FERNANDINA BEACH, FL. State inspectors visiting a Nassau County Taco Bell on July 30 found food sourced from unapproved suppliers, toxic chemicals stored improperly near food, and no evidence that staff could identify a customer's food allergy, and then left the restaurant open.
Taco Bell #042873 at 1858 S 8th Street accumulated six high-severity and three intermediate violations during the inspection, a tally that placed it among the more serious non-closure cases in the region this year. The facility was not emergency-closed.
What Inspectors Found
The most direct threat to customers came from two violations that operated in combination. Inspectors cited the restaurant for serving food from unapproved or unknown sources, and separately for failing to demonstrate any allergen awareness among staff.
Food from unapproved suppliers has no USDA or FDA inspection trail. If a customer gets sick, there is no chain of custody to trace the contamination back to its origin.
The allergen citation compounds that risk. Staff who cannot identify allergens in the food they are preparing cannot warn a customer with a peanut, wheat, or dairy allergy before a reaction occurs. Food allergies send roughly 30,000 Americans to emergency rooms each year.
Toxic chemicals stored improperly near food rounded out the most serious findings. Inspectors also cited the restaurant for food contact surfaces that were not properly cleaned or sanitized, a direct pathway for bacterial transfer between raw ingredients and finished food.
The shell stock identification citation, which concerns traceability records for shellfish like oysters and clams, was an unusual finding for a fast-food taco chain. Inspectors also noted improper sewage or wastewater disposal, single-use items being reused, and inadequate ventilation and lighting.
What These Violations Mean
The unapproved food source violation is not a paperwork problem. When food enters a restaurant without passing through a licensed, inspected supplier, there is no verification that it was handled, stored, or transported safely at any point before it reached the kitchen. Pathogens including Listeria and Salmonella can survive in improperly handled supply chains without any visible sign of spoilage.
The allergen awareness citation is acutely dangerous for a specific group of customers. Pregnant women, elderly diners, people with compromised immune systems, and anyone with a documented food allergy depend on restaurant staff to know what is in the food. When staff cannot demonstrate that awareness, those customers are making ordering decisions without accurate information.
Improperly cleaned food contact surfaces create a secondary contamination route. Cutting boards, prep tables, and utensils that carry residue from one food item to the next can transfer bacteria to food that will be served without further cooking.
The sewage disposal violation adds a separate vector entirely. Improper wastewater handling introduces the risk of fecal contamination in a facility where food is being prepared and plated. That is not a risk that can be managed by cooking temperature alone.
The Longer Record
The July 30 inspection was not an anomaly. Records show this Taco Bell has been inspected 20 times, accumulating 126 total violations across its history.
The most direct precedent came seven months earlier. An inspection on December 23, 2025 produced the same violation count as the July 2026 visit: six high-severity and two intermediate citations. That visit apparently produced no lasting correction, at least not one that held through the summer.
Before that, a clean August 2025 inspection, zero high-severity violations, suggested the problems were controllable. They returned in force.
The pattern across eight documented inspections shows high-severity citations in every single visit except one. The August 2025 inspection is the outlier. Every other inspection on record produced at least one high-severity violation, and the two most recent visits produced six apiece.
The facility has never been emergency-closed. Not once across 20 inspections and 126 violations.
