TAMPA, FL. Kiki's Treats on Davis Boulevard had been inspected by the state only once before this week. On its second visit, inspectors cited the shop for six high-severity violations, including no employee health policy, improper handwashing technique, food in poor condition, missing shellfish traceability records, no consumer advisory for raw or undercooked foods, and no demonstrated allergen awareness, plus a sewage disposal problem flagged as intermediate.
That tally made Kiki's Treats the week's highest-severity offender among 15 Tampa facilities that drew high-priority citations between August 21 and August 27, 2026.
The Violations
La Cabana Restaurant on North Armenia Avenue drew five high-severity violations, including food from an unapproved or unknown source, missing shellfish traceability records, no consumer advisory, employees not reporting illness symptoms, and improper handwashing. Inspectors also cited the restaurant for unclean multi-use utensils, improper sanitizer procedures, and inadequate cooling equipment.
Food from an unapproved source is one of the most serious flags an inspector can write. It means the restaurant cannot demonstrate where its food came from or that it passed regulatory inspection before arriving in the kitchen.
Maikels Kitchen on Waters Avenue matched La Cabana's total with five high-severity violations, including toxic chemicals improperly stored near food areas, improperly cleaned food contact surfaces, no consumer advisory, no employee health policy, and improper handwashing. Inspectors also cited the restaurant for sewage disposal problems and inadequate cold holding equipment.
California Pizza Kitchen at Westshore Plaza had no person in charge present or performing duties during the inspection, which accompanied four other high-severity findings: employees not reporting illness symptoms, food not cooked to required minimum temperature, time not properly used as a public health control, and no consumer advisory. Single-use items were being reused.
Tom's Tavern Forest Hills on Fletcher Avenue also lacked a person in charge and drew four additional high-severity violations: employees not reporting illness symptoms, improper handwashing, missing shellfish records, and no consumer advisory. Single-use items were being reused there as well.
Harborview Cafe on Rocky Point Drive West was cited for five high-severity violations including failure to follow parasite destruction procedures for fish, improperly cleaned food contact surfaces, missing shellfish traceability records, improper handwashing, and no consumer advisory. The cafe also had improper wiping cloth use flagged at the intermediate level.
Parasite destruction procedures exist for a reason. Fish served raw or undercooked, including sushi and ceviche, must be frozen to specific temperatures for specific durations to kill parasites such as Anisakis. Harborview Cafe and Fruitwood Standup Market on West Cypress Street both drew this violation during the same week.
Fruitwood also had employees not reporting illness symptoms and improperly cleaned food contact surfaces, along with improper handwashing and no consumer advisory. The market has only four prior inspections on record.
Saki Endless Sushi and Hibachi Restaurant on North Dale Mabry Highway drew five high-severity violations, among them toxic chemicals improperly stored or labeled and improperly cleaned food contact surfaces. A sushi restaurant storing chemicals improperly near food prep areas, while also failing to properly clean surfaces where raw fish is handled, is a combination that concentrates risk.

China Buffet on East Fowler Avenue was cited for time not properly used as a public health control, improperly cleaned food contact surfaces, missing shellfish records, and improper handwashing, along with four intermediate violations including single-use items being reused and inadequate toilet facilities.
Mi Pueblito Bakery and Restaurant on North Armenia Avenue drew one high-severity violation for toxic substances improperly identified, stored, or used, along with inadequate cooling equipment and inadequate ventilation.
The week's only emergency closure was Sushi Masa Premium at 17021 Palm Pointe Drive, shut down August 21 for roach activity.
What These Violations Mean
The most pervasive high-severity violation this week was the absence of a consumer advisory for raw or undercooked foods, cited at ten of the fifteen facilities. That notice on a menu is not a formality.
