TAMPA, FL. Riverview Fresh Cuisine at 9005 E Adamo Drive was emergency-closed by state inspectors on October 15 after they documented roach and fly activity inside the restaurant, records show. It was not the facility's first encounter with inspectors, not by a long stretch.

The closure was the sharpest finding in a week that saw 15 Tampa restaurants cited for high-severity violations, ranging from food sourced outside the regulated supply chain to employees working without any health policy requiring them to stay home when sick.

The Closure

70Prior Inspections on Record

Riverview Fresh Cuisine had been inspected at least 70 times before the October 15 emergency closure for roach and fly activity.

Riverview Fresh Cuisine's five high-severity violations included food in poor condition or adulterated, food contact surfaces not properly cleaned or sanitized, toxic chemicals improperly stored or labeled, and a missing consumer advisory for raw or undercooked items. An intermediate citation for multi-use utensils not properly cleaned rounded out the list.

The facility had accumulated 70 prior inspections on record before this week's closure order. That volume of regulatory contact makes the pest finding particularly difficult to explain away as an isolated lapse.

The Violations

The week's highest violation counts were split between three restaurants, each drawing nine high-severity citations.

Capriotti's Sandwich Shop at 3817 Northdale Blvd was cited for nine high-severity violations, including food from an unapproved or unknown source, inadequate shellfish identification records, parasite destruction procedures not followed, and no consumer advisory for raw or undercooked foods. Inspectors also noted the person in charge was not present or not performing duties, no employee health policy was in place, and improper hand and arm washing technique was observed.

Lameizi Hot Pot and BBQ at 2501 E Fowler Ave drew nine high-severity violations as well. Among them: food not cooked to the required minimum temperature, food contact surfaces not properly cleaned or sanitized, toxic chemicals improperly stored or labeled, and time as a public health control not properly used. The person in charge was also cited as absent or not performing duties.

Ricky's Cuban Cafe at 5249 Ehrlich Rd matched that count with nine high-severity violations, including an employee not reporting symptoms of illness, inadequate handwashing by food employees, improper hand and arm washing technique, no consumer advisory, and no allergen awareness demonstrated. Time as a public health control was also cited as not properly used.

Golden Egg Cafe at 10442 N Dale Mabry Hwy produced eight high-severity violations, including an employee not reporting symptoms of illness, food from an unapproved source, food not cooked to required minimum temperature, and toxic substances improperly identified, stored, or used. The person in charge was again noted as absent or not performing duties.

Origami Sushi at 3615 W Hillsborough Ave drew seven high-severity violations, including food from an unapproved source, food contact surfaces not properly cleaned or sanitized, inadequate shellfish identification records, and toxic chemicals improperly stored or labeled. Inspectors also noted inadequate cooling and cold holding equipment as an intermediate violation.

Nordstrom Bazille at 2223 N Westshore Blvd, the restaurant inside the Westshore Plaza department store, was cited for seven high-severity violations. Those included an employee not reporting symptoms of illness, food from an unapproved source, food contact surfaces not properly cleaned or sanitized, time as a public health control not properly used, and no consumer advisory for raw or undercooked foods.

Bayamo en Coche Grill at 8206 W Waters Ave also drew seven high-severity violations. Inspectors documented no employee health policy, an employee not reporting symptoms of illness, food contact surfaces not properly cleaned or sanitized, and no consumer advisory. The person in charge was cited as not present or not performing duties.

Renee's Bistro at 4010 Boyscout Blvd was cited for six high-severity violations, including parasite destruction procedures not followed, time as a public health control not properly used, no consumer advisory, and no allergen awareness demonstrated. Intermediate violations included improper sewage or wastewater disposal and improper sanitizing solution or procedures.

Farandulero Pizzeria at 4327 Gun Hwy drew six high-severity violations, among them inadequate handwashing facilities, parasite destruction procedures not followed, food contact surfaces not properly cleaned or sanitized, and no consumer advisory. Improper sewage or wastewater disposal was cited as an intermediate violation alongside multi-use utensils not properly cleaned.

Renee's Bistro and Farandulero Pizzeria were among three restaurants cited for improper sewage or wastewater disposal this week. Table 22 at 4924 E Busch Blvd was the third, also drawing five high-severity violations that included food from an unapproved source, food contact surfaces not properly cleaned or sanitized, and toxic chemicals improperly stored or labeled.

