MIAMI, FL. Club De La Milanesa at 3250 NE 1 Ave Unit 110 was emergency-ordered closed on August 25 after inspectors documented simultaneous rodent activity, roach activity, and fly activity inside the restaurant, one of four Miami establishments shuttered by the state during a single week.

The four closures came as state inspectors also documented high-severity violations at 15 other Miami facilities during the week of August 25 through August 31, 2026. The same building that housed Club De La Milanesa also produced a second closure: Ichimi at 3250 NE 1 Ave #118 was shut down on August 26 for rodent activity.

Giardino Gourmet Salads at 11624 SW 88 St was closed August 27 for fly activity. Joanna's Marketplace at 8247 S Dixie Hwy was closed August 25, also for fly activity. Two closures on the same day, in two different parts of the city.

Rinconcito Miami 2 at 15956 SW 137 Ave drew the highest single-facility violation count of the week, with 14 high-severity citations. Inspectors found no person in charge present or performing duties, employees not reporting illness symptoms, improper handwashing technique, food from unapproved sources, inadequate shell stock records, parasite destruction procedures not followed, unclean food contact surfaces, and food not cooked to required minimum temperatures. Eight distinct high-severity categories in a single visit.

100 Montaditos Midtown at 3252 NE 1 Ave Ste 104 followed with 12 high-severity violations. The citations included no written employee health policy, employees not reporting illness symptoms, food from unapproved sources, inadequate shell stock records, improperly cleaned food contact surfaces, and food not cooked to minimum temperature. The absence of a person in charge was also documented.

Cafe Panisse at 7310 SW 57 Ave drew 9 high-severity violations including food in poor condition or mislabeled, no consumer advisory for raw or undercooked foods, and toxic chemicals improperly stored or labeled. Inspectors also cited inadequate shell stock records and unclean food contact surfaces.

Tropical Smoothie at 13550 SW 120 St accumulated 10 high-severity violations. The citations included no person in charge, no employee health policy, employees not reporting illness, food from unapproved sources, inadequate shell stock records, food not cooked to minimum temperature, and a failure to properly use time as a public health control.

Budare Bistro at 1832 SW 3 Ave also reached 10 high-severity violations. Inspectors cited both inadequate handwashing by employees and improper handwashing technique, a double citation that captures a kitchen where the practice is broken and the execution is wrong even when attempted. Food from unapproved sources and food not cooked to minimum temperature were also documented, along with no consumer advisory for raw or undercooked items.

Baires Grill at 1010 S Miami Ave drew 10 high-severity violations including parasite destruction procedures not followed, food from unapproved sources, improper time-as-public-health-control use, and no consumer advisory. The absence of an employee health policy was also cited.

OL'Days at 3301 NE 1 Ave Ste 103-1, Friends Market and Bistro at 31 SE 6 St Unit CU, and Sea Siam at 12735 S Dixie Hwy each drew 10 high-severity violations.

Bagel Express at 11616 SW 88 St posted the highest intermediate violation count of any facility this week, with 7 intermediate citations alongside 9 high-severity ones. Food in poor condition or mislabeled, inadequate shell stock records, toxic chemicals improperly stored, no consumer advisory, and unclean food contact surfaces were all documented.

Di Napoli at 11755 S Dixie Hwy drew 5 high-severity violations including food from unapproved sources, food not cooked to minimum temperature, no consumer advisory for raw or undercooked foods, and toxic chemicals improperly stored or labeled.

Italica Restaurant at 3201 NE 1 Ave was cited for inadequate handwashing facilities, meaning inspectors found the physical infrastructure for handwashing was itself insufficient. That citation appeared alongside improper handwashing technique, food in poor condition, improperly stored toxic substances, and no consumer advisory.

Sokai Sushi Bar Kendall at 11735 SW 147 Ave Unit 36 and Brickell City Pizza at 75 SE 6 St #102 each drew 9 high-severity violations. Sokai's citations included no employee health policy, employees not reporting illness, improper time-as-public-health-control use, and toxic chemicals improperly stored. Brickell City Pizza was cited for food in poor condition, food not cooked to minimum temperature, inadequate shell stock records, and toxic chemicals improperly stored.

What These Violations Mean

The most frequently cited violation this week was improper handwashing technique, appearing at Rinconcito Miami 2, Cafe Panisse, 100 Montaditos Midtown, Tropical Smoothie, Budare Bistro, Baires Grill, OL'Days, Friends Market and Bistro, Sea Siam, Bagel Express, Sokai Sushi Bar Kendall, and Italica Restaurant. Improper technique means pathogens remain on hands even when a worker attempts to wash. The hands then touch food, prep surfaces, and utensils.

Italica Restaurant's citation for inadequate handwashing facilities goes one level deeper. When the sink, soap, or water supply is itself deficient, proper hand hygiene is structurally impossible regardless of employee intention. That citation alongside an improper technique citation at the same facility means both the infrastructure and the practice were broken simultaneously.

Food from unapproved or unknown sources was documented at Rinconcito Miami 2, 100 Montaditos Midtown, Tropical Smoothie, Budare Bistro, Baires Grill, Friends Market and Bistro, Sea Siam, and Di Napoli. This violation matters most when something goes wrong. If a customer becomes ill, investigators trace the outbreak through the food supply chain. Food that entered a kitchen without documentation cannot be traced, recalled, or linked to a source. The contamination event becomes impossible to contain.

Parasite destruction procedures not followed appeared at Rinconcito Miami 2, Baires Grill, and Sea Siam. For restaurants serving raw or lightly cooked fish, the state requires that fish either be frozen to specific temperatures for specific durations or sourced from suppliers who have done so. Without that step, parasites including Anisakis and tapeworm can survive into the finished dish. Sea Siam and Baires Grill both also lacked consumer advisories for raw or undercooked foods, meaning customers eating those items had no notice.

Di Napoli at 11755 S Dixie Hwy has 33 prior inspections on record, the longest history of any facility cited this week. Despite that accumulated inspection history, this visit still produced 5 high-severity violations including unapproved food sources, minimum temperature failures, and improperly stored toxic chemicals.

Cafe Panisse, OL'Days, Sea Siam, Ichimi, and Sokai Sushi Bar Kendall each carry 30 prior inspections. Ichimi was emergency-closed this week for rodent activity despite those 30 prior visits. OL'Days drew 10 high-severity violations at the same inspection depth.

100 Montaditos Midtown has only 12 prior inspections on record, the shortest history among the high-severity facilities. It still produced 12 high-severity violations this week. Rinconcito Miami 2 has 28 prior inspections and produced the week's worst violation count at 14 high-severity citations.
