Kitchen: In use pizza utensil not stored in continuously running water or water of at least 135
degrees F. COS Person in charge discarded the water and place the pizza utensil in the ware wash sink for
cleaning.
Retail: Single service cups and lids stored under the hand wash sink disposed water lines.
COS Single use cups and lids were removed from this area and relocated.
Retail area: Cups in dispenser end cups are exposed to contamination. COS A lid was
placed on the last cup in the dispensers. Contaminated cups were voluntary destroyed.
Kitchen: Cutting board on the small pre-refrigerated unit next to the oven, and the cutting
board on top f the food processing table observed worn, scored, with black and brown deep cuts that are no
longer easy to clean and sanitize. COS cutting boards were removed from service.
Kitchen: No test kit or other device provided that accurately measures the concentration of
the sanitizing solution. COS Quaternary test strips provided when using quaternary as a sanitizer during the
inspection.
Kitchen: Hand wash sink missing the hot and cold water faucet handles. COS water sink
handles repaired during the inspection. Stop Use Order and release Issue.
Retail: Various kratom products found that do not declare the net quantity of the package
contents. See stop sale order.
