ORLANDO, FL. Bikkuri Sushi Noodle and Grill on East Colonial Drive drew 11 high-severity violations during the week of September 16, the highest single-facility count in Orange County that week, with inspectors citing food from unapproved sources, missing employee health policies, workers not reporting illness symptoms, and improper handwashing technique all in the same visit.

Fourteen other Orlando restaurants also drew high-severity citations during the same seven-day stretch.

Bikkuri Sushi's inspection also flagged inadequate shell stock identification records, meaning inspectors could not confirm where the restaurant's shellfish came from or when it arrived. Inspectors further documented improper use of time as a public health control, and food contact surfaces that were not properly cleaned or sanitized.

The problems at Bikkuri were not limited to food handling. No person in charge was present or performing supervisory duties during the inspection.

Nature's Table at 300 S. Orange Avenue drew eight high-severity violations, including citations for food from unapproved sources, missing shell stock records, failure to follow parasite destruction procedures for fish, no consumer advisory for raw or undercooked foods, and no allergen awareness demonstrated by staff. Inspectors also cited inadequate handwashing facilities and workers not reporting illness symptoms.

Mi Ranchito Oaxaqueno on North Chickasaw Trail drew seven high-severity violations, among them no employee health policy, workers not reporting illness symptoms, improper handwashing technique, missing shell stock records, no consumer advisory for raw or undercooked items, and no allergen awareness. Multi-use utensils were also cited as improperly cleaned.

City Beautiful Cafe at 400 S. Orange Avenue also logged seven high-severity violations. Inspectors found food from unapproved sources, toxic chemicals improperly stored or labeled, no consumer advisory, inadequate handwashing facilities, and no employee health policy.

Bayline Diner and Empl Cafe at 6550 Adventure Way matched that count with seven high-severity violations of its own. Citations there included failure to follow parasite destruction procedures, toxic chemicals improperly stored or labeled, no allergen awareness, missing shell stock records, and workers not reporting illness symptoms.

Marriott Orlando Downtown at 400 W. Livingston Street drew five high-severity violations, including food contaminated by chemical, physical, or biological hazards, food from unapproved sources, food not cooked to the required minimum temperature, inadequate handwashing by employees, and no consumer advisory for raw or undercooked foods.

Nuno's Cafe at 12001 Avalon Lake Drive was cited for six high-severity violations, including no person in charge present, food from unapproved sources, food contact surfaces not properly cleaned or sanitized, and toxic substances improperly identified, stored, or used. Inspectors also documented improper sewage or wastewater disposal.

McDonald's at 7409 E. Colonial Drive drew six high-severity violations, including parasite destruction procedures not followed, food contact surfaces not properly cleaned or sanitized, toxic chemicals improperly stored or labeled, no consumer advisory, and no allergen awareness.

Latin Square at 250 S. Orange Avenue was cited for six high-severity violations, among them workers not reporting illness symptoms, inadequate handwashing by employees, improper handwashing technique, food contact surfaces not properly cleaned or sanitized, time as a public health control not properly used, and toxic chemicals improperly stored or labeled.

Zhang Garden at 13832 Narcoossee Road drew six high-severity violations, including food from unapproved sources, improper handwashing technique, no employee health policy, improperly cleaned food contact surfaces, no consumer advisory, and toxic chemicals improperly stored or labeled.

South Philly Steak at 12001 Avalon Lakes Drive also logged six high-severity violations, with inspectors citing no person in charge, no employee health policy, improper handwashing technique, food contact surfaces not properly cleaned or sanitized, no consumer advisory, and toxic chemicals improperly stored or labeled.

JR Tropical Ice Cream at 1461 N. Goldenrod Road drew six high-severity violations, including no person in charge, no employee health policy, inadequate handwashing by employees, improper handwashing technique, food from unapproved sources, and no consumer advisory.

Blackhawk Social Club at 12001 Avalon Lake Drive drew three high-severity violations, including improper handwashing technique, food contact surfaces not properly cleaned or sanitized, and food not cooked to the required minimum temperature. Inspectors also documented improper sewage or wastewater disposal.

Charley's Philly Steaks at 433 N. Alafaya Trail was cited for three high-severity violations, including food contact surfaces not properly cleaned or sanitized, no consumer advisory, and toxic chemicals improperly stored or labeled, alongside improper sewage or wastewater disposal.

