized food contact surfaces.

Taco Bell #042849 has 23 prior inspections behind it. Both of its high-severity violations this week were chemical-related, a category that should be addressed in basic food handler training and does not require a complex kitchen operation to resolve.

Keke's Breakfast Cafe and Mikuu Ramen each have 20 prior inspections on record. Keke's allergen awareness and consumer advisory failures are not obscure regulatory technicalities; they are required disclosures that protect identifiable, vulnerable customer populations. Wendy's #12397 has 15 prior inspections on record and was still cited for food not cooked to required minimum temperature, the kind of violation that food safety training programs address as a foundational requirement.

Hot Pot and Korean BBQ and Cooper's Hawk each have 11 prior inspections on record. Cooper's Hawk drew a food from unapproved source violation on what is, by inspection count, still a relatively young compliance history. Whether that pattern continues is what the next inspection will show.

The Pattern

Seven of the nine facilities cited this week had been inspected at least 11 times before. The violations documented, unsanitized food contact surfaces, no manager present, employees not reporting illness, improperly stored chemicals, are not findings that require specialized knowledge to prevent. They are the foundational requirements of licensed food service.

Kumo Six Mile's 10 high-severity violations remain the open question from this week's inspections. The facility drew citations in five distinct risk categories, management, hygiene infrastructure, sourcing, traceability, and cooking temperature, in a single visit, and its inspection file now spans 37 entries.
