JACKSONVILLE, FL. City Cafe at 4500 Salisbury Road drew 11 high-severity violations during the week of August 15, including food sourced from unapproved or unknown suppliers, inadequate shellfish traceability records, and parasite destruction procedures that inspectors found were not being followed, all at a restaurant where no person in charge was present or performing duties when inspectors arrived.

The Salisbury Road location also flagged employees not reporting illness symptoms, improper handwashing technique, food contact surfaces that were not properly cleaned or sanitized, food in poor condition, and a failure to follow procedures for parasite destruction in fish. That last violation, combined with the unapproved sourcing citation, means inspectors could not confirm where the food came from or whether it had been handled safely before arriving at the restaurant.

Fourteen other Jacksonville restaurants were cited for high-severity violations during the same seven-day stretch, making it one of the heavier inspection weeks Duval County has seen this year.

Waffle House #2083 at 11844 Lem Turner Road collected nine high-severity violations, including food from unapproved sources, food not cooked to minimum required temperatures, toxic chemicals improperly stored or labeled, inadequate shellfish records, and no consumer advisory for undercooked items.

Hot Wok at 9680 Argyle Forest Boulevard drew eight high-severity violations, among them toxic chemicals improperly stored, no allergen awareness demonstrated, time as a public health control not properly used, and food contact surfaces not properly cleaned. A separate Hot Wok location at 3715 Emerson Street received no high-severity violations, only a single intermediate citation for improperly reused single-use items.

Flying Fish Taphouse at 1341 Airport Road had eight high-severity violations, including two separate chemical storage citations, no person in charge, food not cooked to required temperatures, and no consumer advisory.

Sushi Garden at 4268 Oldfield Crossing Drive was cited for eight high-severity violations, including inadequate handwashing facilities, no person in charge, inadequate shellfish records, and no consumer advisory for raw items, a notable cluster for a restaurant that serves raw fish.

Tea Poke Spot at 4076 Belfort Road drew eight high-severity violations, including toxic chemicals improperly stored, no employee health policy, food in poor condition, and no consumer advisory.

Yan Sushi at 5711 Bowden Road also received eight high-severity violations, including no allergen awareness, food not cooked to required temperatures, improperly used time controls, and toxic chemicals improperly stored or labeled.

Jax Fish Shrimp and Chicken at 3646 Blanding Boulevard drew one high-severity violation for having no employee health policy, alongside an intermediate citation for improper sewage or wastewater disposal. Kazu Japanese Restaurant at 9965 San Jose Boulevard received a single high-severity citation for employees not reporting illness symptoms. Tony's Cantina Mexican Grill at 10131 San Jose Boulevard was cited for one high-severity violation, the absence of a consumer advisory for raw or undercooked foods.

What These Violations Mean

The most common high-severity violation across this week's inspections was employees not reporting illness symptoms, cited at City Cafe, La Nueva Nota, Ciao Trattoria Italiana, Bono's BBQ, Rio Restaurant and Lounge, Hot Wok on Argyle Forest, Flying Fish Taphouse, Sushi Garden, Tea Poke Spot, Yan Sushi, and Kazu Japanese Restaurant. Food workers who are sick and continue handling food are the leading cause of multi-victim outbreaks. Norovirus, which causes the majority of foodborne illness outbreaks nationally, can be transmitted by a single infected worker touching surfaces or food that dozens of customers then contact.

The unapproved food source violations at City Cafe, Ciao Trattoria Italiana, Bono's BBQ, and Waffle House #2083 represent a different category of risk. When food arrives from a supplier that has not been inspected or approved by state or federal regulators, there is no paper trail if someone gets sick. Inspectors cannot trace the food back to its origin, and the facility cannot demonstrate that the product was handled safely before it arrived.

Shellfish traceability failures, cited at City Cafe, Bono's BBQ, Waffle House #2083, Hot Wok on Argyle Forest, Sushi Garden, Tea Poke Spot, and Yan Sushi, carry a specific risk because oysters, clams, and mussels are frequently eaten raw or lightly cooked. State law requires facilities to keep shellfish tags on file so that, if a customer becomes ill, inspectors can identify the harvest location and pull affected product. Without those records, an outbreak investigation stalls before it starts.

Chemical storage violations at Waffle House #2083, Hot Wok on Argyle Forest, Flying Fish Taphouse, Tea Poke Spot, and Yan Sushi point to a hazard that can cause immediate harm rather than the delayed onset typical of bacterial illness. Cleaning chemicals stored near food, or mislabeled containers mistaken for food-safe products, can cause acute poisoning. Flying Fish Taphouse drew two separate chemical-related citations in the same inspection.

The Longer Record

The most striking inspection history in this week's data belongs to Hot Wok on Argyle Forest Boulevard, which has 42 prior inspections on record, the highest count of any facility cited this week. Eight high-severity violations at a location that has been through more than four dozen inspection cycles suggests the corrections made after prior visits have not held. The Emerson Street Hot Wok location, with 27 prior inspections, cleared this week without a high-severity citation.

Kazu Japanese Restaurant carries 40 prior inspections on record, the second-highest count this week. It received only one high-severity violation, for employees not reporting illness symptoms, but the depth of its inspection history means that violation is not a first-time oversight.

Jax Fish Shrimp and Chicken on Blanding Boulevard has 39 prior inspections on file. This week's sewage disposal citation, an intermediate violation, sits alongside a high-severity citation for having no employee health policy. A facility with that many inspections behind it should have a written health policy in place.

At the other end of the history spectrum, Yan Sushi on Bowden Road has only three prior inspections on record, making it one of the newest facilities in this week's roundup. It drew eight high-severity violations, including no allergen awareness, improperly stored chemicals, and food not cooked to required temperatures. Rio Restaurant and Lounge on Philips Highway has nine prior inspections and reached 10 high-severity violations this week, including no person in charge and no employee health policy.

Yan Sushi shares a strip address on Bowden Road with Bono's BBQ, which has 19 prior inspections and drew 10 high-severity violations of its own this week, including parasite destruction failures and unapproved food sourcing. Two restaurants at the same address, both with serious violations in the same inspection cycle, is a fact that remains unresolved in this week's records.