Zen Pho and Noodles Tampa at 3021 W Waters Ave was cited for five high-severity violations including food contact surfaces not properly cleaned or sanitized, no consumer advisory, and toxic substances improperly identified, stored, or used. Inadequate cooling and cold holding equipment and inadequate toilet facilities were among the intermediate citations.

Happy Fish Peruvian Fusion at 4046 N Armenia Ave drew five high-severity violations, including no employee health policy, an employee not reporting symptoms of illness, and time as a public health control not properly used. The person in charge was cited as not present or not performing duties. Intermediate violations included single-use items improperly reused and inadequate toilet facilities.

China Latina at 3715 E Busch Blvd was cited for five high-severity violations: no employee health policy, improper hand and arm washing technique, inadequate shellfish identification records, food contact surfaces not properly cleaned or sanitized, and time as a public health control not properly used. Intermediate violations included single-use items improperly reused and equipment in poor repair.

Yummy House China Bistro at 2620 E Hillsborough Ave drew three high-severity violations, the lowest count among the week's flagged facilities. Those included improper hand and arm washing technique, no consumer advisory for raw or undercooked foods, and toxic chemicals improperly stored or labeled.

What These Violations Mean

The most common single citation across this week's inspections was improper hand and arm washing technique, documented at ten of the fifteen facilities. This is not a paperwork violation. Studies show that employees who go through the motions of handwashing but do not follow proper technique, including adequate soap contact time and rinsing, leave pathogens on their hands capable of transferring to food. At Ricky's Cuban Cafe, inspectors cited both inadequate handwashing by food employees and improper technique, a combination that represents two separate breakdowns in the same critical control point.

The "employee not reporting symptoms of illness" citation, documented at Golden Egg Cafe, Origami Sushi, Nordstrom Bazille, Bayamo en Coche Grill, Renee's Bistro, and Happy Fish Peruvian Fusion, points to a direct transmission route for Norovirus and other pathogens. This violation is not about paperwork. It means an employee who was sick, or showing symptoms, continued working food service without triggering any internal protocol to remove them from the line.

Shellfish traceability violations appeared at eleven of the fifteen facilities, including Capriotti's, Lameizi, Ricky's Cuban Cafe, Golden Egg Cafe, Origami Sushi, Nordstrom Bazille, Bayamo en Coche Grill, Farandulero Pizzeria, Table 22, Zen Pho and Noodles, and China Latina. Shellfish tags exist specifically so that if a customer falls ill after eating oysters, clams, or mussels, the supply chain can be traced back to the harvest bed. Without those records, an outbreak cannot be sourced, and a contaminated supplier continues operating.

Parasite destruction violations at Capriotti's, Renee's Bistro, and Farandulero Pizzeria carry a specific risk most customers do not consider. Raw or undercooked fish must be frozen to specific temperatures for specific durations before service to kill parasites including Anisakis, which causes severe gastrointestinal illness. A missing consumer advisory, cited at twelve of the fifteen facilities this week, means that customers who are pregnant, elderly, or immunocompromised are eating those items without any warning on the menu that they carry elevated risk.

The Longer Record

Riverview Fresh Cuisine's 70 prior inspections make it the most-inspected facility in this week's group by a substantial margin. The emergency closure for roach and fly activity on October 15 follows a record of regulatory contact that stretches back further than any other restaurant cited this week.

Ricky's Cuban Cafe carries 32 prior inspections. Golden Egg Cafe and Origami Sushi each have 31. All three drew eight or nine high-severity violations this week, and all three have inspection histories long enough that the same categories of violations, handwashing failures, shellfish traceability gaps, and illness reporting breakdowns, have had ample opportunity to be corrected.

Yummy House China Bistro has 50 prior inspections on record, the second-highest count in this week's group, yet drew only three high-severity violations. Renee's Bistro, with 26 prior inspections, drew six high-severity citations including both parasite destruction failures and sewage disposal problems.

Bayamo en Coche Grill and Farandulero Pizzeria sit at the other end of the experience spectrum. Bayamo has only 2 prior inspections on record and was already drawing seven high-severity violations, including no employee health policy and no consumer advisory. Farandulero Pizzeria has 4 prior inspections and was cited for six high-severity violations including inadequate handwashing facilities and improper sewage disposal. Both facilities are early in their inspection histories and already accumulating citations in categories that suggest foundational food safety practices were not in place before they opened.

Capriotti's Sandwich Shop, with 9 prior inspections, drew the week's joint-highest violation count of nine high-severity citations. Among those was a citation for food from an unapproved or unknown source, a finding that raises questions that inspection records alone cannot answer.
