FORT LAUDERDALE, FL. Inspectors visiting Baires Grill on East Las Olas Boulevard last week found food sourced from an unapproved or unknown supplier, a violation that means the restaurant's ingredients bypassed federal safety inspections entirely, with no traceability if a customer gets sick. That citation was one of five high-severity violations recorded at the Las Olas restaurant during the week of July 13, 2026, a stretch in which state inspectors flagged 15 Fort Lauderdale facilities for serious food safety failures.

Two restaurants tied for the most serious week: El Portal Maya Restaurant on West Commercial Boulevard and Florida Bar and Grill on Seabreeze Boulevard each drew seven high-severity citations. At El Portal Maya, inspectors found no person in charge present or performing duties, employees not reporting illness symptoms, improper handwashing technique, inadequate shellfish traceability records, improperly cleaned food contact surfaces, food not cooked to required minimum temperatures, and no consumer advisory for raw or undercooked items. The restaurant also drew an intermediate citation for improper sewage or wastewater disposal.

Florida Bar and Grill's seven high-severity violations covered nearly the same ground: no employee health policy, improper handwashing, inadequate shellfish records, improperly cleaned food contact surfaces, undercooking, and no consumer advisory. The seventh violation there was time as a public health control not properly used, meaning food was left in the temperature danger zone longer than permitted without documentation.

El Fogon Dreams on Davie Boulevard drew four high-severity violations, including food not cooked to required minimum temperatures and no employee health policy. Inspectors also cited the restaurant for improper sewage disposal, a violation that carries contamination risk far beyond a single prep station.

At Baires Grill, beyond the unapproved food source citation, inspectors found employees not reporting illness symptoms, improper handwashing technique, food not cooked to required minimum temperatures, and no consumer advisory. Five high-severity violations at a restaurant that has been inspected 18 times.

Handwashing, Shellfish, and Chemicals

Improper handwashing technique was the single most common high-severity violation of the week, appearing at eight of the fifteen flagged facilities: El Portal Maya, Florida Bar and Grill, Mangal Fort Lauderdale on West Broward Boulevard, Baires Grill, Okinawa Teriyaki and Sushi Express on West Broward Boulevard, Big Louie's Pizzeria on East Sunrise Boulevard, Red Door on East Las Olas Boulevard, Miami Subs on West Commercial Boulevard, and Epazote Mexican Restaurant on North Federal Highway.

Shellfish traceability failures appeared at four facilities: El Portal Maya, Florida Bar and Grill, Mangal Fort Lauderdale, Okinawa Teriyaki and Sushi Express, and Getsemani International Cuisine on North Federal Highway. Without adequate shellfish identification records, inspectors and health investigators cannot trace oysters, clams, or mussels back to their harvest source if a customer reports illness.

Improperly stored or mislabeled toxic chemicals were cited at three facilities: Okinawa Teriyaki and Sushi Express, Big Louie's Pizzeria, Red Door, and Miami Subs. Chemicals stored near food or in unmarked containers can cause acute poisoning without any visible sign of contamination.

Quarterdeck Seafood Bar and Neighborhood Grill on SE 17th Street drew one high-severity violation, for employees not reporting illness symptoms, along with a citation for improperly cleaned multi-use utensils.

Kaluz Restaurant on East Commercial Boulevard received a single high-severity citation: no person in charge present or performing duties.

Galway Shawl on East Commercial Boulevard and Tee-Jay Thai Sushi Restaurant on North Federal Highway drew no high-severity violations, with intermediate citations only for ventilation and sewage issues.

What These Violations Mean

The illness-reporting failures documented this week at El Portal Maya, Quarterdeck, Mangal, Baires Grill, Okinawa Teriyaki, Red Door, and Epazote represent a direct transmission risk. Norovirus, which causes the majority of foodborne illness outbreaks in the United States, can be transmitted by a single infected food worker handling ready-to-eat food. When no written health policy exists, as inspectors found at El Fogon Dreams, Florida Bar and Grill, and Getsemani International Cuisine, there is no formal mechanism to keep a sick employee out of the kitchen.

The undercooking violations at El Portal Maya, Florida Bar and Grill, El Fogon Dreams, Mangal, Baires Grill, and Big Louie's Pizzeria are not paperwork failures. Salmonella in poultry survives below 165 degrees Fahrenheit. A burger or piece of fish pulled from the heat too soon can carry live pathogens directly to a customer's plate. These violations are among the leading causes of confirmed foodborne illness cases nationally.

The unapproved food source citation at Baires Grill carries a specific additional risk: there is no chain of custody. If a customer becomes ill after eating there, investigators would have no supplier records to trace the ingredient back through the distribution system. USDA and FDA inspections exist precisely to catch contamination before food reaches a restaurant kitchen.

The shellfish traceability failures at five facilities this week compound that problem for a specific category of food. Oysters and clams are commonly eaten raw or lightly cooked, and they filter large volumes of water, concentrating pathogens like Vibrio and norovirus. Without harvest location tags and dealer records, a shellfish-related illness outbreak becomes nearly impossible to investigate or contain.

The Longer Record

Several of this week's worst performers have long inspection histories with no apparent resolution of recurring problems. El Fogon Dreams has 36 prior inspections on record, the most of any facility in this week's data, and still drew four high-severity violations including undercooking and no employee health policy. Florida Bar and Grill has 28 prior inspections and seven high-severity citations this week. Big Louie's Pizzeria has 33 prior inspections and five high-severity violations, including undercooking and improperly stored chemicals.

Kaluz Restaurant, with 30 prior inspections, drew a high-severity citation for no person in charge. Galway Shawl has 29 prior inspections and received two intermediate violations. Tee-Jay Thai Sushi Restaurant, also with 28 inspections on record, drew only one intermediate citation, the lightest outcome of any facility this week.

The newer locations in this week's data are not faring better for their relative inexperience. Getsemani International Cuisine has only 10 prior inspections on record and already carries four high-severity violations, including no employee health policy and time-as-public-health-control misuse. Mangal Fort Lauderdale, with 15 prior inspections, drew five high-severity violations including shellfish traceability failures and food not cooked to required temperatures.

El Portal Maya, with 26 prior inspections, drew the most violations of any facility this week: seven high-severity citations, including the absence of a responsible person in charge and improper sewage disposal. Inspectors have visited that restaurant on West Commercial Boulevard more than two dozen times. The sewage violation was still on the books.
