Comments: Citation Description and Observation Citation Description: Food packaged on site not labeled with: the common name of the food or an adequate descriptive identity statement; if made from two or more ingredients, a list of ingredients and sub-ingredients in descending order of predominance by weight; an accurate declaration of the net quantity of contents; the name and place of business of the manufacturer, packer, or distributor; or nutritional labeling as specified in 21 CFR 101 and 9 CFR 317 Subpart B. 3-602.11(B)(1)-(4) and (6) Observation: Retail - Reach in Freezers - Observed Squid, Beef Bone, Octopus and lamb not properly labeled. Reach in Produce Cooler - Observed cut pumpkin not properly labeled. COS, All items were removed from sales floor and properly labeled. Citation Description: Outer opening not protected against the entry of insects and rodents by filling or closing holes and other gaps along floors, walls, and ceilings; closed, tight-fitting windows; and solid, self-closing, tight-fitting doors. Windows or doors kept open for ventilation or other purposes not protected against the entry of insects and rodents by 16 mesh to 1 inch screens; properly designed and installed air curtains to control flying insects; or other effective means. 6-202.15 Observation: Retail - Front Entrance Doors - Observed approximately 1/2" gap at base and center seam. Citation Description: Test kit or other device that accurately measures the concentration in MG/L of sanitizing solution not provided. 4-302.14 Pf Observation: Establishment uses chlorine for sanitizer. Chlorine sanitizer test strips unavailable. COS, Test strips made available by end of inspection. Citation Description: Warewashing equipment; sinks, basins or other receptacles used for washing or rinsing raw food or laundering wiping cloths; or drainboards or equipment used to substitute for drainboards not cleaned before use, throughout the day at a frequency necessary to prevent recontamination of equipment and utensils and ensure the equipment performs its intended function, and at least every 24 hours when used. 4-501.14 Observation: Back Room - Ware Wash Sink(s) - Observed unknown residue present. COS, Both sets of sinks were washed, rinsed and sanitized. Citation Description: Plumbing system not installed to preclude backflow of a solid, liquid, or gas contaminant into the water supply system at each point of use, including on a hose bibb, by providing an air gap or installation of an approved backflow prevention device. 5-203.14 Pf Observation: Outside - Rear of Building - Observed back flow device to be absent on threaded faucet. Citation Description: Plumbing system not maintained in good repair. 5-205.15(B) Observation: Back Room - North Side Wall - Ware Wash Sink - Observed slight leak from drain, not affecting operations. Owner notified maintenance personnel. Citation Description: Plumbing fixtures including but not limited to handwashing sinks, toilets and urinals not cleaned as often as necessary to keep them clean. 6-501.18 Observation: Back Room - Both Hand Wash Sinks - Observed unknown residue present. COS. Both sinks were washed, rinsed and sanitized. Citation Description: Toilet room located inside the food establishment not completely enclosed or not provided with a tight-fitting self-closing door. 6-202.14 Observation: Back Room - Both Restrooms - Door not self closing. Citation Description: Drains in receptacles or waste handling units for refuse, recyclables, and returnables do not have drain plugs in place. 5-501.114 Observation: Outside - Dumpster - Observed drain cap missing. Citation Description: Premises not free of litter and items that are unnecessary to the operation or maintenance of the establishment such as equipment that is nonfunctional or no longer used. 6-501.114 Observation: Outside - Rear and West Side of Building - Observed unused pallets, unnecessary cages, drums and clutter present.
