TAMPA, FL. Isekai Sushi Cafe on Gunn Highway was serving fish this week without documented parasite-destruction procedures in place, a violation that means customers had no assurance the raw fish on their plates had been frozen to kill Anisakis worms or tapeworm larvae before it reached them.
That was one of five high-severity violations inspectors cited at the Gunn Highway cafe during the week of September 15. Inspectors also found food in poor condition or adulterated, inadequate shell stock identification records, employees not reporting illness symptoms, and no consumer advisory for raw or undercooked foods. An intermediate violation for improper sewage disposal rounded out the inspection. Isekai has 12 prior inspections on record.
Two Tampa restaurants did not make it through the week without an emergency closure order.

Oasis Los Cuatro Vientos Corp on East Broadway Avenue was ordered closed September 17 after a sewage backup. Outside BBQ LLC on East Martin Luther King Jr Boulevard was shut down September 16 for roach and fly activity. Neither closure was a surprise given the week's broader pattern across Tampa.
The Violations
The week's two highest violation totals belonged to World of Beer on Avion Park Drive and Lolis 2 on West Hillsborough Avenue, each cited for seven high-severity violations.
At World of Beer, inspectors found food coming from unapproved or unknown sources, food not cooked to required minimum temperatures, and toxic chemicals improperly stored or labeled alongside the food preparation area. The facility also had no employee health policy, an employee not reporting illness symptoms, food contact surfaces that were not properly cleaned or sanitized, and no consumer advisory for raw or undercooked foods. An intermediate violation for improper sewage or wastewater disposal was also on the list.
Lolis 2 drew a different set of concerns. Inspectors cited inadequate shell stock identification records, no allergen awareness demonstrated by staff, required procedures for specialized processes not followed, food from unapproved sources, and improper handwashing technique. No employee health policy and an employee not reporting illness symptoms completed the high-severity count.
Shindig Irish Pub and Eatery on West Linebaugh Avenue followed with six high-severity violations. Inspectors found no person in charge present or performing duties, no employee health policy, improper handwashing technique, inadequate shell stock records, food contact surfaces not properly cleaned, and no consumer advisory for raw or undercooked foods. Inspectors also noted inadequate toilet facilities.
Westshore Pizza on Causeway Boulevard also reached six high-severity violations. The citation list included two separate chemical-related violations: toxic chemicals improperly stored or labeled, and toxic substances improperly identified, stored, or used. Inspectors additionally found inadequate shell stock records, food contact surfaces not properly cleaned, food not cooked to required minimum temperature, and no consumer advisory.
Dead Bob's Too on South Westshore Boulevard drew six high-severity violations including time as a public health control not properly used, toxic chemicals improperly stored, improper handwashing technique, food contact surfaces not properly cleaned, no employee health policy, and no consumer advisory. That combination, an untrained staff handling food with no time controls documented and chemicals near the prep area, was among the more layered single-facility findings of the week.
Siam Thai Restaurant on West Linebaugh Avenue, located just a few doors from Shindig Irish Pub, was cited for five high-severity violations including food not cooked to required minimum temperature, employees not reporting illness symptoms, improper handwashing technique, inadequate shell stock records, and no consumer advisory.
Jazzy's BBQ on West Waters Avenue had no person in charge present when inspectors arrived. The five high-severity violations also included no employee health policy, improper handwashing technique, food contact surfaces not properly cleaned, and no consumer advisory.
Sweet Krunch Korean Fried Chicken on East 7th Avenue drew four high-severity violations: improper handwashing technique, food from unapproved sources, inadequate shell stock records, and no consumer advisory. Three intermediate violations included improperly cleaned multi-use utensils, improper use of wiping cloths, and inadequate toilet facilities.
Taco Bell No. 6132 on Mapledale Boulevard was cited for four high-severity violations including an employee not reporting illness symptoms, food contact surfaces not properly cleaned, no consumer advisory, and toxic chemicals improperly stored or labeled. An intermediate violation for improper sewage or wastewater disposal was also documented.
Havana 86 on West Columbus Drive received four high-severity violations: no employee health policy, improper handwashing technique, food from unapproved sources, and inadequate shell stock records.
Sal Rosa on North Florida Avenue had four high-severity violations that all centered on one theme: handwashing. Inspectors found no person in charge present, an employee not reporting illness symptoms, inadequate handwashing facilities, and improper handwashing technique, all at the same location.
Blasy's Pizzeria on West Hillsborough Avenue was cited for four high-severity violations including no person in charge, improper handwashing technique, inadequate shell stock records, and no consumer advisory.
Cambria Hotel Tampa Brandon on Delaney Lake Drive drew four high-severity violations: food not cooked to required minimum temperature, time as a public health control not properly used, no consumer advisory, and toxic chemicals improperly stored or labeled.
Nana Middle Eastern Street Food on North Franklin Street was the week's lightest finding with one high-severity violation, an inadequate employee health policy, and one intermediate citation for improperly cleaned multi-use utensils.
What These Violations Mean
The most common high-severity violation this week, appearing at nine of the fifteen facilities, was the absence of a consumer advisory for raw or undercooked foods. At a sushi cafe like Isekai, where raw fish is the core product, the omission means customers with compromised immune systems, pregnant women, the elderly, and young children have no written notice that what they are ordering carries elevated risk. The Longer Record
Lolis 2 has 40 prior inspections on record, the highest count of any facility in this week's data. Seven high-severity violations on the 40th-plus inspection, including unapproved food sources and no allergen awareness, suggests the volume of prior oversight has not produced sustained compliance. Havana 86 has 34 prior inspections and returned four high-severity violations this week. Siam Thai has 33, Jazzy's BBQ has 31.
At the other end of the history, Blasy's Pizzeria has only 2 prior inspections on record and already drew four high-severity violations including no person in charge and inadequate shell stock records.
