Water bill is attached from landlord and this location address is included with building next door that is owned by owner of property. The food establishment is a Herbalife food service business that processing and dispenses teas, smoothies, waffles and other Herbalife products. This food establishment is operating without a valid food permit. The Food Establishment shall begin the permitting process within 10 days.
