JACKSONVILLE, FL. Bronx House Pizza on Beach Boulevard drew 12 high-severity violations during a single inspection the week of October 17, the worst single-facility tally among 15 Jacksonville restaurants cited for serious food safety failures in Duval County that week.

The violations at Bronx House Pizza included food sourced from unapproved suppliers, food in poor condition, no employee health policy, an employee not reporting illness symptoms, improper handwashing technique, inadequate shell stock identification, and failure to follow parasite destruction procedures for fish. No person in charge was present or performing duties during the inspection.

Cantina Louie on Touchton Road followed with 11 high-severity violations, including food from unapproved sources, inadequate handwashing, food contact surfaces not properly cleaned or sanitized, no employee health policy, and missing shell stock identification records. A person in charge was absent or not performing duties there as well.

Fryday's on Beach Boulevard also reached 11 high-severity violations. Inspectors cited the restaurant for food from unapproved sources, food in poor condition, no employee health policy, both inadequate handwashing and improper handwashing technique, missing shell stock records, improper use of time as a public health control, and no consumer advisory for raw or undercooked foods.

Broskii Fish and Chicken on Lem Turner Road drew 9 high-severity violations and 7 intermediate citations, the highest intermediate count of the week. Inspectors found toxic chemicals improperly stored or labeled alongside food, a separate citation for toxic substances improperly identified and used, food contact surfaces not properly cleaned, food from unapproved sources, and no consumer advisory for raw or undercooked items.

Caribbean Sunrise Bakery and Restaurant on North Main Street was cited for food not cooked to required minimum temperatures, a violation that inspectors also flagged at the Courtyard Jacksonville Butler Boulevard hotel kitchen on Lenoir Avenue South. Both facilities also drew citations for improper handwashing technique, no employee health policy, and employees not reporting illness symptoms.

Caribbean Sunrise was additionally cited for food from unapproved sources, failure to follow parasite destruction procedures, food contact surfaces not properly cleaned, and toxic chemicals improperly stored near food.

Tacos Dona Maria 2 on Beach Boulevard was among the facilities cited for inadequate handwashing facilities, meaning the physical infrastructure for proper hand hygiene was insufficient. Chan's Restaurant on Dunn Avenue received the same citation and was also cited for food directly contaminated by chemical, physical, or biological hazards.

Thai Orchid on Beach Boulevard drew 9 high-severity violations including toxic chemicals improperly stored or labeled, toxic substances improperly identified and used, food contact surfaces not properly cleaned, no consumer advisory for raw or undercooked foods, and improper use of time as a public health control.

Falafel X Bar on Herschel Street was cited for an intermediate violation involving improper sewage or wastewater disposal, in addition to 7 high-severity violations that included food from unapproved sources, parasite destruction failures, and food contact surfaces not properly cleaned.

La Michocana de Tocumbo on University Boulevard South and China 99 Chinese Restaurant on University Boulevard West each drew 8 high-severity violations. China 99 was also cited for no allergen awareness demonstrated by staff, a violation not found at any other facility inspected this week.

Three facilities, Hong Kong Jax Restaurant on Beach Boulevard, China Wok on Deerwood Lake Parkway, and Hot Wok on Emerson Street, drew zero high-severity violations. Each received a single intermediate citation.

What These Violations Mean

The food-from-unapproved-sources citation, found at Bronx House Pizza, Cantina Louie, Fryday's, Broskii Fish and Chicken, Caribbean Sunrise Bakery, Tacos Dona Maria 2, Chan's Restaurant, and Falafel X Bar, is not a paperwork problem. When food bypasses USDA and FDA inspection channels, there is no traceability if a customer gets sick. Investigators cannot identify the source, cannot issue a recall, and cannot determine how many other restaurants received the same product.

The absence of a written employee health policy, cited at ten of the fifteen facilities this week, removes the formal mechanism that requires sick workers to report symptoms and stay off the line. Without that policy, a worker with Norovirus has no documented obligation to report illness, and Norovirus spreads person-to-person with an infectious dose of fewer than 20 viral particles. The combination of no health policy and employees not reporting symptoms, documented together at Caribbean Sunrise Bakery, Cantina Louie, Broskii Fish and Chicken, and the Courtyard hotel kitchen, means both the rule and the compliance with it were absent simultaneously.

The cooking temperature violations at Caribbean Sunrise Bakery and the Courtyard hotel kitchen carry a specific danger: Salmonella in poultry survives below 165 degrees Fahrenheit. A piece of chicken that looks fully cooked can still harbor live bacteria if internal temperatures were not verified and recorded. Neither facility is a fast-food operation where temperature logging is automated.

Toxic chemical storage violations at Broskii Fish and Chicken, Caribbean Sunrise Bakery, Thai Orchid, China 99, and the Courtyard hotel kitchen represent an acute poisoning risk, not a theoretical one. Cleaning chemicals stored near or above food preparation surfaces can contaminate food directly through spills or aerosol drift. When those chemicals are also improperly labeled, as cited at Thai Orchid and Broskii Fish and Chicken, staff cannot identify what they are handling or what the exposure risk is.

The Longer Record

Cantina Louie has 43 prior inspections on record, the longest inspection history of any facility cited this week. Eleven high-severity violations at a location that has been inspected more than four dozen times total is a pattern, not an anomaly. Caribbean Sunrise Bakery and Restaurant has 41 prior inspections and this week drew 9 high-severity violations including a cooking temperature failure and chemical storage violations alongside food.

China 99 has 33 prior inspections on record. China Wok, which passed this week with a single intermediate citation, also has 33 prior inspections. Hot Wok on Emerson Street has 30 prior inspections and also cleared this week with only a minor citation. That three facilities with substantial inspection histories came through the week cleanly, while others with similar or shorter records accumulated eight, nine, and eleven high-severity violations, underscores that history alone does not predict compliance.

La Michocana de Tocumbo is the newest location in this week's data with only 3 prior inspections on record. It drew 8 high-severity violations, including parasite destruction failures, missing shell stock records, no employee health policy, no consumer advisory for raw or undercooked foods, and food in poor condition. A facility that has barely been open long enough to build an inspection history is already accumulating citations in the most serious category.

Falafel X Bar on Herschel Street has 15 prior inspections and this week drew 7 high-severity violations plus an intermediate citation for improper sewage or wastewater disposal. The sewage citation was not resolved in the inspection record available for this week.
