FORT LAUDERDALE, FL. Michael's Cafe Bistro on NE 62nd Street led all Fort Lauderdale restaurants inspected the week of August 3 with seven high-severity violations, including food not cooked to required minimum temperatures, toxic chemicals improperly stored near food, and shellfish on hand without the identification records needed to trace an outbreak back to its source.

That was the highest single-facility count among 15 restaurants that drew high-severity citations during the week. One bar never made it to the inspection report: Sugar Bae at 8253 Sunset Strip was emergency-closed on August 7 for roach activity.

Koi Sushi Lounge and Hibachi on Cordova Road collected six high-severity violations, including one that stands out in any food service setting: inspectors found food from an unapproved or unknown source on the premises. The restaurant also had employees who were not reporting illness symptoms, inadequate handwashing facilities, and no proper records for shellfish on hand.

Bohemian Latin Grill on NE 32nd Street matched that count with six high-severity violations of its own. Three of those violations involved handwashing: employees not washing hands adequately, facilities that could not support proper hand hygiene, and improper washing technique. Inspectors also found no person in charge present and no system for employees to report illness.

Fresh Box Cafe on West Commercial Boulevard drew five high-severity citations, including a violation for required procedures for specialized food processes not being followed. The cafe also had no person in charge, no employee health policy, workers not reporting illness symptoms, and food held under time controls that were not being properly managed.

High Tides on Seabreeze Boulevard also received five high-severity violations. Two of them raise sourcing and safety questions: inspectors found food from an unapproved or unknown source, and toxic chemicals stored or labeled improperly. Employees were not washing hands adequately and no person in charge was present.

Larb BKK on North Federal Highway rounded out the five-violation tier. Inspectors found toxic chemicals improperly stored or labeled, no employee health policy, workers not reporting illness symptoms, improper handwashing technique, and untracked shellfish.

Down the List

Gyroville on North Andrews Avenue was cited for four high-severity violations including food contact surfaces not properly cleaned or sanitized and no consumer advisory posted for raw or undercooked menu items. Inspectors also noted improper waste disposal and inadequate toilet facilities.

Bodega Taqueria y Tequila on West Las Olas Boulevard drew four high-severity violations that spanned sourcing, cooking, and storage. Food arrived from an unapproved source, food was not cooked to required minimum temperatures, no consumer advisory was posted for raw or undercooked items, and toxic chemicals were improperly stored. Inspectors also cited improper sewage or wastewater disposal.

International House of Pancakes at 1101 North Federal Highway received four high-severity violations including no person in charge, employees not reporting illness symptoms, improperly stored chemicals, and no consumer advisory for undercooked foods.

Grano de Mostaza Restaurant on NW 31st Avenue was also cited for four high-severity violations: no employee health policy, improper handwashing technique, no consumer advisory for raw or undercooked foods, and toxic chemicals improperly stored or labeled.

Sheraton Suites Cypress Creek on NW 62nd Street received four high-severity citations covering hand hygiene and surface sanitation. Inspectors found employees not washing hands adequately, improper technique when washing did occur, food contact surfaces not properly cleaned or sanitized, and toxic chemicals improperly stored.

Dairy Queen Grill and Chill on North Federal Highway drew four high-severity violations including no person in charge, employees not reporting illness symptoms, improper handwashing technique, and inadequate shell stock identification records.

Don Carlin Restaurant on West Oakland Park Boulevard, Juicy Crab on North Andrews Avenue, and Ceviche Barrio by Antojitos to Go on Powerline Road each received three high-severity violations. Juicy Crab's included food not cooked to required minimum temperatures and inadequate shell stock records. Don Carlin's included toxic chemical storage and inadequate cooling equipment. Ceviche Barrio's included improperly cleaned food contact surfaces and improperly stored chemicals.

What These Violations Mean

The most urgent pattern this week involves employees working without any system for reporting illness. Koi Sushi Lounge, Bohemian Latin Grill, Fresh Box Cafe, High Tides, Larb BKK, Gyroville, the North Federal Highway IHOP, Dairy Queen, and Juicy Crab all received that citation. When a sick worker handles food with no policy requiring disclosure, a single Norovirus-infected employee can expose dozens of customers before anyone is aware there is a problem. Norovirus is the leading cause of foodborne illness in the United States, and direct person-to-food transmission is its primary route.

The food from unapproved sources violations at Koi Sushi Lounge, High Tides, and Bodega Taqueria y Tequila are a different category of concern. When food enters a kitchen through an uninspected channel, there is no documentation trail. If a customer gets sick and investigators need to trace the source, that paper trail does not exist. The food may never have been inspected for Listeria, Salmonella, or E. coli.

Undercooking violations at Michael's Cafe Bistro, Bodega Taqueria y Tequila, and Juicy Crab represent a direct pathogen survival risk. Salmonella in poultry survives below 165 degrees Fahrenheit. At a seafood-focused restaurant like Juicy Crab, the combination of undercooking and inadequate shell stock records means neither the source of the product nor the temperature at which it was served can be reliably established.

Toxic chemicals improperly stored or labeled appeared at Michael's Cafe Bistro, High Tides, Larb BKK, Bodega Taqueria y Tequila, the IHOP on North Federal, Grano de Mostaza, Sheraton Suites, Don Carlin, and Ceviche Barrio. Nine of the 15 flagged facilities had this violation. Chemicals stored near food or in unlabeled containers can cause acute poisoning through direct contamination, and the absence of labeling makes it impossible to identify the substance quickly if someone is harmed.

The Longer Record

Michael's Cafe Bistro enters this week's findings with 32 prior inspections on record, the highest count of any facility cited this week. Seven high-severity violations at a location with that inspection history is not a new operation finding its footing. The shellfish traceability problem and the undercooking citation at a restaurant with three decades of regulatory contact on file is a harder fact to explain away.

Koi Sushi Lounge and Bohemian Latin Grill each have more than 27 prior inspections recorded. Bohemian Latin Grill's three separate handwashing violations in a single inspection, at a location with 30 prior inspections, suggests the problem is structural rather than incidental.

Fresh Box Cafe presents the starkest contrast. With only nine prior inspections on record, it is among the newest facilities on this list, and it already carries five high-severity violations including a citation for failing to follow required procedures for specialized food processes. New operations accumulating serious citations early are a pattern regulators watch closely.

Sheraton Suites Cypress Creek, with 26 prior inspections, joins Dairy Queen Grill and Chill and Bodega Taqueria y Tequila at the same count. All three were cited this week for chemical storage violations. Larb BKK, with 27 prior inspections, also carries that citation alongside persistent handwashing and illness-reporting failures.

Sugar Bae at 8253 Sunset Strip was emergency-closed on August 7 for roach activity. No prior inspection count was provided for that location in state records available this week.
