FORT LAUDERDALE, FL. Michael's Cafe Bistro on NE 62nd Street racked up seven high-severity violations during the week of July 31, the most of any restaurant inspected in Fort Lauderdale that week, including a finding that food on the premises was in poor condition or adulterated, that shellstock identification records were inadequate, and that food was not cooked to the required minimum temperature.
That last violation carries a specific danger. Salmonella in poultry survives below 165 degrees Fahrenheit, meaning a single undercooked serving can trigger illness with as few as a handful of bacterial cells.
Michael's also drew citations for toxic chemicals stored improperly near food, food contact surfaces that were not properly cleaned or sanitized, improper handwashing technique, and the absence of any written employee health policy. Three intermediate violations accompanied the seven high-severity findings.
The Violations
Koi Sushi Lounge and Hibachi on Cordova Road tied for the second-highest count with six high-severity citations. The most alarming: inspectors found food from an unapproved or unknown source on the premises. At a restaurant serving raw fish, that finding means there is no way to trace where the seafood came from if a customer falls ill.
Koi also drew citations for employees not reporting illness symptoms, inadequate handwashing facilities, improper handwashing technique, inadequate shellstock identification records, and improper use of time as a public health control.
Bohemian Latin Grill on NE 32nd Street also logged six high-severity violations, with a finding that no person in charge was present or performing duties during the inspection. The CDC links that condition to three times more critical violations at a given establishment. The citations also included employees not reporting illness symptoms, inadequate handwashing by food employees, inadequate handwashing facilities, improper handwashing technique, and inadequate shellstock identification records.
Fresh Box Cafe on West Commercial Boulevard drew five high-severity violations, including one for required procedures for specialized processes not being followed. That citation covers practices like reduced-oxygen packaging and curing, processes that require precise controls to prevent botulism and Listeria growth. The cafe also had no person in charge present, no written employee health policy, employees not reporting illness, and improper use of time as a public health control.
High Tides on Seabreeze Boulevard was cited for food from an unapproved or unknown source, the same finding that appeared at Koi Sushi. Inspectors also noted no person in charge, employees not reporting illness, inadequate handwashing by food employees, and toxic chemicals stored improperly near food.
Larb BKK on North Federal Highway collected five high-severity citations including toxic chemicals improperly stored or labeled, inadequate shellstock identification records, no written employee health policy, employees not reporting illness symptoms, and improper handwashing technique.
Gyroville on North Andrews Avenue was cited for not posting a consumer advisory for raw or undercooked foods, meaning customers ordering anything served below minimum temperature have no way to know the risk. The four high-severity findings also included employees not reporting illness, food contact surfaces not properly cleaned or sanitized, and improper use of time as a public health control.
Bodega Taqueria y Tequila on West Las Olas Boulevard drew four high-severity violations including food from an unapproved or unknown source and food not cooked to the required minimum temperature, a pairing that represents the full arc of a foodborne illness risk: unknown origin, then insufficient heat to kill whatever pathogens may have arrived with it. No consumer advisory was posted, and chemicals were improperly stored.
International House of Pancakes at 1101 North Federal Highway was cited for no person in charge, employees not reporting illness, no consumer advisory for raw or undercooked foods, and toxic chemicals improperly stored or labeled.
Grano de Mostaza Restaurant on NW 31st Avenue drew four high-severity citations including no employee health policy, improper handwashing technique, no consumer advisory for raw or undercooked foods, and toxic chemicals improperly stored.
Sheraton Suites Cypress Creek on NW 62nd Street was cited for inadequate handwashing by food employees, improper handwashing technique, food contact surfaces not properly cleaned or sanitized, and toxic chemicals improperly stored, four high-severity violations at a hotel food service operation that serves a transient guest population with no regular relationship to the kitchen staff.
