FORT LAUDERDALE, FL. Five Fort Lauderdale restaurants were cited in a single week for receiving food from unapproved or unknown sources, a violation that state inspectors flagged at locations ranging from a nitrogen ice cream shop on Federal Highway to a hotel restaurant serving cruise port travelers, according to records from the week of August 26 through September 1, 2026.

Pura Vida Miami on North Federal Highway and Chill N Nitrogen Ice Cream at 2422 N Federal Highway each drew five high-severity violations, the highest totals of the week. Both locations sit within a half-mile of each other on the same stretch of Federal Highway.

At Pura Vida Miami, inspectors documented five separate high-severity failures in a single visit. Those included food from an unapproved or unknown source, an employee not reporting symptoms of illness, food not cooked to the required minimum temperature, toxic substances improperly stored or used, and a failure to properly apply time as a public health control.

The toxic substance citation at Pura Vida is among the most acute violations in this week's data. Improper storage of chemicals near food creates an immediate risk of contamination that cannot be detected by taste or smell before a customer is harmed.

Chill N Nitrogen Ice Cream drew its five high-severity citations across a different set of failures. Inspectors found no person in charge present or performing duties, an employee not reporting illness symptoms, inadequate shell stock identification records, no allergen awareness demonstrated, and required procedures for specialized processes not followed. For a concept built around liquid nitrogen, a specialized and potentially hazardous process, the citation for failing to follow required procedures for specialized processes is particularly notable.

Offerdahls Off-the-Grill at 2122 NW 62 Street received four high-severity violations. Inspectors cited no person in charge, an employee not reporting illness symptoms, improper hand and arm washing technique, and time as a public health control not properly used. Three intermediate violations accompanied those findings, including multi-use utensils not properly cleaned and inadequate toilet facilities.

Cafe Roma Italy at 1 E Broward Blvd also drew four high-severity violations, including food from an unapproved source, no employee health policy, an employee not reporting illness symptoms, and improper handwashing technique. The combination of no written health policy and an employee actively not reporting symptoms represents a layered failure: the policy gap creates the environment in which the individual failure becomes likely.

Chili's Grill and Bar at 6302 NW 9 Ave was cited for four high-severity violations as well. Those included inadequate handwashing by food employees, food from an unapproved source, time as a public health control not properly used, and no consumer advisory posted for raw or undercooked menu items.

More Violations Across the City

Four Points by Sheraton Airport and Cruise Port at 1800 S Federal Highway received three high-severity violations: food from an unapproved source, time as a public health control not properly used, and no allergen awareness demonstrated. The hotel's restaurant serves travelers departing and arriving at Port Everglades, a population that includes guests who may be difficult to trace if a foodborne illness develops days after their stay.

Host Table and Tap at 1522 NE 4 Ave was cited for inadequate handwashing by food employees, food from an unapproved source, and food contact surfaces not properly cleaned or sanitized. All three are direct contamination pathways, meaning each one alone creates a route for pathogens to reach a customer's plate.

Checkers Drive-In at 400 E Sunrise Blvd drew three high-severity violations including toxic chemicals improperly stored or labeled, food from an unapproved source, and food contact surfaces not properly cleaned or sanitized. An intermediate citation for improper sewage or wastewater disposal rounded out the inspection.

Caffe Europa at 910 E Las Olas Blvd was cited for no person in charge, improper handwashing technique, and food not cooked to the required minimum temperature. The Las Olas corridor location draws heavy foot traffic from tourists and downtown workers throughout the week.

California Pizza Kitchen at 2301 N Federal Highway received three high-severity violations: inadequate shell stock identification records, food contact surfaces not properly cleaned or sanitized, and toxic substances improperly identified, stored, or used.

Jalisco Inc at 700 N Federal Highway was cited for an employee not reporting illness symptoms, no consumer advisory for raw or undercooked foods, and required procedures for specialized processes not followed.

Freda's Dominican Cuisine at 6868 Powerline Rd drew three high-severity violations including an employee not reporting illness symptoms, improper handwashing technique, and inadequate shell stock identification records.

The Whole Enchilada Fresh Mexican Grill at 745 N Federal Highway was cited for no person in charge, no employee health policy, and an employee not reporting illness symptoms. All three violations point to the same structural gap: no management framework ensuring that sick workers stay out of the kitchen.

Bokamper's at 3115 NE 32 Ave received two high-severity violations: food not cooked to the required minimum temperature, and no consumer advisory posted for raw or undercooked items. Louie Bossi's Ristorante Bar Pizzeria at 1022-1032 E Las Olas Blvd was cited for an employee not reporting illness symptoms and no consumer advisory for raw or undercooked foods.

What These Violations Mean

Food from an unapproved or unknown source, cited at five locations this week including Pura Vida Miami, Chili's, Checkers, Four Points by Sheraton, and Host Table and Tap, means that some portion of what was served to customers arrived outside the federally inspected supply chain. When food bypasses USDA or FDA oversight, there is no traceability record. If a customer becomes ill, investigators cannot identify the origin of the contaminated product or determine how many others were exposed.

The employee illness reporting failures documented at nine facilities this week, including Pura Vida Miami, Chill N Nitrogen Ice Cream, Offerdahls, Cafe Roma, Jalisco, Freda's, The Whole Enchilada, Louie Bossi's, and Freda's Dominican Cuisine, represent the single most common cause of multi-victim outbreaks. Norovirus, which can be transmitted by a single infected food worker touching a surface, causes symptoms that workers may dismiss as minor. Without a functioning reporting system, a sick employee has no mechanism prompting them to stay home.

The undercooking violations at Bokamper's, Pura Vida Miami, and Caffe Europa carry a specific biological consequence. Salmonella in poultry survives temperatures below 165 degrees Fahrenheit. A piece of chicken pulled from the heat too early looks and smells identical to one that reached safe temperature. The customer has no way to know the difference.

At Chill N Nitrogen Ice Cream, the citation for no allergen awareness demonstrated is not a paperwork issue. Food allergies send roughly 30,000 Americans to emergency rooms annually. A customer with a severe allergy to a specific ingredient who asks a staff member whether a dish is safe, and receives an uninformed answer, faces a direct physical risk.

The Longer Record

Jalisco Inc has accumulated 38 prior inspections on record, the longest history of any facility cited this week. This week's three high-severity violations, including the failure to follow required procedures for specialized processes, come after a history substantial enough to suggest that correction has not been sustained across inspection cycles.

Bokamper's has 34 prior inspections on record, and Louie Bossi's has 31. Both received consumer advisory violations this week, meaning neither posted the required notice to customers about the risks of raw or undercooked food. For two establishments with that depth of inspection history, the absence of a posted advisory is not a first-time oversight.

Cafe Roma Italy has 30 prior inspections and Caffe Europa has 29. Both drew handwashing violations this week. Caffe Europa was also cited for food not cooked to minimum temperature, a finding that appears at a location with nearly three dozen inspections behind it.

At the newer end of the spectrum, Freda's Dominican Cuisine has only 4 prior inspections on record and already carries three high-severity violations including shell stock traceability and illness reporting failures. Host Table and Tap has 9 prior inspections and drew three high-severity violations including unapproved food sourcing. Pura Vida Miami also has 9 prior inspections and led the week with five high-severity citations, including the toxic substance storage violation that inspectors had not previously documented at that location.

Freda's Dominican Cuisine, with four total inspections on record, had not yet established a pattern. After this week, it has one.
