MIAMI, FL. Tribute to Tobacco Road by Kush at 650 S Miami Ave drew 14 high-severity violations during the week of May 6, the highest single-facility tally among 15 Miami-Dade restaurants cited for serious food safety failures in state inspection records reviewed this week.

The violations at the Brickell restaurant covered nearly every layer of food safety management. Inspectors found no person in charge present or performing duties, no written employee health policy, employees not reporting illness symptoms, and both inadequate handwashing by employees and improper handwashing technique, five separate failure points in the chain that is supposed to keep sick workers away from food.

The records also showed food from unapproved or unknown sources, inadequate shell stock identification for shellfish, and parasite destruction procedures not followed for fish or other susceptible proteins.

Bonding at 638 S Miami Ave, just down the block from Tobacco Road, was the second-worst facility of the week with 10 high-severity violations. Inspectors cited inadequate handwashing, food from unapproved sources, food in poor condition, inadequate shell stock records, improperly cleaned food contact surfaces, food not cooked to required minimum temperature, improper use of time as a public health control, and no consumer advisory for raw or undercooked foods.

El Cantones Rest at 11865 SW 26 St drew 9 high-severity violations, including no person in charge, no employee health policy, inadequate handwashing facilities, improper handwashing technique, food in poor condition, improperly cleaned food contact surfaces, food not cooked to required minimum temperature, and no consumer advisory. The citation for inadequate handwashing facilities means the physical infrastructure for basic hygiene was not in place.

Bahamas Fish Market and Restaurant #2 LLC at 13399 SW 42 St also drew 9 high-severity violations. The list included no person in charge, no employee health policy, improper handwashing technique, food in poor condition, improperly cleaned food contact surfaces, time abuse violations, no consumer advisory, and toxic chemicals improperly stored or labeled near food.

Mojitos Cuban Cuisine at 8000 SW 8th Street matched that count. Inspectors found no person in charge, no employee health policy, employees not reporting illness symptoms, inadequate handwashing facilities, improper handwashing technique, food from unapproved sources, inadequate shell stock records, and improperly cleaned food contact surfaces.

KPOT Korean BBQ and Hot Pot at 8255 W Flagler St also recorded 9 high-severity violations, including no person in charge, no employee health policy, employees not reporting illness, improper handwashing technique, food from unapproved sources, inadequate shell stock records, improperly cleaned food contact surfaces, and time abuse.

Moshi Moshi Brickell at 1700 SW 3 Ave drew 7 high-severity violations. The citation list included parasite destruction procedures not followed, food contact surfaces not properly cleaned, food not cooked to required minimum temperature, time as a public health control not properly used, no consumer advisory, and toxic chemicals improperly stored. Improperly cleaned multi-use utensils were cited as an intermediate violation.

Rinconcito Dadeland Midtown Inc at 7360 SW 90 St drew 8 high-severity violations, including food from unapproved sources, improperly cleaned food contact surfaces, food not cooked to required minimum temperature, time abuse, no consumer advisory, toxic chemicals improperly stored, and no allergen awareness demonstrated. That last citation means inspectors found no evidence staff could identify or communicate allergen risks to customers.

New Canton at 1825 SW 8 St recorded 8 high-severity violations: inadequate handwashing, food from unapproved sources, food in poor condition, improperly cleaned food contact surfaces, food not cooked to minimum temperature, time abuse, no consumer advisory, and toxic substances improperly identified, stored, or used.

Cayo Esquivel at 7725 SW 40 St drew 7 high-severity violations, including an employee not reporting illness symptoms, food from unapproved sources, inadequate shell stock records, improperly cleaned food contact surfaces, time abuse, no consumer advisory, and toxic chemicals improperly stored.

Inspectors also documented inadequate cooling or cold holding equipment, meaning the physical capacity to keep food at safe temperatures was in question.

Uptown Buffet at 8275 W Flagler St drew 2 high-severity violations, the lowest tally in the group, for improperly cleaned food contact surfaces and no consumer advisory for raw or undercooked foods.

On May 8, state inspectors emergency-closed two locations operating under the name La Vaca Loca Farm: the unit at 13941 SW 143 Ct for fly activity, and the property at 17950 SW 177 Ave for unlicensed activity. Both were shuttered the same day.

What These Violations Mean

The management failures documented this week are the category that tends to precede everything else going wrong. At Tribute to Tobacco Road, El Cantones, Bahamas Fish Market, Mojitos, KPOT, and Guarapo, inspectors found no person in charge present or performing duties. CDC data cited in the inspection records indicates establishments without active managerial control accumulate critical violations at three times the rate of those with a responsible manager on site. When no one is accountable, the cascade follows.

The illness-reporting failures documented at Tribute to Tobacco Road, Mojitos, KPOT, Cayo Esquivel, Guarapo, and Don Domingo are among the most direct transmission risks in a food service setting. Norovirus, which causes the majority of foodborne illness outbreaks in the United States, spreads readily when a sick employee continues working. A written health policy and the expectation that workers report symptoms are the primary barriers between a sick cook and a sick customer. Neither barrier was confirmed in place at those six facilities.

The parasite destruction citations at Tribute to Tobacco Road, Moshi Moshi Brickell, and Don Domingo Cafe Parrillada Argentina are specific to proteins served raw or lightly cooked. Sushi, ceviche, and certain grilled meats require either proper freezing or confirmed cooking temperatures to kill parasites including Anisakis in fish and Trichinella in pork. Without documentation that those protocols were followed, there is no way to verify the food was safe when it reached the table.

Toxic chemical citations appeared at Bahamas Fish Market, Me Kong, Cayo Esquivel, Don Domingo, Taipa, Rinconcito, New Canton, and Moshi Moshi. When cleaning agents or pesticides are stored adjacent to food or in unlabeled containers, the contamination risk is immediate and the source may not be identifiable until someone becomes ill.

The Longer Record

Uptown Buffet has the longest inspection history in this week's group, with 38 prior inspections on record, and it still drew citations for improperly cleaned food contact surfaces. Cayo Esquivel has 34 prior inspections. El Cantones and Bonding each have 29.

Me Kong Chinese Restaurant on S Dixie Hwy has 32 prior inspections on record and drew 6 high-severity violations this week, including a sewage disposal citation and the reuse of single-use items. Rinconcito Dadeland has 31 prior inspections and still lacked demonstrated allergen awareness. These are not new restaurants working out early problems.

KPOT Korean BBQ and Hot Pot on W Flagler has only 2 prior inspections on record. In that short history it has already accumulated 9 high-severity violations, including no person in charge, no employee health policy, employees not reporting illness, and food from unapproved sources. That combination of management failures at a location with almost no inspection history is the profile that tends to produce the worst outcomes.

Don Domingo Cafe Parrillada Argentina has 21 prior inspections and drew citations this week for both parasite destruction failures and no allergen awareness. Whether either violation appeared in prior inspection records is not reflected in the data available.
