FORT MYERS, FL. Pho Bowl on Metro Parkway drew the most serious inspection findings of any Fort Myers restaurant the week of August 19, accumulating eight high-severity violations that included food from unapproved sources, no shellfish traceability records, improperly cleaned food contact surfaces, and a complete absence of allergen awareness among staff. Fourteen other restaurants in Lee County were cited for high-severity violations during the same seven-day stretch.

Pho Bowl's violations went beyond paperwork failures. Inspectors cited the restaurant for food in poor condition or adulterated, inadequate shellfish identification records, and no demonstrated allergen awareness, alongside a missing employee health policy and an employee not reporting illness symptoms. The combination of an unknown food source and no allergen training creates a scenario where a customer with a severe allergy cannot get a reliable answer about what is actually in the food.

Viet Kitchen on Cleveland Avenue matched Pho Bowl's count of illness-reporting failures and added a violation inspectors flagged for improper sewage or wastewater disposal, a finding that places fecal contamination risk throughout the facility. Inspectors also cited the restaurant for food not cooked to required minimum temperature and for failing to follow required procedures for specialized processes.

Fortune Cookie on Palm Beach Boulevard was cited for seven high-severity violations including no consumer advisory for raw or undercooked foods, no allergen awareness, and inadequate cooling or cold holding equipment. A restaurant serving food without a functioning cold holding system and without telling customers which items are served raw or undercooked presents layered risk.

Nick and Stellas Pizza Perfecto on McGregor Boulevard drew citations for food from an unapproved source, inadequate shellfish identification records, food not cooked to minimum temperature, and no consumer advisory for raw or undercooked foods. Inspectors also noted inadequate handwashing facilities, meaning even a motivated employee had no proper place to wash up.

Pascal Restaurant on Simpson Street was cited for food in poor condition or adulterated, food not cooked to required minimum temperature, no allergen awareness, and inadequate handwashing facilities. The facility also lacked adequate cooling equipment, the same infrastructure failure found at Fortune Cookie.

Management Absent, Policies Missing

Sandbar and Grille on South Cleveland Avenue was the only facility this week where inspectors documented the person in charge as not present or not performing duties. That violation accompanied six others: no employee health policy, an employee not reporting illness symptoms, inadequate handwashing facilities, food from an unapproved source, improperly cleaned food contact surfaces, and no consumer advisory for raw or undercooked foods.

Auto Inn Tavern and Restaurant on South Cleveland Avenue also drew a citation for no person in charge, paired with inadequate handwashing facilities, improper handwashing technique, food from an unapproved source, and food not cooked to minimum temperature. Five high-severity violations at a facility where neither adequate management nor adequate hand hygiene infrastructure was present.

LYNQ on Summerlin Road racked up six high-severity violations including food not cooked to required minimum temperature, no consumer advisory for raw or undercooked foods, and no allergen awareness. Inspectors also cited inadequate toilet facilities, a condition that discourages proper restroom hygiene among staff.

Eggcetera, also on Summerlin Road, was cited for five high-severity violations including food in poor condition or adulterated and improper use of time as a public health control. When a facility uses time rather than temperature to keep food safe, it must follow precise written procedures. Inspectors found those procedures were not being properly followed.

Cibo on McGregor Boulevard drew five high-severity violations with no intermediate violations, a pattern that points to serious foundational failures rather than operational sloppiness. Those violations included food from an unapproved source, inadequate shellfish records, food not cooked to minimum temperature, and toxic substances improperly identified, stored, or used.

Iguana Mia on South Cleveland Avenue was cited for toxic substances improperly identified, stored, or used, alongside no consumer advisory for raw or undercooked foods and improper handwashing technique. Saigon Paris Bistro on South Cleveland Avenue drew a similar chemical storage citation, toxic chemicals improperly stored or labeled, in addition to improperly cleaned food contact surfaces and no employee health policy.

O'Leary's Bar and Grill on Summerlin Road was cited for four high-severity violations including improperly cleaned food contact surfaces, improper handwashing technique, and no consumer advisory. Hilton Garden Inn Fort Myers Airport on Corporate Commerce Way drew three high-severity violations, including inadequate handwashing facilities and improper use of time as a public health control. Bada Bing Pizza Co on Gladiolus Drive closed out the week with two high-severity violations, no employee health policy and no allergen awareness, alongside three intermediate citations.

What These Violations Mean

The single most common high-severity violation this week was the absence of an employee health policy, cited at Pho Bowl, Viet Kitchen, Fortune Cookie, Nick and Stellas, Pascal Restaurant, Sandbar and Grille, Saigon Paris Bistro, and Bada Bing Pizza Co. Without a written policy, there is no mechanism to keep a sick worker out of the kitchen. Norovirus, which causes roughly 20 million illnesses annually in the United States, spreads directly from infected food handlers to customers through contaminated food.

Food from unapproved or unknown sources, cited at Pho Bowl, Viet Kitchen, Nick and Stellas, Sandbar and Grille, Auto Inn Tavern, and Cibo, carries a specific danger: there is no traceability. If a customer gets sick after eating at one of these restaurants, investigators cannot trace the food back through the supply chain to identify a contaminated batch or issue a recall. That traceability gap is what turns a single illness into an outbreak.

The shellfish records violations at Pho Bowl, Nick and Stellas, and Cibo compound that traceability problem. Oysters, clams, and mussels are frequently consumed raw or lightly cooked, and shellfish harvested from contaminated waters carry Vibrio and hepatitis A. The identification tags on shellfish shipments are the only way to trace a contaminated batch back to its harvest site. Without those records, there is nothing to trace.

Toxic substance violations at Cibo, Iguana Mia, and Saigon Paris Bistro represent a different category of risk entirely. A cleaning chemical stored near food, or an unlabeled container mistaken for a food ingredient, can cause acute poisoning with no warning. These are not slow-developing bacterial risks. They are immediate.

The Longer Record

Iguana Mia has 41 prior inspections on record, the longest history of any facility cited this week, and still drew four high-severity violations including a toxic substance handling failure. Sandbar and Grille, with 35 prior inspections, was cited for seven high-severity violations including an absent person in charge and food from an unapproved source. Saigon Paris Bistro, with 34 prior inspections, was cited for chemical storage failures this week.

Pho Bowl and Nick and Stellas each carry 32 prior inspections. Fortune Cookie has 31. That Pho Bowl is accumulating eight high-severity violations at inspection number 32, including food from unapproved sources and no allergen awareness, suggests these are not new problems being corrected over time.

Cibo, with 28 prior inspections, drew five high-severity violations and zero intermediate violations this week. Pascal Restaurant, with 24 inspections on record, still lacked adequate cooling equipment and handwashing facilities.

Auto Inn Tavern and Restaurant stands out from the opposite direction. With only seven prior inspections on record, it was already drawing five high-severity violations this week, including no person in charge and food from an unapproved source. Bada Bing Pizza Co, with 13 inspections on record, had no allergen awareness and no employee health policy.

Iguana Mia's 41 inspections without resolving toxic substance handling is the longest unresolved pattern in this week's data.
