TAMPA, FL. Nosh Express at 1715 N West Shore Blvd drew nine high-severity violations in a single inspection this week, including a finding that employees were not reporting symptoms of illness, inadequate handwashing facilities, improper handwashing technique, and a failure to follow parasite destruction procedures for fish, all documented in the same visit.

Nine high-severity violations matched the total at Urban Cantina at 200 E Madison St, where inspectors found the restaurant operating without a written employee health policy, sourcing food from unapproved or unknown suppliers, and failing to cook food to the required minimum temperature.

Those two restaurants led a week in which state inspectors documented high-severity violations at 15 Tampa facilities. Two restaurants were emergency-closed entirely.

Twins Delicious Seafood and Soul Food at 5102 N 40th St was emergency-closed on July 10 after inspectors found no handwashing sink available. La Ceibena at 8806 W Flora St was emergency-closed two days earlier, on July 8, after inspectors found no potable water.

Both closures represent the most immediate category of public health risk. No running water and no handwashing sink are not paperwork failures. They are conditions under which food cannot be safely prepared.

The Violations

China Buffet at 1245 E Fowler Ave accumulated six high-severity violations, including a finding that no person in charge was present or performing duties during the inspection. Inspectors also cited employees for not reporting illness symptoms and for improper handwashing technique, and found no consumer advisory posted for raw or undercooked foods.

China House at 4050 W Hillsborough Ave drew six high-severity violations as well, among them food sourced from unapproved or unknown suppliers, inadequate handwashing facilities, and a failure to properly use time as a public health control.

Five high-severity violations were documented at Dunkin Donuts at 2300 W Hillsborough Ave, including improperly stored toxic substances and a finding that no allergen awareness was demonstrated by staff. Inspectors also cited improper sewage or wastewater disposal.

Retro House at 934 E Henderson Ave received five high-severity violations, including food not cooked to required minimum temperature and toxic chemicals improperly stored or labeled, alongside a finding of inadequate toilet facilities.

KFC at 8201 Florida Ave was cited for five high-severity violations, including food in poor condition or adulterated, toxic chemicals improperly stored, and a failure to follow required procedures for specialized processes.

Maloney's Local Irish Pub at 1120 E Kennedy Blvd drew five high-severity violations, among them employees not reporting illness symptoms, inadequate shellfish identification records, and no consumer advisory for raw or undercooked menu items.

Top Shelf Sports Lounge at 401 E Jackson St was cited for five high-severity violations including food from unapproved or unknown sources and improperly cleaned food contact surfaces.

Courtyard by Marriott Tampa Northwest at 12730 Citrus Park Lane received five high-severity violations, with inspectors finding no person in charge present, time as a public health control not properly used, and no consumer advisory for raw or undercooked foods.

Boulon Brasserie at 1001 Water Street was cited for four high-severity violations, including failure to follow parasite destruction procedures, no consumer advisory for raw or undercooked foods, and no allergen awareness demonstrated.

Applebee's Neighborhood Grill and Bar at 4006 W Hillsborough Ave drew four high-severity violations, including food not cooked to required minimum temperature and two separate chemical storage violations: toxic chemicals improperly stored or labeled and toxic substances improperly identified, stored, or used.

Hilton Garden Inn Tampa Ybor at 1700 E 9th Ave received four high-severity violations, among them improper handwashing technique, food not cooked to required minimum temperature, and no consumer advisory for raw or undercooked foods.

Rocco's Tacos and Tequila Bar at 2223 N Westshore Blvd was cited for three high-severity violations, including food from unapproved or unknown sources, inadequate shellfish identification records, and food not cooked to required minimum temperature.

Big G's Pizza and Grille at 3742 W Lambright St drew two high-severity violations, including no person in charge present and employees not reporting illness symptoms, along with an intermediate violation for improper sewage or wastewater disposal.

What These Violations Mean

The single most common high-severity violation across this week's inspections was improper handwashing technique, cited at Nosh Express, Urban Cantina, China House, Dunkin Donuts, Retro House, KFC, Maloney's, Top Shelf Sports Lounge, Courtyard by Marriott, and Hilton Garden Inn Ybor. This is distinct from simply not washing hands. It means employees made an attempt at handwashing but did so incorrectly, leaving pathogens on their hands before touching food or surfaces. An incomplete wash does not significantly reduce the microbial load.

The employee illness reporting failures at Nosh Express, Urban Cantina, China Buffet, Maloney's, Top Shelf Sports Lounge, and Big G's carry a different kind of risk. Norovirus is the leading cause of foodborne illness outbreaks in the United States, and it spreads most efficiently when a sick food worker continues handling food without disclosure. A written health policy, absent at Urban Cantina and China Buffet, is what creates the documented obligation for workers to report symptoms before they reach the kitchen.

Food sourced from unapproved or unknown suppliers, documented at Urban Cantina, China House, Top Shelf Sports Lounge, and Rocco's Tacos, bypasses the federal inspection chain entirely. If a customer becomes ill, investigators cannot trace the product back to a source, test it, or issue a recall. The food arrives with no verifiable safety record.

The chemical storage violations at Dunkin Donuts, Retro House, KFC, and Applebee's are not minor organizational issues. Toxic substances stored near or above food preparation surfaces can contaminate food through spills, splashes, or mislabeled containers. Two separate chemical violation categories were cited at Applebee's in the same inspection, suggesting the problem was not isolated to a single shelf.

The Longer Record

China Buffet on Fowler Ave has 55 prior inspections on record, the highest count of any facility in this week's data. This week's six high-severity violations, including no person in charge and no employee health policy, are not the findings of a restaurant encountering inspectors for the first time. Fifty-five inspections represent years of regulatory contact, and the same foundational failures in management oversight and illness reporting protocols appeared again this week.

Urban Cantina on Madison Street has 45 prior inspections on record. Nine high-severity violations in a single visit, including operating without any written employee health policy, is a significant finding for a facility with that depth of inspection history.

Maloney's Local Irish Pub and Applebee's on Hillsborough Ave each have 32 prior inspections. Rocco's Tacos has 35. China House has 28. None of these are new restaurants encountering health code requirements for the first time.

Top Shelf Sports Lounge has only 9 prior inspections on record, making it among the newest facilities in this week's data. Five high-severity violations in its early inspection history, including food from unapproved sources and improperly cleaned food contact surfaces, is a notable start. Big G's Pizza and Grille has just 3 prior inspections, and its second or third visit already produced a sewage disposal violation alongside both of its high-severity citations.

Boulon Brasserie and Dunkin Donuts each have 13 prior inspections. The parasite destruction failure at Boulon, a Water Street brasserie where raw and lightly prepared fish appear on the menu, was paired with no consumer advisory for raw or undercooked foods. As of this inspection, customers eating there had no posted notice that those risks existed.
