Bambu Mexican Restaurant Inc at 5731 S Orange Blossom Trail was ordered shut on April 23 for roach and fly activity. That same day, Taqueria Ameca Margarita LLC at 4400 S Orange Blossom Trail was closed for the same reason, roach and fly activity documented by inspectors.

The Violations

The week's highest violation counts came from two restaurants that could hardly be more different in setting. Baires Grill at 8050 International Drive, a tourist-corridor steakhouse, drew 10 high-severity citations including food from unapproved or unknown sources, failure to follow parasite destruction procedures for fish, food not cooked to required minimum temperatures, and no written employee health policy. Inspectors also cited improper handwashing technique and food in poor condition or adulterated.

Nikki's Place at 742 W Carter St matched that count with 10 high-severity violations of its own. Those included food from unapproved sources, inadequate shell stock identification records, no parasite destruction procedures, no consumer advisory for raw or undercooked foods, and employees not reporting illness symptoms, in addition to no written health policy and improperly sanitized food contact surfaces.

China Lee at 2338 S Kirkman Rd drew 9 high-severity violations. Inspectors cited employees not reporting illness symptoms, inadequate handwashing by food employees, improper handwashing technique, inadequate shell stock identification records, food contact surfaces not properly cleaned, and no consumer advisory for raw or undercooked foods.

Taverna Opa Orlando at 9101 International Drive also drew 9 high-severity violations. The person in charge was cited as not present or not performing duties, employees were not reporting illness symptoms, handwashing technique was cited as improper, shell stock identification records were inadequate, and food was not cooked to required minimum temperatures.

China Garden at 2550 W Colonial Drive rounded out the nine-violation tier. Inspectors cited food from unapproved sources, inadequate shell stock records, no consumer advisory for raw foods, and toxic chemicals improperly stored or labeled, alongside employee illness reporting failures and improper handwashing.

Zen Dumpling at 423 N Alafaya Trail drew 8 high-severity violations including inadequate handwashing facilities, food not cooked to required temperatures, toxic chemicals improperly stored, and no allergen awareness demonstrated by staff. The allergen citation is notable: inspectors found no evidence that employees could identify or communicate allergen risks to customers.

Lotus Garden at 7536 Dr Phillips Blvd also drew 8 high-severity violations, including food from unapproved sources, inadequate shell stock records, food not cooked to minimum temperatures, and toxic chemicals improperly stored or labeled.

JoJo's Shake Bar at 9101 International Drive, in the same building as Taverna Opa, drew 8 high-severity violations. Inspectors cited food in poor condition or adulterated, food not cooked to minimum temperatures, toxic substances improperly identified or stored, and no allergen awareness demonstrated.

Mama Lau and OC LLC at 5038 W Colonial Drive drew 7 high-severity violations including food from unapproved sources, failure to follow required procedures for specialized processes, toxic chemicals improperly stored, and food not cooked to minimum temperatures.

Landmark Center / Omega Deli at 315 Robinson Ave drew 7 high-severity violations including food from unapproved sources, parasite destruction procedures not followed, toxic substances improperly stored, and an intermediate citation for improper sewage or wastewater disposal.

East Wok at 13807 Landstar Blvd drew 7 high-severity violations including inadequate handwashing facilities, improper handwashing technique, parasite destruction procedures not followed, and toxic chemicals improperly stored. Inspectors also cited inadequate cooling and cold holding equipment as an intermediate violation.

Gators Dockside at 4982 New Broad St drew 7 high-severity violations. Inspectors cited food from unapproved sources, food not cooked to required minimum temperatures, time as a public health control not properly used, no consumer advisory for raw foods, and toxic chemicals improperly stored.

Bento at 863 N Alafaya Trail drew 7 high-severity violations including food from unapproved sources, inadequate shell stock records, parasite destruction procedures not followed, and toxic chemicals improperly stored.

YH Seafood Clubhouse at 8081 Turkey Lake Rd drew 2 high-severity violations: the person in charge was not present or not performing duties, and food contact surfaces were not properly cleaned or sanitized.

What These Violations Mean

The single most common high-severity citation this week, appearing at Baires Grill, Nikki's Place, China Lee, Taverna Opa, China Garden, JoJo's Shake Bar, East Wok, Landmark Center/Omega Deli, and Hi 5 Yummy, was the absence of a written employee health policy or employees not reporting illness symptoms. These are not paperwork violations. Without a formal policy requiring sick workers to stay home, a single employee with norovirus can infect dozens of customers before anyone realizes what is happening. Norovirus is responsible for 20 million illnesses in the United States every year, and food workers are among its most efficient transmission routes.

The parasite destruction citations at Baires Grill, Nikki's Place, East Wok, Landmark Center/Omega Deli, and Bento carry a different kind of risk. Fish served raw or undercooked, including sushi, ceviche, and certain preparations of salmon or tuna, must be frozen to specific temperatures for specific durations before service to kill parasites including Anisakis and tapeworm larvae. When that step is skipped, the parasites survive into the finished dish.

Food from unapproved sources, cited at Baires Grill, Nikki's Place, China Garden, Lotus Garden, Mama Lau, Landmark Center/Omega Deli, Gators Dockside, and Bento, means inspectors could not confirm that the food came through a USDA or FDA-inspected supply chain. If someone gets sick, there is no traceable origin for the ingredient. That makes outbreak investigation nearly impossible and puts any recall out of reach.

Toxic chemicals improperly stored or labeled, cited at China Garden, Zen Dumpling, Lotus Garden, Hi 5 Yummy at 5683 Vineland Rd, Mama Lau, East Wok, Gators Dockside, and Bento, means cleaning agents, sanitizers, or pesticides were found near food or without proper labeling. A mislabeled spray bottle used on a food prep surface can cause acute chemical poisoning in customers with no warning.

The Longer Record

China Garden on W Colonial Drive has 70 prior inspections on record, the longest history of any facility cited this week. That volume of inspections across years of operation, combined with 9 high-severity violations in a single visit, including food from unapproved sources and toxic chemicals improperly stored, suggests the problems documented last week are not new ones.

China Lee on S Kirkman Road has 64 prior inspections on record and drew 9 high-severity violations this week. Employees at China Lee were cited for both failing to report illness symptoms and for inadequate handwashing, two violations that, together, describe a facility where a sick worker has both the opportunity and the means to contaminate food.

Mama Lau on W Colonial Drive has 32 prior inspections on record. Bento on N Alafaya Trail has 31. Taverna Opa has 30. All three drew 7 or more high-severity violations this week, and none of their inspection histories appear to have produced lasting correction in the categories cited.

Baires Grill, the week's co-leader with 10 high-severity violations, has only 2 prior inspections on record. It is among the newest facilities in this week's data, and it is already accumulating the kind of citation list that older establishments have built across dozens of visits.

JoJo's Shake Bar has 9 prior inspections on record and drew 8 high-severity violations this week, including no allergen awareness demonstrated by staff. The allergen citation at a shake bar, where dairy, nuts, and other common allergens are routine ingredients, remains unresolved in the public record.