Juicy Crab on North Andrews Avenue was cited for inadequate shellstock identification records, food not cooked to the required minimum temperature, and employees not reporting illness symptoms. At a seafood restaurant where shellfish is a featured item, the absence of shellstock records is a direct gap in the traceability chain.
Ceviche Barrio by Antojitos to Go on Powerline Road drew three high-severity citations: improper handwashing technique, food contact surfaces not properly cleaned or sanitized, and toxic chemicals improperly stored.
Tree House Play and Cafe on Cordova Road was cited for food in poor condition or adulterated, food not cooked to the required minimum temperature, and no consumer advisory for raw or undercooked foods. The facility serves families with children.
Just 1 More on NE 33rd Avenue drew citations for inadequate handwashing by food employees, food in poor condition or adulterated, and food not cooked to the required minimum temperature.
What These Violations Mean
The most frequently cited violation this week was employees not reporting illness symptoms, which appeared at nine of the fifteen facilities, including Koi Sushi Lounge, Bohemian Latin Grill, Fresh Box Cafe, High Tides, Larb BKK, Gyroville, International House of Pancakes, and Juicy Crab. This is not a paperwork problem. Norovirus, the most common cause of foodborne illness in the United States, spreads through a single infected food worker touching ready-to-eat food. An ill employee who is not required by policy to stay home can infect dozens of customers before anyone connects the illness to the meal.
The unapproved food source citations at Koi Sushi Lounge, High Tides, and Bodega Taqueria y Tequila represent a different category of risk. When food arrives through an uninspected channel, there is no USDA or FDA record of how it was handled, stored, or processed. If a customer gets sick, investigators have no supply chain to trace.
The shellstock identification failures at Michael's Cafe Bistro, Koi Sushi Lounge, Bohemian Latin Grill, Larb BKK, and Juicy Crab are a specific version of that traceability problem. Oysters, clams, and mussels are typically eaten raw or lightly cooked. When shellstock tags are missing or records are incomplete, a single contaminated harvest lot cannot be identified and pulled before more people are served.
Toxic chemicals improperly stored near food, a violation found at Michael's Cafe Bistro, High Tides, Larb BKK, Bodega Taqueria y Tequila, International House of Pancakes, Grano de Mostaza, Sheraton Suites Cypress Creek, and Ceviche Barrio by Antojitos to Go, is the kind of violation that sounds administrative but is not. A cleaning chemical stored above a food prep surface can contaminate food through a single spill or accidental transfer, and chemical poisoning from that kind of exposure produces symptoms that are often misattributed to foodborne bacterial illness.
The Longer Record
Michael's Cafe Bistro has 32 prior inspections on record, the highest count among the fifteen facilities cited this week. Seven high-severity violations on the 32nd inspection is not the profile of a restaurant working through early-stage compliance issues.
Koi Sushi Lounge and Hibachi has 28 prior inspections. Bohemian Latin Grill has 30. Both have been through enough inspection cycles to have corrected the handwashing infrastructure and illness-reporting failures that appeared this week. Larb BKK, with 27 prior inspections, and Ceviche Barrio by Antojitos to Go, also with 27, show the same pattern: facilities with long inspection histories still drawing citations in the same foundational categories.
Bodega Taqueria y Tequila and Sheraton Suites Cypress Creek each have 26 prior inspections on record. The Sheraton finding is notable because hotel food service operations typically have more structured management oversight than independent restaurants. Four high-severity violations at a property on its 26th inspection, including both handwashing failures and improperly stored chemicals, suggests the oversight is not reaching the kitchen.
Fresh Box Cafe has only 9 prior inspections and Tree House Play and Cafe only 7, making them among the newest facilities in this week's data. Fresh Box drew five high-severity violations including a specialized-process failure. Tree House, which serves families with children, drew citations for adulterated food and undercooking on just its seventh inspection. Just 1 More, with 16 prior inspections, drew undercooking citations as well. All three of those facilities are still accumulating their inspection histories, and none of them started that history cleanly.
