PALM HARBOR, FL. Toxic substances were improperly identified, stored, or used at Bar 23Fifty on Curlew Road when state inspectors arrived on September 25, and no one in a position of authority was present or performing their duties to stop it.
That combination, chemical hazards and absent management, headlined a six-count list of high-severity violations that inspectors documented at the Palm Harbor bar. The facility was not emergency-closed.
What Inspectors Found
Bar 23Fifty: September 25, 2026 Violations by Severity
The toxic substances citation is among the most immediately dangerous a food or beverage establishment can receive. Improper storage or labeling of chemicals near food preparation areas creates a direct route for contamination of drinks, glassware, or surfaces that customers contact.
Inspectors also cited the bar for food contact surfaces that were not properly cleaned or sanitized. Every glass, shaker, or surface that touched a customer's drink is a potential transfer point when sanitation breaks down.
The no-consumer-advisory violation applied to raw or undercooked foods on the menu. Customers who are pregnant, elderly, immunocompromised, or otherwise vulnerable have no way to make an informed choice about what they are ordering if that disclosure is absent.
Two intermediate violations rounded out the report: improper sewage or wastewater disposal and inadequate ventilation and lighting. Neither is minor. Sewage disposal failures introduce the risk of fecal contamination spreading through the facility. Poor ventilation allows grease vapor and airborne contaminants to accumulate in a space where food and drink are prepared.
What These Violations Mean
The absent-manager violation is not a paperwork problem. CDC data cited in the inspection record indicates establishments without active managerial control accumulate critical violations at three times the rate of those with engaged supervision. Every other violation on this list, the chemical storage, the unsanitized surfaces, the handwashing failures, is exactly the kind of lapse that on-site management is supposed to prevent and correct in real time.
The employee health policy violation compounds that risk. Without a written policy requiring sick workers to stay home, a single employee with norovirus can transmit the illness to dozens of customers before anyone intervenes. Norovirus accounts for an estimated 20 million cases of acute illness in the United States annually, and bars and restaurants are among its most efficient transmission environments.
Improper handwashing technique is a distinct citation from simply not washing hands. It means employees attempted to wash their hands and still left pathogens behind. Studies show that even a single missed step, not scrubbing long enough, skipping between fingers, not rinsing properly, can leave enough bacteria or viral particles to contaminate the next surface touched.
The sewage disposal citation carries its own specific hazard. Raw sewage contains E. coli, hepatitis A, and a range of other pathogens. When disposal is improper, contamination does not stay contained to a single drain or pipe.
The Longer Record
The September 25 inspection did not happen in isolation. Bar 23Fifty has 29 inspections on record and 250 total violations accumulated across that history.
The most recent prior inspections tell a consistent story. On October 6, 2025, inspectors cited the bar for six high-severity and five intermediate violations, a count nearly identical to the September 2026 report. Two follow-up inspections on October 21 and October 22 of that same month each found additional high-severity violations, suggesting the corrections from the October 6 visit were incomplete or short-lived.
The bar was emergency-closed on May 9, 2025, after inspectors documented rodent, roach, and fly activity. It reopened the same day. Two separate inspection entries from that same date each carried high-severity violations, meaning the facility was cited multiple times on the day it was shut down and allowed back open.
The 2024 inspection record shows the same pattern continuing further back. A visit on October 23, 2024, found three high-severity violations. A follow-up two days later found two more.
Still Open
Six high-severity violations, including improperly stored toxic substances, unsanitized food contact surfaces, no employee health policy, and no person in charge, are documented at Bar 23Fifty as of September 25, 2026.
The bar was not emergency-closed.
It has 250 violations on record across 29 inspections, one prior emergency closure for rodent, roach, and fly activity, and a recent inspection history that shows high-severity citations appearing in nearly every visit going back at least two years.
It remained open.
