China Buffet's eight high-severity violations covered nearly every major food safety failure category in a single visit. The combination of missing handwashing infrastructure and documented improper technique means hand hygiene failures were happening at both the structural and behavioral level simultaneously.

Skateworld on Paula Drive collected six high-severity violations and three intermediate ones. Inspectors cited food in poor condition, mislabeled, or adulterated; inadequate shell stock identification records; food contact surfaces not properly cleaned; no employee health policy; an employee not reporting illness symptoms; and no consumer advisory. Multi-use utensils not properly cleaned and inadequate ventilation rounded out the intermediate findings. Skateworld is a roller rink, and its food operation drew more serious citations this week than several full-service restaurants.

La Vitanuova Sports Bar and Grill on North Armenia Avenue logged six high-severity violations on only its second inspection on record. Bern's Steak House on South Howard Avenue, one of Tampa's most celebrated dining institutions, was cited for five high-severity violations. Inspectors found food from an unapproved or unknown source, inadequate shell stock identification records, improper handwashing technique, time as a public health control not properly used, and no consumer advisory. An intermediate citation for inadequate ventilation and lighting was also recorded. Bern's has 25 prior inspections on record.

Good Taste on North Florida Avenue was cited for food from an unapproved or unknown source, alongside inadequate handwashing facilities, improper handwashing technique, and no employee health policy. Inspectors also found improper sewage or wastewater disposal, an intermediate violation that carries its own serious contamination risk. Good Taste has 34 prior inspections on record.

Copper Shaker on East 7th Avenue drew a similar sewage-related citation alongside inadequate handwashing facilities, no employee health policy, food contact surfaces not properly cleaned, and no consumer advisory. That is four high-severity violations and an intermediate sewage citation at a bar with 20 prior inspections on record.

Sweet Krunch Korean Fried Chicken on East 7th Avenue, on only its fourth inspection, was cited for food from an unapproved or unknown source and food contaminated by chemical, physical, or biological hazards. Both are among the most serious violation categories inspectors can document. Inspectors also found inadequate shell stock records, improper handwashing technique, and no consumer advisory.

Barterhouse on 15th Street was the only facility this week cited for no allergen awareness demonstrated, a violation that carries emergency room-level consequences for the 32 million Americans with food allergies. Donatello's on North Dale Mabry drew four high-severity citations including toxic substances improperly identified, stored, or used, which joins China Buffet as the only two facilities this week with chemical-related high-severity findings. NY NY Pizza on East 7th Avenue drew a single high-severity violation: parasite destruction procedures not followed. That citation applies specifically to fish, pork, or wild game served raw or undercooked without the required freezing treatment to kill parasites including Anisakis and Trichinella. One violation. A specific and serious one.

What These Violations Mean

The single most repeated high-severity violation this week, appearing at more than half the cited facilities, was no consumer advisory for raw or undercooked foods. That omission matters most for the customers least equipped to absorb the risk: pregnant women, elderly diners, and anyone with a compromised immune system. The food-from-unapproved-sources citation at Bern's Steak House, Good Taste, and Sweet Krunch Korean Fried Chicken represents a different category of risk entirely. Food that bypasses USDA and FDA inspection has no traceability. If a customer gets sick, investigators cannot trace the contamination back to its origin. That traceability gap is what turns a single illness into a prolonged outbreak.

The employee illness violations at China Buffet, Skateworld, La Vitanuova, Toastique, Splitsville, and Abella's Cuban Restaurant describe a direct transmission route. Norovirus, which causes the majority of foodborne illness outbreaks in the United States, spreads most efficiently when a sick food worker handles food without restriction. A written health policy that requires workers to report symptoms is the first line of defense. None of those six facilities had one functioning.

Sewage disposal violations at Good Taste and Copper Shaker sit in a separate category from most other citations on this list. Improper wastewater handling introduces fecal bacteria into the facility environment, and unlike a handwashing lapse or an unlabeled container, it is not corrected by a single employee changing behavior.

The Longer Record

China Buffet's 52 prior inspections make it the most-inspected facility in this week's data by a wide margin.
