Establishment did not registered as a food facility with the Food and Drug Administration (FDA). This is a repeat violation from the reports of previous inspections conducted on 08/21/2023 and on 10/08/2021. Corrected onsite, operator registered with the FDA as a food facility during the course of the inspection.

Specifically, observed that establishment did not registered as a food facility with the Food and Drug Administration (FDA). o 117.20(b) Citation Description: Shatter-resistant light bulbs, lighting fixtures, skylights, or glass suspended over exposed food in any step of preparation not provided. 21 CFR 117.20(b)(5) Observation: Specifically, observed multiple light bulbs without shield protection in the staging area in front of walk-in cooler 1. This is a repeat violation from the report from the previous inspection conducted on 10/08/2021. Operator already installed LED lighting in area, and stated that the light bulbs without shield protection will be removed in the coming days. o 117.20(b) Citation Description: Fans and other air-blowing equipment not located or operated in a manner that minimizes the potential for allergen cross-contact and contaminating food, food-contact surfaces, or food-packaging materials. 21 CFR 117.20(b)(6) Observation: Specifically, in walk-in cooler 2 observed fans grills of the southernmost evaporator soiled with gray substance. o 117.20(b) Citation Description: Sufficient space not provided for placement of equipment or storage of materials as necessary for the maintenance of sanitary operations and the production of safe food. 21 CFR 117.20(b)(1) Observation: Specifically, observed in a covered area in the back of the facility pallets of empty boxes stored too close to the wall and too close between the rows of said pallets to allow for proper cleaning and inspection of said area. o 117.35(a) Citation Description: Buildings, fixtures and other physical facilities of the plant not maintained in a clean and sanitary condition or kept in repair adequate to prevent food from becoming adulterated. 21 CFR 117.35(a) Observation: Specifically, observed plastic strip curtain leading to walk-in cooler 1 encrusted with mold-like substance.
