Observation: SPECIFICALLY, OBSERVED ESTABLISHMENT OPERATING WITHOUT A VALID 2022 FOODPERMIT. PERSON IN CHARGE STATED THAT THEY STARTED OPERATING IN JANUARY, 2021. ANAPPLICATION HAS BEEN SUBMITTED. Citation Description: Shatter-resistant light bulbs, lighting fixtures, skylights, or glass suspended over exposed food in any step of preparation not provided. 21 CFR 117.20(b)(5) Observation: SPECIFICALLY, OBSERVED UNSHIELDED LIGHT BULBS IN ALL 5 WALK-IN COOLERS AND INTHE RETAIL AREA OF THE FACILITY. Citation Description: Fans and other air-blowing equipment not located or operated in a manner that minimizes the potential for allergen cross-contact and contaminating food, food-contact surfaces, or food-packaging materials. 21 CFR 117.20(b)(6) Observation: SPECIFICALLY, OBSERVED REFRIGERANT EVAPORATOR FAN GRILLS SOILED WITH DARK SUBSTANCE IN THE EASTERNMOST WALK-IN COOLER IN THE CENTER OF THE FACILITY. Citation Description: Buildings, fixtures and other physical facilities of the plant not maintained in a clean and sanitary condition or kept in repair adequate to prevent food from becoming adulterated. 21 CFR 117.35(a) Observation: SPECIFICALLY, OBSERVED PLASTIC CURTAINS LEADING TO EASTERNMOST WALK-IN COOLER IN THE CENTER OF THE FACILITY ENCRUSTED WITH DARK MOLD-LIKE SUBSTANCE. FURTHERMORE, OBSERVED CEILING ENCRUSTED WITH MOLD-LIKE SUBSTANCE IN SECTIONS OF SAID WALK-IN COOLER AND IN THE WESTERNMOST WALK-IN COOLER IN THE CENTER OF THE FACILITY. BOTH WALK-IN COOLERS ARE USED TO STORE BOXES OF EXPOSED PRODUCE cos 117.35(b)(2) Citation Description: Toxic cleaning compounds, sanitizing agents, or pesticide chemicals not properly identified, held or stored in a manner that protects against contamination of food, food-contact surfaces, or food-packaging materials. 21 CFR 117.35(b)(2) Observation: SPECIFICALLY, OBSERVED IN THE RETAIL AREA MULTIPLE BOTTLES OF TOXIC CLEANING COMPOUNDS STORED IN A SHELF DIRECTLY ABOVE SHELVES WITH BOXES OF EXPOSED PRODUCE. 117.93 Citation Description: Storage and transportation of food,including ingredients,not under conditions that will protect against allergen cross-contact and against biological, chemical (including radiological), and physical contamination of food, as well as against deterioration of the food and the container. 21 CFR 117.93 Observation: SPECIFICALLY, OBSERVED IN THE SOUTHERNMOST REACH-IN COOLER IN THE RETAIL AREA MULTIPLE STYROFOAM CARTONS OF RAW EGGS STORED IN A SHELF DIRECTLY ABOVE BOTTLES OF SOFT DRINKS.
