JACKSONVILLE, FL. Bronx House Pizza on Beach Boulevard racked up 12 high-severity violations in a single inspection this week, including food sourced from unapproved suppliers, inadequate shellfish traceability records, and no written employee health policy in place, state records show.

The Beach Boulevard location was the worst performer among 15 Jacksonville restaurants cited for high-severity violations during the week of October 16 through October 22, 2026. Inspectors also documented that no person in charge was present or performing duties, that employees were not reporting illness symptoms, and that parasite destruction procedures for fish were not being followed.

Cantina Louie on Touchton Road drew 11 high-severity violations, among them food from unapproved sources, food contact surfaces not properly cleaned or sanitized, and inadequate shellfish identification records. Like Bronx House Pizza, Cantina Louie had no person in charge present and no written employee health policy.

Fryday's on Beach Boulevard also reached 11 high-severity violations. Inspectors cited both inadequate handwashing by employees and improper handwashing technique, meaning the facility failed on the practice itself and on the method used when a handwashing attempt was made. Fryday's was also cited for not using time as a public health control properly and for lacking a consumer advisory for raw or undercooked foods.

Broskii Fish and Chicken on Lem Turner Road accumulated nine high-severity violations, including toxic chemicals improperly stored or labeled and toxic substances improperly identified, stored, or used. Both chemical violations appeared in the same inspection, alongside food from unapproved sources and food contact surfaces not properly cleaned.

Caribbean Sunrise Bakery and Restaurant on North Main Street was cited for food not cooked to the required minimum temperature, one of the most direct pathogen-survival violations an inspector can document. The same inspection found food from unapproved sources, parasite destruction procedures not followed, and toxic chemicals improperly stored.

Thai Orchid on Beach Boulevard drew nine high-severity violations including both chemical storage violations, improper time-as-public-health-control use, and no consumer advisory for raw or undercooked items. Inspectors also cited food contact surfaces not properly cleaned or sanitized.

Tacos Dona Maria 2 on Beach Boulevard was cited for inadequate handwashing facilities, meaning inspectors found the physical infrastructure for proper hand hygiene was insufficient, in addition to improper handwashing technique and employees not reporting illness symptoms.

Courtyard Jacksonville Butler Boulevard on Lenoir Avenue South drew eight high-severity violations including food not cooked to required minimum temperature and required procedures for specialized processes not followed. The Marriott-branded hotel property also had no person in charge present and no employee health policy.

La Michocana de Tocumbo on University Boulevard South was cited for parasite destruction procedures not followed and inadequate shellfish identification records, in addition to no employee health policy and employees not reporting illness symptoms.

China 99 Chinese Restaurant on University Boulevard West drew eight high-severity violations including no allergen awareness demonstrated, a citation that documents inspectors found staff unable to identify or communicate allergen information to customers. Toxic chemicals were also improperly stored or labeled.

China Wok on Deerwood Lake Parkway was cited for seven high-severity violations including time as a public health control not properly used and food contact surfaces not properly cleaned. Inspectors also noted multi-use utensils not properly cleaned, a finding that compounds the surface contamination risk.

Falafel X Bar on Herschel Street drew seven high-severity violations including improper sewage or wastewater disposal, a finding that signals potential fecal contamination risk throughout the facility. Parasite destruction procedures were also not followed.

TownePlace Suites on Lenoir Avenue was cited for six high-severity violations including no allergen awareness demonstrated and improper sewage or wastewater disposal. Inspectors also documented inadequate ventilation and lighting in the same visit.

Two facilities came through the week without high-severity violations. Hong Kong Jax Restaurant on Beach Boulevard received a single intermediate citation for multi-use utensils not properly cleaned. Hot Wok on Emerson Street drew one intermediate violation for single-use items improperly reused.

What These Violations Mean

The food-from-unapproved-sources citation, documented at Bronx House Pizza, Cantina Louie, Fryday's, Broskii Fish and Chicken, Caribbean Sunrise Bakery, Tacos Dona Maria 2, and Falafel X Bar, is not a paperwork problem. When food enters a restaurant through channels that bypass USDA and FDA inspection, there is no traceability if a customer becomes ill. An outbreak can start and investigators may have no supply chain to trace back.

Parasite destruction failures, cited at Bronx House Pizza, Caribbean Sunrise Bakery, La Michocana de Tocumbo, and Falafel X Bar, matter most when a restaurant serves fish or pork that is raw, lightly cooked, or prepared for dishes where internal temperatures never reach the threshold needed to kill organisms like Anisakis or Trichinella. The required protocol is specific freezing at precise temperatures for precise durations. When it is not followed, parasites can survive to the plate.

The food-not-cooked-to-minimum-temperature violations at Caribbean Sunrise Bakery and Courtyard Jacksonville Butler Boulevard represent one of the most direct illness pathways in food service. Salmonella in poultry survives below 165 degrees Fahrenheit. When a kitchen skips or shortcuts the cooking step, the pathogen reaches the customer intact.

The dual chemical violations at Broskii Fish and Chicken, Thai Orchid, China 99, and China Wok, covering both improper labeling and improper storage, create conditions where a cleaning chemical can contaminate food either through proximity or through a mislabeled container that an employee uses incorrectly. The risk is acute poisoning, not the gradual risk associated with most food safety failures.

The Longer Record

Cantina Louie carries the longest inspection history among this week's worst performers, with 43 prior inspections on record. Eleven high-severity violations in a single visit at a facility that has been inspected that many times is a pattern, not an anomaly. Caribbean Sunrise Bakery on North Main Street has 41 prior inspections on record and this week produced nine high-severity violations, including the food-not-cooked-to-temperature citation.

China 99 on University Boulevard West has 33 prior inspections on record. China Wok has 31. Both produced high-severity violation totals this week, and both have been through enough inspections that the findings cannot be attributed to unfamiliarity with state requirements.

Broskii Fish and Chicken, Fryday's, and Thai Orchid each have between 28 and 29 prior inspections on record. All three produced nine or more high-severity violations this week.

La Michocana de Tocumbo on University Boulevard South stands apart from the rest of this list. The facility has only three prior inspections on record, making it among the newest establishments cited this week. Eight high-severity violations in what amounts to an early inspection history, including parasite destruction failures and no consumer advisory for raw or undercooked foods, suggests the problems are not the result of gradual drift from prior standards.

TownePlace Suites and Falafel X Bar each have 15 prior inspections on record. Both produced violations this week that go beyond the most common administrative failures, with sewage disposal citations appearing at both locations.

Bronx House Pizza, which led the week with 12 high-severity violations, has 25 prior inspections on record. Whether those earlier visits produced similar findings is not reflected in this week's data.