Uma Sushi and Ramen at 486 N. Orange Avenue drew one high-severity violation, for employees not reporting symptoms of illness.

What These Violations Mean

The single most widespread violation pattern this week was the combination of no employee health policy, no illness reporting, and improper handwashing, a cluster that appeared together at Bikkuri Sushi, Mi Ranchito Oaxaqueno, City Beautiful Cafe, Bayline Diner, Nuno's Cafe, South Philly Steak, and JR Tropical Ice Cream. When a facility has no written health policy, workers have no formal obligation to stay home when sick. When illness reporting is also absent, a worker with norovirus or hepatitis A can handle food for an entire shift without any mechanism to stop it. Norovirus spreads to other people through as few as 18 viral particles, and a single sick food worker can infect dozens of customers before anyone notices.

Food from unapproved sources, cited at Bikkuri Sushi, Nature's Table, City Beautiful Cafe, Nuno's Cafe, Zhang Garden, and JR Tropical Ice Cream, is a traceability problem. If a customer becomes ill after eating at one of these restaurants, investigators cannot determine where the food came from, whether other recipients of the same shipment are also affected, or whether a recall should be issued. The supply chain is severed at the point of purchase.

The parasite destruction citations at Nature's Table, Bayline Diner, and McDonald's are specific to raw or undercooked fish and certain meats. Parasites including Anisakis in fish and Trichinella in pork are killed by proper freezing protocols or thorough cooking. Without documented proof that either process was followed, customers eating sushi, ceviche, or undercooked pork at these locations had no assurance that the food was parasite-free.

Toxic chemicals improperly stored or labeled, flagged at City Beautiful Cafe, Bayline Diner, Charley's Philly Steaks, McDonald's, Zhang Garden, South Philly Steak, Latin Square, and Nuno's Cafe, means cleaning agents or other hazardous substances were kept in proximity to food or in unlabeled containers. The risk is not theoretical: chemical contamination of food is one of the fastest-acting foodborne illness vectors, capable of causing acute symptoms within minutes of ingestion.

Blackhawk Social Club has 43 prior inspections on record, the deepest history of any facility cited this week, and this inspection still produced three high-severity violations including a food temperature violation and improperly sanitized food contact surfaces. Forty-three inspections is a long time to still be drawing citations in the same fundamental categories.

Nuno's Cafe has 34 prior inspections behind it and this week drew six high-severity violations including no person in charge and toxic substances improperly stored. Bikkuri Sushi and South Philly Steak each have 33 prior inspections on record. Bikkuri used that history to accumulate 11 high-severity violations in a single week. South Philly Steak, which shares a shopping center address on Avalon Lakes Drive with Nuno's Cafe and Blackhawk Social Club, drew six high-severity violations of its own, including no person in charge and no employee health policy.

Zhang Garden has 29 prior inspections and Latin Square has 30, yet both drew six high-severity violations this week in categories including food from unapproved sources and toxic chemicals improperly stored. Marriott Orlando Downtown has 28 inspections on record and still drew a citation for food not cooked to the required minimum temperature, one of the most fundamental violations in food service.

The newest facilities in this week's data tell a different story. Mi Ranchito Oaxaqueno has only seven prior inspections and already drew seven high-severity violations, including missing shell stock records and no allergen awareness. City Beautiful Cafe has eight prior inspections and matched that with seven high-severity violations of its own. Both restaurants are still in the early stages of their inspection histories, and both are already accumulating citations in the categories most directly linked to outbreak risk.

The Longer Pattern

Three restaurants sharing the same Avalon Lake Drive address, Blackhawk Social Club, Nuno's Cafe, and South Philly Steak, all drew high-severity violations during the same inspection week. Nuno's Cafe was cited for improper sewage or wastewater disposal in addition to its six high-severity violations. Whether the shared building has contributed to shared compliance problems is a question the inspection records alone cannot answer.

Uma Sushi and Ramen, with 16 prior inspections and only one high-severity violation this week, stood apart from the rest of the list. That single violation, employees not reporting illness symptoms, is also the one that inspectors cannot see until someone gets sick.
