ORLANDO, FL. Sea Thai Restaurant on East Colonial Drive logged 14 high-severity violations during the week of October 17, more than any other facility inspected in Orange County that week, including citations for food sourced from unapproved or unknown suppliers and shellfish on hand with no identification records to trace where it came from.

State inspectors flagged 15 Orlando restaurants for high-severity violations during the seven-day stretch. The problems ranged from employees not reporting illness symptoms to improperly stored toxic chemicals to food never reaching safe cooking temperatures. Sea Thai's total stood alone at the top.

Sea Thai's inspection sheet read like a compounding failure. Inspectors cited the restaurant not only for food from unapproved sources but also for inadequate handwashing facilities, improper handwashing technique, employees not reporting illness symptoms, and the absence of a written employee health policy. The shellfish traceability violation added another layer: without harvest records tied to oysters, clams, or mussels, there is no way to identify the source if a customer gets sick.

Three restaurants tied for the second-highest high-severity count at nine each. Sus Hi Eatstation on East Colonial Drive had no responsible person in charge on site during the inspection, lacked a consumer advisory for raw or undercooked foods, and inspectors found that time was being used as a public health control without the required documentation to back it up. Parasite destruction procedures for fish were also not being followed.

Maroush Shawrma and Grill on North Alafaya Trail also had no person in charge present, no employee health policy, and shellfish on hand with inadequate identification records. Inspectors additionally cited the restaurant for food in poor condition and for failing to follow parasite destruction procedures, a violation that matters acutely at a restaurant serving fish dishes.

Simon Parrilla Bar and Grill at Florida Mall rounded out the three-way tie. Inspectors found food from unapproved sources, shellfish without proper identification records, inadequate handwashing facilities, and improperly stored toxic chemicals, all at a location that has accumulated 40 prior inspections on record, the highest history count of any facility cited this week.

Lam's Garden on East Colonial Drive drew eight high-severity violations, including two that stood out: toxic chemicals improperly stored or labeled, and a separate citation for toxic substances improperly identified, stored, or used. Those are two distinct chemical safety failures at the same facility in the same inspection. Inspectors also found no consumer advisory for raw or undercooked foods and documented that parasite destruction procedures were not being followed.

Volcano Hot Pot and BBQ on East Colonial Drive was cited for food not reaching required minimum cooking temperatures and for a complete absence of allergen awareness among staff. The restaurant also had no consumer advisory for raw or undercooked foods and no written employee health policy. At a hot pot concept where customers cook their own food at the table, the cooking temperature violation raises a particular question about what guidance, if any, diners were receiving.

Dimsum King and Seafood on Conroy Road was cited for food from unapproved sources, shellfish without adequate identification records, and no allergen awareness demonstrated, three violations that together describe a kitchen with limited traceability and no documented plan for protecting customers with food allergies.

Helena Modern Riviera on International Drive, a restaurant in one of Orlando's busiest tourist corridors, had no person in charge present, no employee health policy, inadequate handwashing facilities, and food from unapproved sources. International Drive draws millions of visitors annually, many of them tourists with no prior knowledge of a restaurant's inspection history.

Agave Azul Cocina Mexicana on South Kirkman Road logged eight high-severity violations including a citation for required procedures for specialized processes not being followed. That violation applies to techniques like smoking, curing, fermenting, or reduced-oxygen packaging, processes that require specific written plans because they create conditions where dangerous bacteria can multiply if not tightly controlled. Inspectors also found food not cooked to minimum temperature and food from unapproved sources.

Korea House II on East Colonial Drive was cited for parasite destruction procedures not followed, no consumer advisory for raw or undercooked foods, and dual chemical violations: improperly stored chemicals and improperly identified or used toxic substances. The restaurant serves dishes that include raw or lightly cooked proteins, making the absence of a consumer advisory a direct gap in diner information.

El Cilantrillo Restaurant at Florida Mall drew four high-severity violations, including food not cooked to required minimum temperature and food contact surfaces not properly cleaned or sanitized. Thai House of Orlando on East Colonial was cited for improper sewage or waste water disposal alongside its four high-severity violations, a combination that inspectors flag as a fecal contamination risk throughout the facility.

Teak Neighborhood Grill on Times Square Avenue and Pigzza on North Mills Avenue each drew two high-severity violations. Both also had improper sewage or waste water disposal cited as an intermediate violation. Memories of Peru on Crystal Clear Lane rounded out the week with two high-severity violations: no employee health policy and improperly stored chemicals.

What These Violations Mean

The employee health policy violation, cited at Sea Thai, Sus Hi Eatstation, Maroush Shawrma, Volcano Hot Pot, Lam's Garden, Dimsum King, Helena Modern Riviera, Agave Azul, Korea House II, and Memories of Peru, is not a paperwork problem. Without a written policy, sick employees have no formal instruction to stay home or report symptoms, and supervisors have no documented standard to enforce. The CDC identifies sick food workers as the primary driver of multi-victim outbreaks, particularly for Norovirus, which can spread from a single infected employee to dozens of customers in a single shift.

The unapproved food source violation, documented at Sea Thai, Simon Parrilla, Dimsum King, Helena Modern Riviera, and Agave Azul, removes the inspection chain entirely. Food that enters a kitchen from an unlicensed supplier has bypassed USDA and FDA safety checks. If someone gets sick and investigators need to trace the source, there may be no records to follow.

The shellfish traceability violations at Sea Thai, Maroush Shawrma, Simon Parrilla, Dimsum King, Agave Azul, and Korea House II compound that problem specifically for high-risk foods. Shellfish are frequently consumed raw or lightly cooked, and state law requires harvest tags and receiving records precisely because shellfish-linked illnesses, including those from Vibrio bacteria and hepatitis A, can be traced and contained only if the source is documented.

The parasite destruction failures at Lam's Garden, Sus Hi Eatstation, Maroush Shawrma, and Korea House II describe a different but equally direct risk. Fish served raw or undercooked must be frozen to specific temperatures for specific periods to kill parasites like Anisakis. When that step is skipped, the parasite goes to the customer's plate.

The Longer Record

Simon Parrilla Bar and Grill carries the longest inspection history of any facility flagged this week, with 40 prior inspections on record. This week's nine high-severity violations, including food from unapproved sources and inadequate handwashing facilities, follow a pattern that 40 inspections have not resolved. Agave Azul Cocina Mexicana has 38 prior inspections and still drew eight high-severity violations this week, including the specialized process failure that describes a kitchen conducting advanced food preparation without documented safety controls.

Lam's Garden has 37 prior inspections on record and logged eight high-severity violations this week. Teak Neighborhood Grill has 36, Helena Modern Riviera 35. These are not new restaurants finding their footing. They are established operations with long inspection histories that continue to generate serious citations.

Volcano Hot Pot and BBQ is the newest location in this week's group, with only 8 prior inspections on record. It still accumulated eight high-severity violations, including the allergen awareness failure and the cooking temperature citation. A restaurant in its early inspections is expected to have some learning curve, but allergen awareness and minimum cooking temperatures are foundational, not advanced requirements.

Sea Thai, with 14 high-severity violations and 29 prior inspections on record, has been through this process nearly 30 times. The shellfish traceability records, the unapproved food sources, the handwashing failures: those are not new categories of violation for a restaurant at this inspection count. Whether the same violations appeared in prior inspections is a question the records can answer.
