ORLANDO, FL. Ming Garden at 3922 E Colonial Drive drew 12 high-severity violations in a single inspection during the week of October 12, the highest count among 15 Orlando-area restaurants flagged for serious food safety failures by state inspectors.

The violations at Ming Garden included food from an unapproved or unknown source, no written employee health policy, employees not reporting illness symptoms, and inadequate handwashing by food employees. Inspectors also cited improper handwashing technique, failure to follow parasite destruction procedures, inadequate shellfish identification records, and food contact surfaces not properly cleaned or sanitized.

That is eight distinct categories of high-severity violations at one address, across one visit.

Isan Zaap Thai Cafe at 4693 Garden Park Blvd drew 10 high-severity violations, second only to Ming Garden. Among the most serious: no person in charge present or performing duties, no written employee health policy, employees not reporting illness symptoms, and inadequate handwashing facilities. Inspectors also cited improper handwashing technique, improperly stored toxic chemicals, no consumer advisory for raw or undercooked foods, and food contact surfaces not properly cleaned or sanitized.

Capybara Ramen and Sushi Orlando at 8957 International Drive accumulated nine high-severity violations, including food from an unapproved or unknown source, inadequate shellfish identification records, and failure to follow parasite destruction procedures. No person in charge was present, employees were not reporting illness symptoms, and no written health policy was in place.

Applebee's Neighborhood Grill and Bar at 6324 International Drive also drew nine high-severity violations. Inspectors found food not cooked to the required minimum temperature, food from an unapproved or unknown source, failure to follow parasite destruction procedures, and improper use of time as a public health control. There was no consumer advisory for raw or undercooked foods, and toxic chemicals were improperly stored or labeled.

Olive Garden Italian Restaurant at 8984 International Drive was cited for eight high-severity violations, including food from an unapproved source, employees not reporting illness symptoms, inadequate handwashing, improperly stored toxic chemicals, and two separate toxic-substance violations, one for improper labeling and one for improper use.

Denny's 5114 at 9880 International Drive drew eight high-severity violations as well. One of the more acute findings: food contaminated by chemical, physical, or biological hazards, a violation that appears alongside citations for food from an unapproved source, parasite destruction failures, and two toxic-substance violations.

Cooper's Hawk Winery and Restaurant at 529 N Alafaya Trail was cited for eight high-severity violations, including food from an unapproved or unknown source, inadequate shellfish identification records, improper use of time as a public health control, and no allergen awareness demonstrated. Allergen violations are among the least commonly cited high-severity findings in weekly inspection data, and Cooper's Hawk was one of only two facilities flagged for it this week.

Pho Bar Vietnamese Kitchen at 1012 E Colonial Drive drew eight high-severity violations including inadequate handwashing facilities, food not cooked to the required minimum temperature, no consumer advisory for raw or undercooked foods, and toxic substances improperly identified, stored, or used.

Michi Ramen at 3683 E Colonial Drive was cited for eight high-severity violations, including inadequate handwashing facilities, food from an unapproved source, food not cooked to the required minimum temperature, and no written employee health policy.

La Estacion de la Arepa at 12253 S Orange Blossom Trail drew eight high-severity violations with no person in charge present, no employee health policy, employees not reporting illness symptoms, food from an unapproved or unknown source, and inadequate shellfish identification records.

Authentic Gyros and Grill at 2401 W Colonial Drive was cited for five high-severity violations, including failure to follow procedures for specialized processes, parasite destruction failures, toxic chemicals improperly stored, and no consumer advisory for raw or undercooked foods. Inspectors also found improper sewage or wastewater disposal, an intermediate violation that signals potential fecal contamination risk throughout the facility.

Happy Snapper Seafood at 6327 S Orange Ave drew seven high-severity violations including food not cooked to the required minimum temperature, no allergen awareness demonstrated, and toxic chemicals improperly stored or labeled. Inspectors also cited improper sanitizing solution or procedures.

East Garden at 5136 W Colonial Drive was cited for seven high-severity violations, including food from an unapproved source, improper use of time as a public health control, and toxic chemicals improperly stored.

A Gourmet Chinese Cuisine at 1101 E Colonial Drive drew three high-severity violations, including toxic chemicals improperly stored or labeled and no consumer advisory for raw or undercooked foods. Inspectors also found improper sewage or wastewater disposal.

Zen Dumpling at 423 N Alafaya Trail drew the lowest violation count of the week, two high-severity findings for improper handwashing technique and improperly stored toxic chemicals.

What These Violations Mean

The combination of no written employee health policy and employees not reporting illness symptoms, found together at Ming Garden, Isan Zaap Thai Cafe, Capybara Ramen, Denny's, Pho Bar, Michi Ramen, La Estacion de la Arepa, and East Garden, is the documented setup for multi-victim outbreaks. Norovirus spreads through a single infected food handler who has no policy requiring them to stay home. Without a written policy, there is no mechanism for a supervisor to act even if symptoms are known.

Inadequate handwashing facilities, found at Isan Zaap Thai Cafe, Pho Bar Vietnamese Kitchen, and Michi Ramen, compounds this risk. A facility can require handwashing on paper, but if the infrastructure to do it properly does not exist, the requirement is unenforceable at the moment it matters most.

Food from unapproved or unknown sources, cited at Ming Garden, Capybara Ramen, Applebee's, Olive Garden, Denny's, Cooper's Hawk, Michi Ramen, La Estacion de la Arepa, East Garden, and Cooper's Hawk, removes the chain of traceability that makes outbreak investigation possible. When a USDA or FDA-inspected supplier ships contaminated product, there is a paper trail. When the source is unknown, there is not.

Failure to follow parasite destruction procedures, found at Ming Garden, Authentic Gyros and Grill, Capybara Ramen, Applebee's, and Denny's, is specific to raw or undercooked fish and certain meats. Parasites including Anisakis in fish and Trichinella in pork survive without proper freezing protocols. For a ramen and sushi restaurant, a gyros and grill, and a national chain all drawing this violation in the same week, the pattern is not coincidental.

The Longer Record

A Gourmet Chinese Cuisine has 42 prior inspections on record, the longest history of any facility in this week's data. Three high-severity violations and an improper sewage disposal citation at a restaurant with that many inspections is not a new problem being discovered. The same applies to Zen Dumpling on N Alafaya Trail, which has 40 prior inspections and drew two high-severity violations this week.

Ming Garden has 34 prior inspections and produced the worst single-visit count in the week, 12 high-severity findings. Authentic Gyros and Grill has 41 prior inspections and drew five high-severity violations including sewage disposal failures. Cooper's Hawk has 33 prior inspections. These are not facilities encountering inspectors for the first time.

Michi Ramen and La Estacion de la Arepa each have only two prior inspections on record. Both drew eight high-severity violations this week, the joint-highest count in the data alongside Pho Bar, Olive Garden, Denny's, and Cooper's Hawk. For Michi Ramen, the violations include food from an unapproved source and inadequate handwashing facilities, foundational failures on a second inspection. La Estacion de la Arepa had no person in charge present, no employee health policy, and food from an unapproved source in what amounts to an early introduction to inspectors.

Pho Bar Vietnamese Kitchen has five prior inspections. It drew eight high-severity violations this week, including food not cooked to the required minimum temperature and inadequate handwashing facilities. Whether Pho Bar's record improves from here or continues on the trajectory of facilities like A Gourmet Chinese Cuisine, which has 42 inspections behind it and is still drawing high-severity violations, is a question the records do not yet answer.
