JACKSONVILLE, FL. City Cafe on Salisbury Road drew 11 high-severity violations during the week of August 16, including food sourced from unapproved or unknown suppliers, inadequate shellfish traceability records, parasite destruction procedures not followed, and no person in charge present or performing duties during the inspection.

That tally put City Cafe at the top of a troubling week for Jacksonville restaurants. State inspectors cited 15 facilities for high-severity violations between August 16 and August 22, 2026, with three restaurants each reaching 11 high-severity findings and several others close behind.

The Violations

8LOWHot Wok, 3715 Emerson St0 high-severity

City Cafe's inspection also turned up employees not reporting symptoms of illness, improper handwashing technique, food in poor condition, and food contact surfaces not properly cleaned or sanitized. The combination of unknown food sourcing and absent managerial oversight is among the most serious clusters an inspector can document in a single visit.

La Nueva Nota on Southside Boulevard matched that count with its own 11 high-severity findings. Inspectors cited the restaurant for inadequate handwashing by food employees, improper handwashing technique, no employee health policy, employees not reporting illness symptoms, food in poor condition, food contact surfaces not properly sanitized, food not cooked to required minimum temperature, and no consumer advisory for raw or undercooked foods.

Ciao Trattoria Italiana on Belfort Road also reached 11 high-severity violations. The findings there included food contaminated by chemical, physical, or biological hazards, food from unapproved sources, inadequate shellfish identification records, and no person in charge. That chemical contamination citation is among the more acute findings of the week.

Bono's BBQ on Bowden Road accumulated 10 high-severity violations, including food from unapproved sources, parasite destruction procedures not followed, shellfish traceability failures, and time as a public health control not properly used. The parasite destruction citation means fish or pork served at the restaurant may not have been subjected to the freezing or cooking protocols required to kill organisms like Anisakis or Trichinella.

Rio Restaurant and Lounge on Philips Highway also drew 10 high-severity violations. Inspectors found no person in charge, no employee health policy, employees not reporting illness, inadequate handwashing, food in poor condition, unsanitized food contact surfaces, improper use of time as a public health control, and no consumer advisory for raw foods.

Waffle House on Lem Turner Road was cited for nine high-severity violations, including toxic chemicals improperly stored or labeled, food not cooked to required minimum temperature, food from unapproved sources, and no consumer advisory. A Waffle House location accumulating nine high-severity findings in a single inspection, including a chemical storage violation, is notable given the chain's high customer volume and 24-hour operation.

More Facilities, More Findings

Hot Wok on Argyle Forest Boulevard drew eight high-severity violations, including toxic chemicals improperly stored or labeled, no allergen awareness demonstrated, time as a public health control not properly used, shellfish traceability failures, and employees not reporting illness. A separate Hot Wok location on Emerson Street received no high-severity violations, only a single intermediate citation for improperly reused single-use items.

Flying Fish Taphouse on Airport Road was cited for eight high-severity violations, including two separate toxic substance citations: chemicals improperly stored or labeled, and toxic substances improperly identified, stored, or used. Inspectors also found no person in charge, food not cooked to required minimum temperature, and no consumer advisory for raw or undercooked foods.

Sushi Garden on Oldfield Crossing Drive reached eight high-severity violations as well, including inadequate handwashing facilities, meaning the physical infrastructure for proper hand hygiene was itself found deficient. Inspectors also cited the restaurant for no person in charge, no employee health policy, employees not reporting illness, shellfish traceability failures, and no consumer advisory.

Tea Poke Spot on Belfort Road drew eight high-severity violations, including toxic chemicals improperly stored, food in poor condition, shellfish traceability failures, and no consumer advisory. Yan Sushi on Bowden Road also reached eight high-severity violations, with findings that included no allergen awareness, food not cooked to minimum temperature, time as a public health control not properly used, and toxic chemicals improperly stored.

Jax Fish Shrimp and Chicken on Blanding Boulevard drew one high-severity violation for no employee health policy, alongside an intermediate citation for improper sewage or wastewater disposal. Tony's Cantina Mexican Grill on San Jose Boulevard received one high-severity violation for no consumer advisory on raw or undercooked foods. Kazu Japanese Restaurant on San Jose Boulevard drew a single high-severity citation for employees not reporting illness symptoms.

What These Violations Mean

The most common high-severity finding this week, appearing at City Cafe, La Nueva Nota, Ciao Trattoria, Bono's BBQ, Rio Restaurant, Flying Fish Taphouse, Sushi Garden, Tea Poke Spot, Yan Sushi, Hot Wok on Argyle Forest, Waffle House, and Kazu, was employees not reporting symptoms of illness. Food workers shedding Norovirus before they feel fully sick can contaminate surfaces, utensils, and food directly. A single infected employee working a busy shift at a high-volume location is enough to produce a multi-victim outbreak.

The food from unapproved sources violation, documented at City Cafe, Ciao Trattoria, Bono's BBQ, and Waffle House, matters for a specific reason: traceability. When food enters a restaurant from an uninspected or unknown supplier, there is no chain of records connecting a sick customer to a lot number, a farm, or a distributor. If someone becomes ill, investigators have nowhere to start.

Parasite destruction failures at Bono's BBQ and City Cafe represent a different category of risk. Anisakis, a parasite found in raw or undercooked fish, causes severe gastrointestinal illness. Trichinella, found in undercooked pork, can cause muscle inflammation. The required protocols, specific freezing temperatures held for specific durations, exist because cooking alone does not always kill these organisms at the temperatures most customers would consider acceptable.

Toxic chemical violations at Waffle House, Hot Wok on Argyle Forest, Flying Fish Taphouse, Tea Poke Spot, and Yan Sushi are among the least visible risks on this list but among the most acute. Sanitizers and cleaners stored near food or in unlabeled containers have caused documented poisoning cases. Flying Fish Taphouse drew two separate chemical-related citations in the same inspection, suggesting the problem was not limited to a single storage location.

The Longer Record

Kazu Japanese Restaurant has 40 prior inspections on record, the second-highest count among facilities cited this week. A single high-severity violation at a location with that inspection history is a different data point than the same violation at a newer restaurant, but 40 inspections also means inspectors have been watching this location for a long time.

Hot Wok on Argyle Forest Boulevard has 42 prior inspections on record, the highest count of any facility in this week's data, and still drew eight high-severity violations. That is the longest documented history on this list and one of the week's worst outcomes.

Jax Fish Shrimp and Chicken carries 39 prior inspections. The sewage and wastewater disposal citation it received this week is an intermediate violation, but improper sewage handling introduces fecal contamination risk throughout a facility, and 39 inspections is a long record against which to still be drawing that finding.

At the other end of the history scale, Yan Sushi on Bowden Road has only three prior inspections on record and already accumulated eight high-severity violations this week, including allergen awareness failures and toxic chemical storage problems. Rio Restaurant and Lounge on Philips Highway has nine prior inspections and drew 10 high-severity violations, including no person in charge and no employee health policy. Both locations are relatively new to the inspection record and already generating some of the week's most serious findings.

City Cafe and La Nueva Nota each have 26 prior inspections and each reached 11 high-severity violations this week. Whether those histories include prior citations in the same categories, food sourcing, illness reporting, handwashing, is a question the full inspection records would answer.
