ORLANDO, FL. Catrinas Mexican Fusion on North Semoran Boulevard drew 10 high-severity violations during the week of September 25, including food from an unapproved or unknown source, inadequate shellfish traceability records, and no consumer advisory for raw or undercooked items on the menu, according to state inspection records.

That tied for the highest single-week high-severity count among 15 Orlando-area restaurants cited during the same inspection period.

House of Pho Orlando on South John Young Parkway matched Catrinas with 10 high-severity violations of its own. Inspectors cited the restaurant for failing to cook food to the required minimum temperature, inadequate shellfish traceability records, no employee health policy, employees not reporting illness symptoms, and improper handwashing technique, among others.

YH Seafood Clubhouse on Turkey Lake Road collected nine high-severity violations, including food from an unapproved or unknown source, food not cooked to the required minimum temperature, inadequate shellfish identification records, and improperly stored toxic chemicals near food.

Kyoto Sushi and Grill on East Colonial Drive also reached nine high-severity violations. Among them: parasite destruction procedures not followed, no person in charge present or performing duties, and no consumer advisory for raw or undercooked items, a notable gap for a sushi operation.

El Palacio Buffet on South Orange Blossom Trail drew eight high-severity violations, including two separate chemical storage violations: toxic chemicals improperly stored or labeled, and toxic substances improperly identified, stored, or used. Inspectors also cited the buffet for no person in charge and no employee health policy.

Mordisco on International Drive reached eight high-severity violations, including food from an unapproved source, food not cooked to the required minimum temperature, no allergen awareness demonstrated, and inadequate shellfish traceability records.

Amalfi on Avalon Lake Drive also drew eight high-severity violations. The citation list included no allergen awareness demonstrated, two separate chemical storage violations, and inadequate shellfish identification records, in addition to employees not reporting illness symptoms.

Allure Orlando on West Pine Street accumulated seven high-severity violations, including food from an unapproved source, food not cooked to the required minimum temperature, and improper sewage or wastewater disposal noted at the intermediate level. The person in charge was not present or not performing duties.

Kingdom Sushi on International Drive was cited for seven high-severity violations, including both inadequate handwashing and improper handwashing technique cited as separate violations, parasite destruction procedures not followed, and food not cooked to the required minimum temperature.

Tomasinos Pizza on East Colonial Drive drew six high-severity violations. Among them: food from an unapproved or unknown source, parasite destruction procedures not followed, and no allergen awareness demonstrated. Inspectors also cited the restaurant for single-use items being improperly reused.

Kalalou Caraibbean Bar and Grill on South John Young Parkway collected six high-severity violations, including food from an unapproved source, food not cooked to the required minimum temperature, and inadequate handwashing. An intermediate citation for improper sewage or wastewater disposal was also on the report.

Elixir Bar Room and Hash House on West Washington Street drew three high-severity violations, including required procedures for specialized processes not followed, improperly identified or stored toxic substances, and improperly cleaned food contact surfaces. Single-use items were also cited as improperly reused.

Crowne Plaza Downtown Orlando on West Colonial Drive received three high-severity violations: food from an unapproved or unknown source, improperly stored toxic chemicals, and no consumer advisory for raw or undercooked foods.

Pho Vinh on North Primrose Drive and Relax Grill at Lake Eola on Eola Parkway each drew two high-severity violations, the lowest counts among the 15 facilities cited this week. Pho Vinh was cited for improper handwashing technique and improperly cleaned food contact surfaces, along with an intermediate violation for improper sewage disposal. Relax Grill was cited for improperly cleaned food contact surfaces and improperly stored or labeled toxic chemicals, also with a sewage disposal intermediate.

What These Violations Mean

The food-sourcing citations at Catrinas Mexican Fusion, YH Seafood Clubhouse, Mordisco, Allure Orlando, Tomasinos Pizza, Kalalou Caraibbean Bar and Grill, and Crowne Plaza Downtown Orlando carry a specific practical consequence: if a customer gets sick, investigators have no chain of custody to trace. Food from unapproved sources bypasses USDA and FDA inspection systems entirely, meaning contamination with Listeria, Salmonella, or E. coli goes undetected before the food reaches the kitchen.

The shellfish traceability failures at Catrinas, House of Pho, YH Seafood Clubhouse, Amalfi, and Mordisco compound that risk. Oysters, clams, and mussels are commonly eaten raw or barely cooked, and shellfish are among the most frequent vehicles for Vibrio and Norovirus outbreaks. Without the required identification tags and harvest records, there is no way to link a sick customer to a contaminated batch.

That any of those four facilities are still drawing high-severity citations for foundational issues like handwashing and food contact surface sanitation, after dozens of documented inspection visits, is a fact the record makes plain.

House of Pho has 34 prior inspections, as does Tomasinos Pizza. Both returned 10 and 6 high-severity violations respectively this week. Kyoto Sushi and Grill, at 20 prior inspections, and Catrinas Mexican Fusion, at just 19, are newer to the inspection record but have accumulated serious violation totals quickly.

Allure Orlando stands out in a different direction: only 7 prior inspections on record, yet this week's report included food from an unapproved source, improper sewage disposal, and no person in charge. YH Seafood Clubhouse, with 17 inspections, drew nine high-severity violations including unapproved food sourcing and undercooked food. Both are relatively early in their inspection histories, and both are already accumulating the categories of violations that regulators associate with systemic rather than incidental failures.
