Comments: All food service other than prepackaged foods, was placed under stop use order based on a consumer complaint and corresponding water test result letter provided to FDACS by Florida Department of Health stating that the well water supplying this establishment does not meet State and Federal Drinking water standards. During this visit, it was observed that the water supply for this establishment had been switched to different well that also supplies the adjacent restaurant (not currently operating ). This was apparent by PVC piping laying above ground. The food establishment must provide proof of current approved water supply by getting the “Interagency Agency Coordination of Regulated Establishments” form (provided during this visit) signed by the agency having jurisdiction over the well water supplying this establishment. Interagency Form provided during the visit. Stop Use Order issued. Food establishment can visit https://foodpermit.fdacs.gov or contact the Business Center at (850) 245-5520 when corrections have been made and/or when required documents have been obtained to request food safety inspector, Kenneth Davidson at [email protected], for a written release of equipment. Failure to comply with the request for written release may or will result in Administrative Action.
